The human body forms its structure through skin and hair, blood vessels, sinews, muscles, and bones, while internally governed by the Liver, Heart, Spleen, Lung, and Kidney. If one indulges emotions recklessly, lacks moderation in joy and anger, or combines overexertion with excessive rest, it leads to depletion of essence and blood in the five zang organs, disruption of Ying and Wei (nutritive and defensive qi) regulation, manifesting as chills and fever. This causes the skin, blood, sinews, bones, and muscles to become flaccid and weak, unable to move, thus resulting in Flaccidity Syndrome (Wei Zheng). Its presentation resembles Soft Wind (Rou Feng) and Hong Kong Foot (Gouty Arthritis/Chronic Arthritis), but must be distinguished by pulse, symptoms, and etiology; they should not be confused. Soft Wind and Hong Kong Foot are external causes; Flaccidity Syndrome belongs to insufficiency of qi within the five zang organs. Careful examination is required.
When the patient has Lung Heat, the skin is weak and thin, the feet are flaccid, the complexion is white, and hair is ruined, it is named Skin Flaccidity (Pi Wei). This is caused by Lung Heat leading to withering of lung leaves. The Lung is the leader of the five zang organs. If there is loss or unfulfilled desires, Lung Ming (lung resonance/sound) occurs; when Lung Ming arises, the lung leaves wither. The treatise states: When the five zang organs suffer from Lung Heat and withering, Flaccidity develops.
When the patient has heat below the heart, the knee and ankle joints feel as if broken and disconnected, the calf sinews are relaxed and slack, unable to support the body, the complexion is red, and collateral vessels are full and overflowing, it is named Vessel Flaccidity (Mai Wei). This is due to excessive sorrow causing internal movement of Yang Qi and frequent bleeding from urination. Thus, the *Ben Bing Lun* states: When the large channels are empty, Muscle Bi syndrome develops, transmitting to Vessel Flaccidity.
When the patient has Liver Heat, the mouth is bitter, sinews are dry, tendons are tight and contracted, the complexion is dark green, and nails are withered, it is named Sinew Flaccidity (Jin Wei). This is due to endless thinking, unfulfilled wishes, mental wandering, excessive sexual intercourse, leading to relaxation of the Zong Jin (convergence of sinews) and occurrence of Bai Yin (white discharge). Thus, the *Xia Jing* states: Sinew Flaccidity originates in the Liver, caused by sexual activity.
When the patient has Spleen Heat, the stomach is dry with thirst, and muscles are insensible. The complexion is yellow, and muscles twitch, it is named Flesh Flaccidity (Rou Wei). This results from gradual exposure to damp ground, working with water, dwelling in low and marshy areas, where dampness infiltrates, causing obstruction and insensibility. Thus, the *Xia Jing* states: Flesh Flaccidity is acquired from damp places.
When the patient has Kidney Heat, the waist and spine cannot lift, bones are withered, and marrow is diminished, the complexion is black, and teeth are decayed, it is named Bone Flaccidity (Gu Wei). This is due to long journeys and fatigue, encountering intense heat and thirst, leading to internal deficiency of Yang Qi. Heat lodges in the Kidney, causing Water to fail to control Fire, resulting in withered bones and deficient marrow. Thus, the *Xia Jing* states: Bone Flaccidity originates from intense heat.
