The patient presents with dysentery (Zhi Xia). It is well known that red blood indicates heat, while white pus indicates cold. However, few realize that pure clear blood discharge is caused by wind, and stool resembling bean soup juice is caused by dampness. The six qi pathogenic factors do not initially differ in severity; summer-heat and heat constitute one qi, dryness and dampness share the same origin, and they condense into four seasonal influences. Therefore, cold, heat, wind, and dampness cannot be neglected. Ancient formulas state: after wind lingers in the skin and interstices, it invades the stomach and intestines when deficiency exists, leading to either dark blood or fresh blood discharge, with unceasing diarrhea; damp-toxin descending like bean soup juice are all explicit records of external causes. Ancient formulas describe five types of diarrhea, caused by stagnation and knotting of zang-fu qi, where pus and blood are discharged according to their manifestation, appearing in different colors of green, yellow, red, white, and black; this constitutes internal causes. Furthermore, consuming hot or cold wines, vinegars, and sauces, indulging in desires, excessive sexual activity, or improper work and rest, leads to damage of essence and blood, causing dryness and astringency of the stomach and intestines. Long-term accumulation of cold and heat eventually forms toxic dysentery; these belong to neither internal nor external causes. In treatment, first deduce the annual climate movement to balance external causes, observe stagnation to regulate internal causes, and examine the specific injury to treat non-internal/non-external causes, ensuring clarity to avoid indiscriminate medication.
For treating heat dysentery with bloody stool persisting for months without healing:
Baitouweng (Radix Pulsatillae) (2 liang), Huanglian (Rhizoma Coptidis), Baipi (Cortex Phellodendri), Qinpi (Cortex Fraxini) (each 2 liang).
Grind the above into a coarse powder. Take 4 qian per dose, decoct in 1.5 sheng of water until reduced to 0.7 sheng, strain, and drink.
For treating cold dysentery with abdominal pain and discharge of white matter resembling fish brain:
Chishizhi (Halisacum) (appropriate amount), Ganjiang (Sicci Zingiberis Rhizoma Praeparata) (equal parts).
Grind the above into powder, mix with paste made from steamed buns to form pills. Take 30-50 pills with rice water.
For treating wind-cold invading the stomach and intestines due to deficiency, leading to indigestion of food and fluids, profuse watery diarrhea, empty fullness in the abdomen and hypochondrium, and borborygmus with pain; as well as damp-toxin in the stomach and intestines, causing stool like bean juice or dark blood discharge, day and night without limit, this formula is suitable. (See the section on Wind Dysentery with Blood Stool under Loss of Blood.)
For treating wind dysentery with pure clear blood discharge:
Xingren (Semen Armeniacae Amarum) (7 grains, remove skin and tips), Ruomuchuang (unknown herb, likely a local name or variant, context suggests bark or root) (size of a palm), Wumei (Fructus Mume) (2 pieces), Caoguo (Fructus Tsaoko) (1 piece), Suan Shiliu Pi (Cortex Granati Acidi) (half a piece), Qingpi (Cortex Citri Reticulatae Viridis) (2 pieces), Gancao (Radix Glycyrrhizae) (2 cun).
Grind the above into a coarse powder as one dose. Decoct in 2 bowls of water with 3 slices of Shengjiang (Rhizoma Zingiberis Recens) until reduced to 0.7 bowl. Expose to the starry night sky, and take on an empty stomach the next morning.
