(Note: Hypochondriac Pain)
The disease of Heartache (Xintong), as recorded in medical classics, comprises nine types: first, Insect Heartache; second, Infectious Heartache; third, Wind Heartache; fourth, Palpitation Heartache; fifth, Food Stagnation Heartache; sixth, Fluid Retention Heartache; seventh, Cold Heartache; eighth, Heat Heartache; ninth, Wandering Heartache. Although their names differ, the causes are all due to external contraction of the Six Excesses (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and internal injury by the Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright), or consumption of cold, raw fruits and foods. This allows pathogenic factors to contend with healthy Qi, leading to a clash between pathogen and Zhengqi, obstruction of the Qi mechanism, and stagnation in the Middle Jiao, thereby forming Heartache. The Heart is the master of the five Zang and six Fu organs; its regular meridians must not be injured. If injured, pain ensues. If the pain is severe, causing cyanosis of the hands and feet extending beyond the joints, it is named True Heartache (Zhenxintong). Patients with True Heartache who contract it in the morning will die by evening, and those who contract it in the evening will die by morning. If the pain comes and goes intermittently, appearing in paroxysms without being fatal, it is named Rebellious Heartache (Juxintong); this is merely due to pathogenic factors invading the branch collateral networks of the Heart. A tight pulse at the Cun Kou position, or an extremely rapid Heart pulse, both indicate Heartache. There are also cases where the pain is severe while the Heart pulse is deep and hidden. Wang Shuhe stated: "In Heart and Abdominal Pain, a deep and fine pulse indicates recovery; a floating, large, wiry, and long pulse indicates certain death." The treatment method should deduce the origin of the pathology and regulate it, so that the pain will cease.
Furthermore, it is discussed that Heartache includes True Heartache and Rebellious Heartache. As the master of the five Zang and six Fu organs, the Heart theoretically does not suffer from disease directly. If the pain is severe, accompanied by cyanosis and coldness of the hands and feet, it is named True Heartache; this signifies the departure of Shen (Spirit) and exhaustion of Qi, leading to death within half a day. If caused by injuries from the Six Excesses or Seven Emotions, resulting in rebellious Qi of the five Zang organs, with pain that fluctuates in intensity and appears in paroxysms without being fatal, it is named Rebellious Heartache; this is due to pathogenic factors invading the branch collateral networks of the Heart. Generally, pain due to excess syndromes should be treated with purgation, while pain due to cold syndromes should be treated with warming. For warming and promoting circulation, Que Tong San (Pain-Dispersing Powder) is the primary formula. For women suffering from sudden stabbing Heartache due to blood stasis, Li Ying San (Instant-Response Powder) is used for rapid efficacy. Other types of Heartache should be treated according to specific formulas based on careful differentiation, aiming for the restoration of balance.
This formula treats diseases caused by the imbalance of the Seven Emotions: Joy, Anger, Worry, Pensiveness, Sorrow, Fear, and Fright. Upon onset, there is unbearable stabbing pain in the heart and abdomen, occurring intermittently, with episodes so severe they feel like impending death. It is also suitable for pain caused by external contraction of Wind-Cold-Dampness.
