Section 3: Gynecology and Pediatrics
Beng Lou (Metrorrhagia and Metrostaxis)
I once treated a patient, Jiang XX, female, adult. She had suffered from metrorrhagia and metrostaxis for over two months. Initially, a physician observed her cold pain in the abdomen and prescribed Jiao Ai Si Wu Tang (Cinnamon Twig and Moxa Four Substances Decoction), but this exacerbated the bleeding. Subsequently, observing bitter taste in the mouth, dry mouth, and a yellow tongue coating, he diagnosed it as reckless bleeding due to blood heat and administered heat-clearing and fire-purging agents. This resulted in severe colic-like abdominal pain without cessation of bleeding. Reluctantly, she sought another doctor. The second doctor diagnosed "Spleen failing to control blood" and prescribed Gui Pi Tang (Restore the Spleen Decoction), which was ineffective. He then tried Sheng Yang Ju Jing Tang (Raise Yang and Raise Menses Decoction) with Zong Tan (Charred Palm Fruit Spine), also without effect. She came to me for treatment. Upon examination, her complexion was pale and lusterless, she appeared fatigued and weak, had almost lost her appetite, her menstrual blood was pale with clots, she experienced cold abdominal pain, bitter taste, yellow urine, cold limbs, a yellow tongue coating, and a deep, fine, and choppy pulse. Integrating symptoms and pulse, I diagnosed a syndrome of mixed cold and heat, deficiency and excess, specifically Spleen failing to control blood. Recalling Mr. Li Hanqing's statement: "Handling syndromes of mixed deficiency/excess and cold/heat is most difficult; the challenge lies in determining the proportion of cold, heat, deficiency, and excess, and distinguishing their quantities." He stated: "To measure the degree of cold and heat, one generally uses a comparative method of symptoms, tongue, and pulse. A pulse indicating heat suggests more heat than cold; a pulse indicating cold suggests more cold than heat; a pulse indicating deficiency suggests more deficiency; a pulse indicating excess suggests more excess." Following his principle, I prescribed Huang Tu Tang (Yellow Earth Decoction) with modifications. Prescription: Long Gu (Fossilized Earth) 30g (decocted separately), E Jiao (Donkey-hide Gelatin) 10g (melted), Huang Qin (Scutellaria Root) 9g, Sheng Di Huang (Rehmannia Root) 9g, Bai Zhu (Atractylodes Rhizome) 9g, Fu Zi (Aconite Lateral Root) 9g, Gan Cao (Licorice Root) 6g, Chao Ling Zhi (Stir-fried Liquidambar Resin) 6g. After two doses, the bleeding decreased significantly. After eight additional doses, bleeding stopped, appetite increased greatly, and abdominal pain disappeared.
Patient Li XX, female, adult. Suddenly experienced heavy menstrual bleeding with many clots. A physician initially treated her with Du Shen Tang (Single Ginseng Decoction) without effect, then used astringent methods, still without success. Another physician diagnosed it as caused by blood heat combined with blood stasis and urgently applied heat-clearing, blood-cooling, blood-activating, and hemostatic methods. After three doses, symptoms did not improve. Reluctantly, she went to a hospital gynecology department. Her hemoglobin was 5g%, she received 500ml of blood transfusion, and other Western medicines were used for three days, all without effect. She invited me for consultation. Examining her pulse and symptoms, her complexion was pale and lusterless, she had shortness of breath and faint breathing, abdominal distension and pain, a dry yellow tongue coating, constipation for several days, and a deep, slippery, and rapid pulse. Reflecting on the syndrome: pale complexion and faint breathing indicate deficiency, which should be tonified, yet previous physicians' tonifying attempts failed. Abdominal distension/pain, dry yellow coating, and constipation indicate excess, which should be purged, yet previous physicians' cooling attempts failed. I hesitated for a long time until I recalled Mr. Li Hanqing saying: "When the proportion of mixed deficiency and excess is hard to determine, seek guidance from the pulse." Considering this case, the pulse is deep, slippery, and rapid. A deep, slippery, and rapid pulse indicates excess and heat; thus, this syndrome is caused by excess heat. If it is excess heat, why did heat-clearing and blood-cooling fail? Because they cleared heat and cooled blood but did not unblock the bowels (Fu organs). The Yangming Channel is rich in Qi and Blood. When excess heat is blazing, it often manifests as vomiting, epistaxis, lower bleeding, or metrorrhagia. Therefore, unblocking the bowels is essential for effectiveness. Prescription: Sheng Di Huang (Rehmannia Root) 9g, Mu Dan Pi (Moutan Bark) 9g, Huang Qin (Scutellaria Root) 9g, Xiang Fu (Cyperus Rhizome) 9g, Mu Xiang (Vladimiria Root) 6g, Da Huang (Rhubarb Root) 4g. After one dose, menstrual bleeding stopped.
Insufficient Lactation
I once treated patient Zhang XX, female, adult. For one month postpartum, her milk secretion was sufficient for the infant. Recently, lactation suddenly decreased, causing the infant to cry incessantly. A physician prescribed galactagogues, but milk production decreased further. Her pulse was deep. Symptoms included chest fullness and irritability. The cause was identified as sudden anger. Integrating symptoms and pulse, I diagnosed Liver Qi Stagnation. I prescribed Xiao Yao San (Free and Easy Wanderer Powder) with Qing Pi (Citrus Aurantium Peel). After three doses, lactation returned to normal.
Postpartum Body Pain
I once treated patient Liu XX, female, 24 years old. For two months postpartum, she suffered from headache, body pain, and joint pain. A physician treated her with wind-dispelling, cold-scattering, and damp-draining agents, which worsened the pain. Extension and flexion of joints were unbearable. She felt fatigued and weak, with spontaneous sweating and night sweats. Besides these, she exhibited five-center heat (heat in palms, soles, and chest), pale complexion, thin white tongue coating, pale tongue body, and a deficient, large, slippery, and rapid pulse. Integrating symptoms and pulse, I diagnosed dual deficiency of Qi and Blood, with damp-heat failing to transform. Treatment should involve boosting Qi and nourishing Blood, drying dampness and clearing heat. I prescribed Qi Mai San Miao Tang (Four Ingredient Decoction for Qi and Pulse) with modifications. Prescription: Huang Qi (Astragalus Root) 15g, Dang Gui (Angelica Sinensis Root) 10g, Dang Shen (Codonopsis Root) 10g, Mai Dong (Ophiopogon Tuber) 10g, Wu Wei Zi (Schisandra Fruit) 10g, Shi Hu (Dendrobium Stem) 10g, Cang Zhu (Atractylodes Lancea Rhizome) 10g, Huang Bo (Phellodendron Bark) 10g, Mu Gua (Chaenomeles Fruit) 10g, Huai Niu Xi (Achyranthes Root) 9g. The previous physician asked why Fang Feng (Saposhnikovia Root), Qiang Huo (Notopterygium Rhizome), and Qin Jiao (Gentiana Macrophylla Root) were not added. I replied: Zhang Zhongjing stated, "Those who have lost blood should not be induced to sweat." Postpartum blood deficiency with external pathogenic invasion requires tonifying Zheng Qi (Upright Qi) to expel pathogens. Using wind-dispersing or drying herbs would injure the blood, worsening deficiency and increasing pain. After four doses, pain decreased. After thirty additional doses, pain disappeared.
Zhi XX, female, 35 years old. Three years postpartum, she developed body pain and aversion to wind. Physicians treated her with wind-dispelling and exterior-releasing drugs. Pain subsided after sweating but returned when sweating stopped. She took over 800 doses, yet pain and aversion to wind intensified. Examination revealed that even in summer, she kept windows and doors closed, covered herself with mosquito nets and cloth bundles. Removing these revealed a strong odor of sweat. Appetite was poor, eating only about two bowls of noodles daily, with a bland taste. She feared wind severely but felt irritability at night. Tongue body was pale, coating white, pulse string-taut and fine. Integrating symptoms and pulse, I diagnosed insufficiency of Qi, Blood, Yin, and Yang. I prescribed Shi Si Wei Jian Zhong Tang (Fourteen Ingredient Middle-Building Decoction). Prescription: Huang Qi (Astragalus Root) 15g, Rou Gui (Cinnamon Bark) 10g, Dang Gui (Angelica Sinensis Root) 10g, Chuan Xiong (Ligusticum Rhizome) 10g, Shu Di Huang (Prepared Rehmannia Root) 10g, Bai Shao (White Peony Root) 10g, Dang Shen (Codonopsis Root) 10g, Bai Zhu (Atractylodes Rhizome) 10g, Fu Ling (Poria) 10g, Gan Cao (Licorice Root) 10g, Ban Xia (Pinellia Tuber) 10g, Da Yun (Cistanche Stem) 15g, Fu Zi (Aconite Lateral Root) 10g, Sheng Jiang (Fresh Ginger) 3 slices, Da Zao (Jujube Fruit) 5 pieces. After ten doses, all symptoms reduced. After forty additional doses, she recovered.
Fever
In 1963, during travel, I encountered an infant, about one year old, suffering from high fever and convulsions, with eyes rolling back and opisthotonos. Passengers were helpless. The parents begged me to help. Having no tools, I hesitated, but under pressure, I reluctantly used Finger Pointing Therapy (acupressure). I first pressed Feng Men (BL12) and Da Zhui (GV14) acupoints with my thumb. Then, using thumb and index finger, I kneaded muscles of the neck and from Que Pen (ST12) to Feng Men (BL12) repeatedly. Soon, the infant cried loudly, sweated profusely, fever subsided, and the illness resolved.
Zhu XX, male, 5 months old. High fever persisted for a day and night, temperature 40.1°C, with occasional confusion and delirium. Intravenous fluids, penicillin, and streptomycin were ineffective. Tongue coating was slightly yellow, pulse slippery and rapid, no bowel movement for two days, palms hotter than backs. Diagnosed as Exterior Heat with Interior Excess. I prescribed Sheng Jiang San (Ascending-Descending Powder) with modifications. Chan Tui (Cicada Slough) 6g, Jiang Can (Bombyx Batryticatus) 6g, Pian Jiang Huang (Turmeric) 6g, Da Huang (Rhubarb Root) 3g, Bo He (Mint) 4g. After 3 hours, fever slightly decreased to 38.9°C. After 4 hours, slight sweating occurred, temperature dropped to 38.1°C, consciousness cleared. After 5 hours, one bowel movement occurred, temperature normalized, and the infant recovered the next day.
Diarrhea
I treated a 1-year-old infant with diarrhea, occurring several to over ten times daily. Western medicine treatment at a pediatric department for over two months was ineffective. Traditional Chinese Medicine pediatric treatment was also ineffective. Reflecting on previous treatments—drying dampness, stopping dysentery, strengthening Spleen, digesting food, astringing intestines, warming Middle Jiao, warming and tonifying Spleen and Kidney—I realized these failed because infants have "tender Yin and tender Yang." Warming generates fire, while cooling injures Yang. I considered that the Shen Que (CV8, Umbilicus) acupoint connects with Spleen and Kidney Qi. I decided to use external therapy. Equal parts of Ding Xiang (Clove) and Rou Gui (Cinnamon Bark) were ground into powder, placed in the umbilicus to warm Spleen and Kidney Yang, and Shang Shi Zhi Tong Gao (Cold-Dampness Pain-Stopping Plaster) was applied externally to disperse cold. The next day, bowel movements reduced to three times. By the third day, stools were normal, appetite returned, and the infant recovered.
Whooping Cough (Tun Ke)
The *Su Wen: Xuan Ming Wu Qi Pian* states: "The Lung governs cough." Thus, cough is often cured by formulas that diffusing Lung Qi, resolving phlegm, descending Qi, and stopping cough. However, chronic coughs often affect other Zang-Fu organs, forming the so-called "Cough of the Five Zang and Six Fu Organs." The *Su Wen: Ke Lun* states: "All Five Zang and Six Fu organs can cause cough, not just the Lung." It describes symptoms: "Lung cough presents with coughing, wheezing with sound, and expectoration of blood in severe cases. Heart cough presents with heart pain, a sensation of obstruction in the throat, and severe swelling/bi-syndrome of the throat. Liver cough presents with pain under both hypochondria, inability to turn, and fullness under the flanks. Spleen cough presents with pain under the right hypochondrium radiating to the shoulder/back, inability to move, and worsening cough upon movement. Kidney cough presents with pulling pain between waist and back, and expectoration of saliva." It further states: "Chronic cough of the Five Zang transfers to the Six Fu. Unresolved Spleen cough affects the Stomach (Stomach cough: cough with vomiting, expulsion of parasites if severe). Unresolved Liver cough affects the Gallbladder (Gallbladder cough: cough with bile vomiting). Unresolved Lung cough affects the Large Intestine (Large Intestine cough: cough with fecal incontinence). Unresolved Heart cough affects the Small Intestine (Small Intestine cough: cough with flatus loss). Unresolved Kidney cough affects the Bladder (Bladder cough: cough with urinary incontinence). Chronic cough affecting the San Jiao presents with cough, abdominal fullness, and lack of desire to eat/drink." Whooping cough, also known as Hundred-Day Cough or Egret Cough, is a common infectious disease in children. Herbal antitussives and chicken gall preparations are often effective, but repeated use may fail. Example: Han XX, male, 1 year old. Coughing for over two months, hospitalized twice, ineffective for 60+ days. Symptoms: paroxysmal cough with sound, facial edema, abdominal distension, poor appetite, expectoration of phlegm and food during severe coughing, dry stool. Integrating symptoms and pulse, this is San Jiao Cough. I prescribed to unblock the San Jiao and regulate Lung and Stomach. Prescription: Zi Su Ye (Perilla Leaf) 6g, Ban Xia (Pinellia Tuber) 6g, Chen Pi (Tangerine Peel) 6g, Huang Qin (Scutellaria Root) 6g, Bo He (Mint) 6g, Xing Ren (Apricot Seed) 6g, Zi Wan (Aster Root) 6g, Da Huang (Rhubarb Root) 3g, Si Gua Luo (Luffa Reticulate Fiber) 9g, Bai Bu (Stemona Root) 12g. After two doses, cough decreased significantly. After six doses, recovery was swift. Later, four children in the hospital nursery with chronic whooping cough were cured using the same method. This confirmed the significance of "All Five Zang and Six Fu organs can cause cough, not just the Lung."
Dementia
I treated a 3-year-old male infant. Dementia was noted since birth. At age three, walking was still unstable, speech was delayed and hesitant. He could not say "Dad" or "Mom." Walking twenty steps led to falling due to instability. During meals, he did not know hunger or satiety. Family history revealed both maternal uncles and granduncles suffered from the same condition; the granduncle (over 50) and uncles (20-30s) could not care for themselves. The mother feared her son would become similarly disabled and sought treatment. Examination showed dementia, delayed speech, unstable gait, thin white tongue coating, and string-taut, fine pulse. Integrating pulse and symptoms, I reasoned this congenital condition involves the Kidney. Delayed speech and confused spirit involve the Heart. I prescribed tonifying Heart and Kidney, resolving phlegm, using Di Huang Yin Zi (Rehmannia Decoction) with modifications. Prescription: Sheng Di Huang (Rehmannia Root) 12g, Shan Yu Rou (Cornus Fruit) 4g, Shi Hu (Dendrobium Stem) 3g, Mai Dong (Ophiopogon Tuber) 4g, Wu Wei Zi (Schisandra Fruit) 3g, Shi Chang Pu (Acorus Rhizome) 3g, Yuan Zhi (Polygala Root) 3g, Fu Ling (Poria) 3g, Da Yun (Cistanche Stem) 5g, Fu Zi (Aconite Lateral Root) 1g, Rou Gui (Cinnamon Bark) 1g, Ba Ji Tian (Morinda Root) 4g, Bo He (Mint) 1g. After 40 doses, walking strength improved, running became stable, speech became more frequent and flexible. When asked why his mother wasn't there, he said, "She is sick." After another 40 doses, he could run freely and speak extensively. To further improve efficacy, he continues medication.
