Lao Zhai (chronic consumptive disease, similar to pulmonary tuberculosis) is primarily due to Yin deficiency, with pathological changes involving phlegm and blood. The gradual emaciation of Xu Lao (deficiency fatigue) belongs to Fire syndrome; the Yin fire consumes and melts fluids, which is caused by accumulated heat. In the initial stage when the body is still robust, He Zihe's method of attacking pathogens can be used; in the later stage, if the body becomes emaciated and weak, Si Wu Tang (Four Substance Decoction) should be modified and taken with Xiao Ji Wan (Accumulation-Dissolving Pill) to avoid damaging intestinal Qi. The occurrence of zheng zheng fa re (steaming fever, a continuous fever like steam) is mostly due to accumulation diseases; for Lao Bing (consumptive disease), Si Wu Tang (Four Substance Decoction) should be added with Chao Bai (Stir-fried Phellodendri Cortex), Zhu Li (Bamboo Sap), Ren Nao (Human Urine), and Jiang Zhi (Ginger Juice), with great tonification as the superior approach. Cases with muscle wasting and severe high fever are difficult to treat.
Prescription:
Qing Hao (Artemisiae Annuae Herba) one dou five sheng, Tong Bian (Child's Urine) three dou. Boil over moderate and strong fire until the Tong Bian reduces to two dou. Remove Qing Hao and continue boiling until it reduces to one dou. Add seven Pig Gallbladders (Suis Dan), boil a few more times. Finally, add Gan Cao Mo (Licorice Root Powder) to form a paste. Take one spoonful each time, mixed with warm water.
[Appendix] The syndromes of Lao Zhai are not limited to one type. Its onset is invariably due to physical weakness and labor injuring the Heart and Kidney. Since the Heart governs Blood and the Kidney governs Essence, exhaustion of Essence and dryness of Blood lead to Lao Bing. Therefore, transmission varies, including Gu Zheng (Bone-steaming sensation) and Fu Lian Shi Zhu (Contagious Corpse-Transmitting Disease). "Zhu" means to transmit; pathogenic factors move from top to bottom, spreading through bones and flesh, sometimes leading to the extinction of an entire family. The symptoms involve worms biting between the Zang organs, Heart, and Lung, named Zhai Ji (Tuberculous Disease), which is difficult to cure. For Chuan Shui Lao Zhai (Transmitted Corpse Consumptive Disease) with alternating chills and fever, chronic cough with hemoptysis, and progressive emaciation, San Ao Tang (Three Unrestrained Decoction) combined with Lian Xin San (Lotus Plumule Powder) decocted together should be administered first, ensuring success without failure.

[Appendix] From Mr. Ge Kejiu's *Ten Medicines Divine Book* on Lao Syndromes, seven prescriptions are excerpted. Human life relies on inheriting the chaotic Qi of Heaven and Earth. If one guards the True Yuan (Original Qi) and consolidates the root, ten thousand diseases will not arise, and the four limbs will be healthy. If one fails to nourish the True Yuan and consolidate the root, diseases arise from this. The True Yuan and root are essentially Qi, Blood, Essence, and Fluids. I once heard my master say: "Among all diseases, none is harder to treat than Lao Syndromes." The cause of Lao lies in people during their prime, when Qi and Blood are complete and Essence and Fluids are full, failing to preserve their life, indulging in wine and lust, day and night without rest, thereby dissipating the True Yuan and ruining the Essence. This leads to vomiting blood and spitting phlegm, resulting in bone-steaming and body heat, Kidney deficiency with essence exhaustion, pale face with red cheeks, dry mouth and throat, white turbidity, spermatorrhea, and night sweats, difficulty eating, and total loss of strength. This is called Fire Excess and Metal Deficiency (Heart Fire overwhelming Lung Metal). Severe cases die within half a year; mild cases within a year. Moreover, physicians who do not exhaustively investigate the source and root may administer overly cold formulas or overly hot medicines, treating recklessly without effect. It is unknown that excessive cold further empties the middle jiao, and excessive heat further depletes the interior. Thus, among physicians treating Lao diseases today, not one in ten thousand succeeds. My master's use of medicine for Lao was like Hou Yi shooting arrows—never missing the target. The following outlines the sequence of medication. If there is vomiting or coughing up blood, first use Shi Hui San (Ten Charcoals Powder) to arrest it. If severe, use Hua Rui Shi San (Flower Ochre Stone Powder) to stop bleeding.
Generally, Heat in Blood causes flow, Cold in Blood causes coagulation, and seeing black stops bleeding; this is inevitable. After stopping bleeding, the patient will likely feel fatigued. Next, use Du Shen Tang (Single Ginseng Decoction) to tonify, allowing them to sleep soundly without disturbance. Upon waking, the disease will have receded by 50-60%, then other medicines can be administered.
Bao He Tang stops cough and calms the Lung; Bao Zhen Tang tonifies deficiency and clears heat; Tai Ping Wan (Peaceful Pill) moistens the Lung and eliminates flaccidity; Xiao Hua Wan (Digestion Pill) resolves phlegm and moves Qi.
In Bao He Tang, six aspects are distinguished: Blood Excess, Phlegm Excess, Asthma Excess, Heat Excess, Wind Excess, and Cold Excess, with added ingredients to harmonize. In Bao Zhen Tang, six aspects are distinguished: Palpitations/Fright, Dysuria/Turbidity, Difficult Urination, Spermatorrhea, Dry Heat, and Night Sweats, with added ingredients for use; no other additions are made. The method of taking medicine: daily, decoct thick Bo He (Mint) soup to gargle the throat. With Tai Ping Wan, chew one pill slowly and swallow, then hold another pill in the mouth to dissolve slowly. When going to bed, use it similarly. At night, the Lung orifices open, and the medicine must flow into the orifices; this secret is crucial. If Phlegm is obstructing, first take 100 Xiao Hua Wan pills mixed with Yi Tang (Maltose) swallowed, then follow the previous method of holding and chewing Tai Ping Wan, lying on the back to sleep. After taking the first seven medicines, if there is Lung cough, Lu Run Wan (Lung-Moistening Pill) can be boiled and eaten. If there is spare time before taking the usual seven medicines, this can be boiled and taken; alternatively, Bai Feng Gao (White Phoenix Paste) can be continuously boiled and eaten to solidify the root and complete the foundation. After the disease subsides, Shi Zhen Wan (Ten Treasures Pill) can be compounded and taken; this is the wonderful application for concluding treatment and recovery.
Shi Hui San treats Lao syndrome vomiting blood, hemoptysis, and coughing blood; use this first to arrest bleeding.
Da Ji (Great Thistle Root), Xiao Ji (Small Thistle Root), Bai Ye (Thuja Leaf), He Ye (Lotus Leaf), Mao Gen (Imperatae Rhizoma), Qian Gen (Rubiae Radix), Da Huang (Rhei Radix et Rhizoma), Shan Zhi (Gardeniae Fructus), Mu Dan Pi (Moutan Cortex), Zong Lv Hui (Palma Ash).
Equal parts, charred but retaining nature, ground fine, wrapped in paper under a bowl placed on the ground overnight to remove fire toxicity. When using, first crush fresh White Lotus Root (Ou) and squeeze juice, or Radish Juice, to mix with the powder.
Another Prescription:
