疡家不治证(外科医生认为难以治愈的证候)
The pulse is rapid and should be accompanied by fever, yet there is aversion to cold; if there is a painful area, it indicates the impending formation of an abscess (Yong).
A rapid and deficient pulse, with coughing and spitting of saliva, indicates Lung Wei (Lung Atrophy).
A rapid and excess pulse, or slippery pulse, with coughing and dull pain in the chest, indicates Lung Abscess (Fei Yong).
A tight and rapid pulse indicates that pus has not yet formed; if the tightness disappears leaving only a rapid pulse, it indicates that pus has already formed.
A slippery and rapid pulse, with hardness and fullness in the lower abdomen, occasional difficulty in urination, sweating, or aversion to cold, indicates Intestinal Abscess (Chang Yong).
If the pulse is slow, tight, and knotted, it indicates blood stasis; evacuating the blood will lead to recovery.
If the pulse is surging and rapid, it indicates that pus has already formed. A slippery pulse indicates excess, and a rapid pulse indicates heat. The rapidity of Wei Qi descends, while the slipperiness of Ying Qi ascends; when Ying and Wei interfere with each other, blood transforms into turbid waste. In severe cases, abdominal distension occurs, and water sounds can be heard when turning over.
Lung Wei: Heat resides in the Upper Jiao. The disease often presents with excessive saliva and sputum, difficult and frequent urination, stools resembling pig brain, a desire to cough but inability to do so, dry frothy sputum, blood in the saliva, shortness of breath and chest fullness, or dryness and thirst. The Cun Kou pulse is rapid and deficient, feeling choppy upon pressure.
Gan Cao (Licorice Root, 4 liang), Gan Jiang (Dried Ginger, 2 liang), Ren Shen (Ginseng Root, 1 liang), Sheng Jiang (Fresh Ginger, 3 slices), Da Zao (Jujube Fruit, 3 pieces). Decoct together.
Lung Abscess (Fei Yong): This occurs when Wind pathogen injures the Wei level, and Heat pathogen excessively affects the Ying level, causing blood stagnation and heat accumulation in the lungs. Symptoms include coughing up pus, dry mouth, dull chest pain, wheezing and chest fullness without thirst, foul-smelling sputum, frequent chills, and vomiting material resembling rice porridge. The Cun Kou pulse is rapid and excess, feeling slippery upon pressure.
Xiao Qing Long Tang (Minor Blue Dragon Decoction): Used to release exterior pathogens first in the treatment of Lung Abscess; this is a method for treating unruptured abscesses.
Tingli Dazao Xiedang Tang (Descurainia Seed and Jujube Decoction to Drain the Lung): Used to treat Lung Abscess with wheezing preventing lying down.
Fry Ting Li until yellow, grind into pills the size of a bullet. Use 3 sheng of water, decoct Da Zao first until 2 sheng remains, add Ting Li and decoct until 1 sheng remains. Take in one dose. Administer three doses of Xiao Qing Long Tang first, then administer this formula.
Jie Geng Tang (Platycodon Decoction): Used to treat cough with chest fullness, sputum like rice porridge, indicating the need to expectorate pus and blood.
Gan Cao (Licorice Root), Jie Geng (Platycodon Root). Decoct in water.
Wei Ye Tang (Reed Leaf Decoction): Used to treat cough with slight fever and irritability, and rough skin on the chest and heart area like fish scales; this is a method for treating ruptured ulcers.
Wei Ye (Reed Leaves, 2 sheng, cut), Yi Yi Ren (Coix Seed, 8 liang), Gua Lou Ren (Trichosanthes Seed, 8 liang), Tao Ren (Peach Seed, 50, peeled and cored). Decoct and take.
Secret Formula: Decoct one Gua Lou fruit including the pulp.
Intestinal Abscess (Chang Yong): Caused by Damp-Heat.
Yi Yi Ren (Coix Seed, 7 fen), Fu Zi (Prepared Aconite Root, roasted and peeled), Bai Jiang Cao (Patrinia Herb, 2 fen). Make into powder. Treats Intestinal Abscess with tense abdominal skin, body skin rough like scales, sensation of bloating, but no prior mass accumulation, no body fever, and a rapid pulse. Decoct in water and take.
Da Huang Mu Dan Tang (Rhubarb and Moutan Decoction): Treats Intestinal Abscess where pus has not yet formed; purgation can be used.
Da Huang (Rhubarb Root, 4 liang), Mu Dan Pi (Moutan Bark, 3 liang), Mang Xiao (Mirabilite, 2 liang), Gua Zi (Melon Seeds, 1 sheng), Tao Ren (Peach Seed, 50). Decoct in water and take in one dose.
Yun Mu Gao (Cloud Mother Ointment): A woman suffered from abdominal pain, unresponsive to hundreds of treatments. Her pulse was slippery and rapid, abdominal skin tense. The pulse should have been deep and fine, but was instead slippery and rapid; thus, purgation was used.
Make Yun Mu Gao pills the size of Sophora seeds, 100 pills per dose. Add E Jiao (Donkey-hide Gelatin) dissolved in wine, and take with wine. Stop when pus and blood are evacuated.
Scrotal Abscess (Nang Yong): Caused by Damp-Heat sinking downward. Turbid Qi flows into the excretory channels due to deficiency in the vaginal/urinary tract and poor water passage function. Peace returns naturally after pus is exhausted.
Dang Gui Gan Cao Fang Feng Tang (Angelica, Licorice, and Saposhnikovia Decoction): Li's formula for treating Perianal Abscess (Bian Yong).
Tao Ren Cheng Qi Tang (Peach Kernel and Magnolia Bark Decoction): Zhang's formula for treating Perianal Abscess (Bian Yong).
Perianal Disease Formula: Hu Lu Ba (Fenugreek Seed, taken as powder). Also, Chuan Lian Zi ash (also good).
Breast Abscess (Ru Yong): Caused by rich flavors leading to Damp-Heat phlegm accumulating between the diaphragm, interacting with stagnant milk. It can also be caused by anger stirring stagnant milk, or by an infant's breath blowing on stagnant milk.
The breast belongs to the Yangming channel, and the nipple is traversed by the Jueyin channel. All breast diseases involve the Yangming channel. Deep lesions become Yan (cancerous tumors). Treatment should soothe Jueyin stagnation, clear Yangming heat, move sweat and blood, and dissipate swelling and nodules.
Shi Gao (Gypsum Fibrosum, clears Yangming heat), Qing Pi (Citrus Peel, soothes Jueyin), Hua Pi (Birch Bark, burnt), Bai Zhi (Angelica Dahurica Root), Gua Lou Pi (Trichosanthes Peel, reduces swelling), Gan Cao Jie (Licorice Joint, moves blood), Feng Fang (Honeycomb). For Qi stagnation, add Tai Xiong (Ligusticum Chuanxiong Rhizome) and Xiang Fu (Cyperus Rhizome). Ge Gen (Pueraria Root) guides the herbs.
Take the above with ginger-infused wine.
Another Formula: Da Huang (Rhubarb Root), Tian Hua Fen (Trichosanthes Root), Chuan Dang Gui (Angelica Sinensis Root), Gan Cao Jie (Licorice Joint, 1 liang), Gua Lou Zi (Trichosanthes Seed), Shan Jian Jia (Sunda Pangolin Scale, stir-fried with old wall earth, 1.5 liang each). Make into wine pills.
Bone Ju (Gu Ju): Caused by rich flavors and wading in water after drinking alcohol, leading to Cold attack. Heat pathogen penetrates deeply around the Bi Shu acupoint, where phlegm accumulation and blood stasis interact to form Bone Ju (Attaching-to-Bone Abscess).
Formula: Cang Zhu (Atractylodes Rhizome), Chuan Bai (Phellodendron Bark), Qing Pi (Citrus Peel, moves Qi). For deficiency, add Niu Xi (Achyranthes Root). Jiang Zhi (Ginger Juice, pungent dispersal), Gan Cao (Licorice Root). If inflammation does not subside, add Ma Huang (Ephedra Herb); in winter, add Gui Zhi (Cinnamon Twig); in summer, add Huang Qin (Scutellaria Root).
Another Formula: Fang Feng Tong Sheng San (Saposhnikovia Decoction to Unblock the Hairy Surface), remove Mang Xiao (Mirabilite) and Da Huang (Rhubarb Root), add Sheng Xi Jiao (Rhino Horn Powder) and Fu Ping (Spirodela Powder). Treats Bone Ju.
Attaching-to-Bone Ju resembles Bai Hu Fei Shi and Li Jie.
Li Jie moves unpredictably; Bai Hu Fei Shi can form pus upon pressure; Attaching-to-Bone Ju arises from the bone, causes pain, and pressure yields no benefit.
Internal Ju (Nei Ju): Caused by Fire from diet and Fire from seven emotions, mutually constrained and erupting within the thoracic cavity inwardly, not in the stomach/intestines or blind membrane. Because it cannot be seen, it is named Internal. Treatment should use Si Wu Tang (Four Substances Decoction) plus cooling agents.
The Master said: Someone with an impatient nature and rich taste experienced pain at one point under the left hypochondrium. After taking hot and drying medicines, the pulse was wiry on light pressure and hollow on heavy pressure. Knowing there was pus at the painful site, he treated it as Internal Ju.
Goiter Types (Ying Zhuang): Often appear on the shoulder and neck. Hard and immovable is Stone Goiter (Shi Ying). Skin color unchanged is Internal Goiter (Nei Ying). Red vessels interlacing is Blood Goiter (Xue Ying). Visible tendon knots is Tendon Goiter (Jin Ying). Fluctuates with worry and anger is Qi Goiter (Qi Ying).
Tumor Types (Liu Zhuang): Formed by Qi condensation, including bone, fat, pus, blood, and flesh types.
Mouth Ulcer (Kou Chuang): Yan Xiao (Saltpeter), Peng Sha (Borax). Hold in mouth without opening. Grind Nan Xing (Arisaema Rhizome) with vinegar and apply to Yong Quan acupoint.
Alcohol-induced Mouth Ulceration: Dao Chi San (Guide Out Redness Powder), Wu Ling San (Five Ingredient Powder with Poria).
Wind-Cold constricting and cutting off Yang Qi, resulting in loss of voice: Ban Xia (Pinellia Tuber, 1 liang), Cao Wu (Aconitum Carmichaelii Root), Gui Zhi (Cinnamon Twig, 1 qian each). Decoct and take.
Red Mouth Ulcer: Bai Fan (Alumen, refined), Mo Yao (Myrrh), Ru Xiang (Frankincense), Tong Lv (Malachite). Apply as powder.
White Mouth Ulcer: Xiong Huang (Realgar), Mo Yao (Myrrh), Ru Xiang (Frankincense, 1 qian each), Qing Fen (Calomel, 5 fen), Ba Dou (Croton Seed). Apply as powder.

Painful Mouth Ulcer: Wu Bei Zi (Galla Chinensis, 1 liang), Huang Bai (Phellodendron Bark, honey-fried), Huo Shi (Calcined Rock Salt, 5 qian each). Apply Tong Lv (Malachite) powder.
Also, rinse with juice from Bai Qiang Wei Gen (Root of White Rose).
Pediatric Mouth Ulcer with Anorexia: Soak feet halfway up in Fan Tang (Alum Decoction) for immediate relief. Use Mi Zhi Chuan Huang Bo (Honey-fried Phellodendron Bark, stir-fried) and Jiang Can (Silkworm) ground into powder for application. Nursing and calmness follow immediately.
Toxic Sore on Feet: Mi Tuo Seng (Lead Carbonate), Huang Lian (Coptis Rhizome). Apply as powder.
Also, Du Niu Xi (Cyathula Root) salted and pounded.
Also, Han Lian Cao (Eclipta Herb, also known as Mo Ji Zhi Cao), salted and pounded, covered with a thin cake of crushed Sang Bai Pi (Mulberry Root Bark). Replace when dry.

Also, apply Xiong Huang (Realgar).
Hand Chronic Ulcer (Shou Chi Chuang): Zao Jiao (Tamarisk Bean), Qing Fen (Calomel), Ku Fan (Alum, calcined), Huang Lian (Coptis Rhizome), Huang Bai (Phellodendron Bark).
Sand Rash (Sha Chuang): Ta Di Teng (Solena Amplexicaulis Stem), burnt to ash for application.
Malignant Sore (E Chuang): Dried powder of withered Banana leaves after frost, mixed with Sesame Oil for application, covered with oil paper. Wash first with decoction of Jin Yin Teng (Honeysuckle Stem), Jin Si Cao (Hypericum Japonicum), Cong Bai (Scallion White), and Jiao (Pepper).
Also, white ants from pine trees and Huang Dan (Red Lead), both burnt black, mixed with Sesame Oil for application, covered with oil paper daily. Later, apply Long Gu (Fossilized Bone) powder to promote tissue regeneration.
Also, fry Huang Dan (Red Lead) in Sesame Oil, add Po Xiao (Niter) and smear on.
Golden Thread Rash (Jin Si Chuang): Like threads of varying thickness. If extending up and down to the heart, death ensues. Prick the head of the sore to bleed, then chew Ping Cao Gen (Water Fern Root) and apply for relief.
