Ma (66 years old): Warm pathogenic factors emerge from the interior. Both pulses are surging and large (Hong Da). There is irritability, thirst, and loose stools. The epigastrium feels stifled with poor appetite. The tongue coating is crimson in the center and white at the edges. This corresponds to simultaneous manifestation of exterior and interior syndromes of the Lung and Large Intestine. Tentatively prescribe clearing the interior heat, with a slight addition of herbs to vent the pathogen.

Zhao (68 years old): Warm pathogenic factors transmit reversely to the Danzhong region. Heart Yang is obscured and cannot circulate freely. Symptoms include vomiting and hiccuping, coma, irritability, and thirst. The pulse is rapid, and the tongue is crimson. In an elderly patient, the fluids of the five zang organs are all depleted. There is great danger of orifice closure (obstruction of consciousness) and collapse of jue (yang collapse). Prescribe clearing heat from the Ying (Nutrient) level and collaterals.

Yu (30 years old): Alternating chills and fever. Headache and thirst. Cold damages Ying (Nutrient), Wind damages Wei (Defensive). Exterior and interior pathogens have remained for two days. Occasional abdominal pain and knee pain occur. The pulse is floating with spontaneous sweating. This is due to uneven cold and warm weather in winter, contracting seasonal pathogens. In spring, when Yang Qi erupts, latent pathogens move internally. Treatment differs from malaria.

Su (26 years old): Summer-heat and dampness are not yet cleared. White tongue coating and epigastric stifling. Pulse is soggy and weak. Thirst and loose stools. It is stated that initially, running caused chaotic Qi movement, leading to Lung Qi stagnation with ascending but no descending. Subsequently, it spread throughout the San Jiao (Triple Burner). Hence, fever does not subside with sweating. Taking bitter-cold and acrid-cold formulas is ineffective. The San Jiao is the father of Qi; Qi stagnation, combined with summer-heat and dampness obstruction, relies on the clearing Yang above expanding. Dampness will naturally descend, and once warmth is removed, heat clears naturally.

Lu (64 years old): After taking a summer-clearing formula, heavy head and yellow tongue coating persist. Body pain and cough with phlegm have eased, but stool remains unpassed. Craving cold drinks, no hunger, stifling sensation, frequent belching. Clearly, summer-heat夹杂着 dampness occupies the Qi level of the Upper Jiao, obstructing Qi flow. However, for the elderly, acrid-cold and bitter-cold may hinder the stomach appetite. Previous misuse was common. Therefore, prescribe acrid-warm methods to diffuse, reach out, and unblock Yang.

Propose Jingyue's Yü Nv Jian (Jade Maiden Decoction). Remove Niu Xi (Radix Achyranthis Bidentatae) and Mai Dong (Radix Ophiopogonis). Add Dan Pi (Cortex Moutan), Zhu Ye (Lamina Folii Nymphaeae), and Chuan Bei (Bulbus Fritillariae Cirrhosae).

Second visit: Crimson tongue and thirst. Brief chills followed by fever. Hands and feet are numb. True Yin of the Foot-Shaoyin Kidney and Foot-Jueyin Liver is nearly exhausted. Yang refuses to hide. Frequent thirst cannot save the internal burning. As the classics say, "Entering Liver causes numbness; entering Kidney causes Xiao Ke (Wasting-Thirst)." Pathogens in Liver and Kidney are deep and free-roaming. Acrid-cold, acrid-cool, and bitter-draining methods cannot enter the realm of Supreme Yin. Following ancient methods, use heavy medicines lightly administered to avoid delay and complications.
Nong Shu Di Huang (Radix Rehmanniae Preparata, 4 qian) Shang Qing E Jiao (Colla Corii Asini, 2 qian) Zhi Gan Cao (Radix Glycyrrhizae Preparata, 4 fen) Jian Mai Dong (Radix Ophiopogonis, 1.5 qian) Da Ma Ren (Semen Cannabis, 1.5 qian) Dan Tian Dong (Radix Asparagi, 1.5 qian) Yang Shen (Radix Panacis Quinquefolii, 8 fen)
Dong: Right pulse is short and rapid; Left pulse is fine and rapid. Awake with little sleep. Body cool but with alternating chills and fever. Occasional asthma. Epigastric stifling and white tongue coating. This belongs to heat pathogen sinking inward. It is a severe condition. Tentatively prescribe clearing and draining the interior of Shaoyin (Kidney), combined with resolving the exterior of Shaoyang (Gallbladder). Await correction from experts.
