The sage treats diseases before they occur and predicts future illnesses; this is excellent. Those who are about to suffer from stroke (Zhong Feng) will certainly have prodromal symptoms: numbness and insensitivity in the thumb and index finger, weakness in the limbs, or slight muscle twitching. These are prodromes, indicating that severe wind disease (stroke) will inevitably occur within three years. The Classics state: "In acute cases, treat the branch (symptoms); in chronic cases, treat the root (cause)." It is advisable to regulate Ying and Wei (Nutritive and Defensive Qi), first taking one dose each of Bafeng San (Eight Winds Powder), Yufeng Tang (Wind-Preventing Decoction), and Tianma Wan (Gastrodia Pill) to achieve effect. For prevention, Changfu Jiajian Fangfeng Tongsheng San (Modified Fangfeng Tongsheng Powder) should be taken regularly so that wind diseases do not arise, ensuring peace and health.
Yufeng Tang (Wind-Preventing Decoction): When one first feels signs of wind movement, taking this medicine prevents collapse. It is a sacred medicine for treating disease before it occurs.
It also treats stroke syndromes where internal pathogens have been eliminated and external pathogens have been exhausted. One should take this medicine to guide and unblock the meridians. Long-term use will eliminate severe wind pathogens; even if minor pathogens remain, treatment can be adjusted based on this formula. However, the method of treating disease must not lose balance between unblocking and obstructing. Sometimes inducing mild sweating over one qi-cycle (solar term/period), or promoting bowel movements over ten days, constitutes the method for regular use. Over time, clear and turbid energies will naturally separate, and Ying and Wei will harmonize.
Ingredients: Qianghuo (Notopterygii Rhizoma et Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Fangfeng (Saposhnikoviae Radix), Danggui (Angelicae Sinensis Radix), Manjingzi (Vernoniae Fructus), Chuanxiong (Ligustici Chuanxiong Rhizoma), Xixin (Asari Radix et Rhizoma), Huangqi (Astragali Radix), Zhike (Aurantii Fructus Immaturus), Renshen (Ginseng Radix), Mahuang (Ephedrae Herba), Xiangbaizhi (Angelicae Dahuricae Radix), Ganjuhua (Chrysanthemi Flos), Bohe (Menthae Haplocalycis Herba), Gouqi (Lycii Fructus), Chaihu (Bupleuri Radix), Zhimu (Anemarrhenae Rhizoma), Digupi (Lycii Cortex), Duhuo (Angelicae Pubescentis Radix), Duzhong (Eucommiae Cortex), Qinjiao (Gentianae Macrophyllae Radix), Banxia (Pinelliae Rhizoma Praeparatum), Qianhu (Peucedani Radix), Houpo (Magnoliae Officinalis Cortex), Shudihuang (Rehmanniae Radix Praeparata), Fangji (Stephaniae Tetrandrae Radix) [each 2 liang]; Fuling (Poria), Huangqin (Scutellariae Radix), Shaoyao (Paeoniae Radix Alba) [each 3 liang]; Shigao (Gypsum Fibrosum), Cangzhu (Atractylodis Rhizoma), Shengdihuang (Rehmanniae Radix) [each 4 liang]; Rougui (Cinnamomi Cortex) [1 liang].
Decoct these ingredients. Take 1 liang per dose with 2 cups of water and 3 slices of fresh ginger. Take on an empty stomach. Decoct the dregs again and take before sleep. Taking one dose on an empty stomach followed by Er Dan Wan (Two Cinnabar Pills) is called the heavy dose; taking one dose before sleep followed by Sibai Dan (Four White Pills) is called the light dose. The principle of this prescription is: movement calms the spirit, stillness clears the lungs.
For example, if mild symptoms appear after one qi-cycle, use 3 liang of Yufeng Tang, add 1 liang of Mahuang, divide into four doses, add fresh ginger, take on an empty stomach, and follow with congee. Stop when mild sweating occurs. If bowel unblocking is needed after ten days, use 3 liang of Yufeng Tang, add 1 liang of Dahuang (Rhei Radix et Rhizoma), divide into four doses, take as above, before sleep, until bowels move freely. When taking this medicine regularly, one must not ignore the auxiliary effects of the four seasons.
After the great cold of spring, add Banxia, Renshen, and Chaihu (2 liang each) to the base formula, totaling 4 liang of additions. This is to intercept and drain the Qi of Shaoyang.
After Grain Rain in summer, add Shigao, Huangqin, and Zhimu (2 liang each) to the base formula, totaling 4 liang. This is to intercept and drain the Qi of Yangming.
In the late summer month, add Fangji, Baizhu (Atractylodis Macrocephalae Rhizoma), and Fuling (2 liang each) to the base formula, totaling 4 liang. This is to overcome the dampness of Spleen Earth.
After the great heat of autumn, add Houpo and Huoxiang (Agastache Herba) (2 liang each) and Rougui (1 liang) to the base formula, totaling 4 liang. This is to intercept and drain the Qi of Taiyin.
After frost descent in winter, add Fuzi (Aconiti Lateralis Radix Praeparata) and Guanzi Gui (Cinnamomi Ramulus) (1 liang each) and Danggui (2 liang) to the base formula, totaling 4 liang. This is to overcome the Qi of Shaoyin.
If spring climatic conditions prevail, reduce the winter additions; the four seasons follow similar principles. Although seasonal modifications are established, one must examine deficiency/excess and cold/heat during actual treatment, consider local geographical suitability, and assess the severity of pathogens. This medicine regulates the Seven Emotions, Six Desires, and Four Temperatures, preventing excessive dominance of any Zang organ and maintaining Ying and Wei harmony. For wind-induced constipation (Feng Mi), it prevents stagnation forever. This medicine acts internally with Tianma Wan and is a sacred medicine for preventing disease. If disease has already occurred, it should be taken regularly. Regardless of gender, age, or condition—such as convulsions, spasms, acute or chronic infantile convulsions, or seasonal cold damage—it yields miraculous results.
Bafeng San (Eight Winds Powder) [from *Heji Fang*]: Treats wind pathogen attacking upward, causing dizziness and vertigo of head and eyes, limb stiffness, skin itching, and urticaria turning into sores. Also treats disharmony of cold and heat, nasal congestion, and heavy voice.
Ingredients: Huoxiang (Patchouli Herba) [de-soiled, 8 jin], Baizhi (Angelicae Dahuricae Radix), Qianhu (Peucedani Radix) [remove rhizome, each 1 jin], Huangqi (Astragali Radix) [remove rhizome], Gancao (Glycyrrhizae Radix et Rhizoma), Renshen (Ginseng Radix) [remove rhizome, each ...].
Grind into powder. Take 2 qian per dose with 1 cup of water, add a little Bohe, decoct into soup, and take after meals.
Tianma Wan (Gastrodia Pill): Treats wind generating heat; when heat dominates, movement occurs. Stillness should overcome restlessness; this nourishes Blood.
Ingredients: Tianma (Gastrodiae Rhizoma), Niuxi (Achyranthis Bidentatae Radix) [these two soaked in wine for three days, then dried], Xuanshen (Scrophulariae Radix), Bibi (Dioscoreae Hypoglaucae Rhizoma) [each 6 liang, ground separately], Duzhong (Eucommiae Cortex) [stir-fried until fibers break, 7 liang], Fuzi (Aconiti Lateralis Radix Praeparata) [processed, 1 liang], Qianghuo (Notopterygii Rhizoma et Radix) [14 liang], Guizhi Tang (Angelicae Sinensis Radix - note: text says Chuan Gui, likely Danggui) [10 liang], Shengdihuang (Rehmanniae Radix) [16 liang].
Another version includes Duhuo (Angelicae Pubescentis Radix) [5 liang] for wind between the kidneys.
Grind into powder, mix with honey to make pills the size of paulownia seeds. Take 50-70 pills per dose on an empty stomach with warm wine or plain water.
My commentary: The method of using medicine to treat disease involves using hot medicine for cold patterns and cold medicine for hot patterns; this is direct treatment (Zheng Zhi). Currently, stroke paralysis is caused by wind-fire yang pathogens, yet hot medicines like Wutou (Aconiti Radix) and Fuzi are used. Why? Stroke paralysis is actually caused by damp-phlegm and dead blood stagnating in the zang-fu organs and meridians. Without hot medicines like Wutou and Fuzi, how can they open and circulate? This is not direct treatment, but indirect treatment (Cong Zhi / Counter-directional Treatment). The book states: "Follow less or more, observe the matter." Thus, counter-directional medicines serve only as guiding herbs. Danxi said: "Fat and pale people have much dampness; using small amounts of Wutou and Fuzi to move meridians is correct." Since this formula uses Fuzi, and assists it with Cezi (Aconiti Radix lateralis non-prepared?) and Guanzhi Gui, what is the rationale? I originally intended to remove it, but kept it temporarily for those with fat bodies and much dampness to use in reduced doses. Stop when the disease is cured; do not overdose. Fat and pale people with much dampness should not use this formula.
