Warm Disease: When Taiyang disease presents with fever, thirst, and aversion to cold absent, it is termed Warm Disease. If after inducing sweating, the body remains burning hot, it is termed Wind-Warmth. In Wind-Warmth, the pulse is floating at all three positions (Cun, Guan, Chi), there is spontaneous sweating, heaviness of the body, somnolence, heavy breathing, and difficulty in speech. If purgation is mistakenly applied, it leads to difficult urination, staring gaze, and incontinence of urine and feces. If fire therapy (such as moxibustion or heat application) is mistakenly used, mild cases present with a slightly yellow complexion, while severe cases manifest symptoms similar to convulsive disorders or epilepsy, with intermittent spasms, resembling smoke scorching. One misstep may prolong the illness, but two missteps will hasten death.

Spasm Disease: When Taiyang disease presents with fever, sweating, and absence of aversion to cold, it is termed Soft Spasm. When Taiyang disease presents with fever, absence of sweating, and presence of aversion to cold, it is termed Hard Spasm. Spasm disease can also result from excessive sweating in Taiyang disease. The patient presents with fever, cold feet, stiff and tight neck and nape, aversion to cold, occasional heat in the head, red face, red eye vessels, isolated head tremor, sudden lockjaw, and opisthotonos; this is Spasm Disease. When Taiyang disease presents with fever and a deep, thin pulse, it is termed Spasm Disease.

Dampness Disease and Formulas: When Taiyang disease presents with joint pain and irritability, and a deep, thin pulse, it is termed Damp-Bi. The manifestation of Damp-Bi includes difficult urination and loose stools; treatment should focus on promoting diuresis. Patients with dampness diseases present with generalized body pain, fever, and a yellowish complexion resembling smoke staining. These patients may have sweating only on the head, stiffness in the back, and a desire for warmth from blankets or fire. If purgation is applied prematurely, it leads to hiccups, chest fullness, difficult urination, and a greasy tongue coating, due to heat in the Lower Jiao and cold in the Middle Jiao, causing dry mouth and irritability despite an inability to drink. If a dampness patient undergoes purgation leading to forehead sweating, slight wheezing, and free urination, the prognosis is fatal; similarly, persistent diarrhea is fatal. For dampness patients presenting with body pain, fever, yellow face, wheezing, headache, nasal congestion, and irritability, with a large pulse, normal appetite, and no abdominal discomfort, the pathology lies in cold-damp obstruction in the head causing nasal congestion; administering medication via the nose cures it. When Wind-Dampness combines, causing generalized body pain, sweating should resolve it. However, if rain persists and sweating fails to cure, it is because profuse sweating expels wind but leaves dampness behind. To treat Wind-Dampness, induce slight perspiration so that both wind and dampness are expelled. If a patient has generalized body pain, fever, and worsening symptoms in the late afternoon (Shen hour), it is termed Wind-Dampness, caused by sweating followed by exposure to wind or prolonged cold injury. After eight or nine days of Cold Damage, if Wind-Dampness combines causing severe body pain, inability to turn over, no vomiting, no thirst, and a floating, deficient, and choppy pulse, Guizhi Fuzi Tang is indicated. If stool is hard and urination is free, remove Guizhi and add Baizhu Tang. If Wind-Dampness combines causing severe joint pain, pulling pain, inability to flex or extend, exacerbation upon touch, sweating, shortness of breath, difficult urination, aversion to wind, reluctance to remove clothes, or slight edema, Gancao Fuzi Tang is indicated.

Heatstroke (E): In Taiyang Heatstroke, there is fever, aversion to cold, heaviness and pain in the body, a string-taut, thin, hollow, and slow pulse, chills after urination, cold extremities, fever upon slight exertion, open mouth with dry anterior teeth. Inducing sweating worsens aversion to cold; applying warm needles worsens fever; repeated purgation worsens strangury. Taiyang Zhongre (Summerheat) is synonymous with E. It presents with sweating, aversion to cold, fever, and thirst. Taiyang Heatstroke with fever, pain, heaviness, and a weak pulse is caused by summer cold water injury, where water resides in the skin.

Huo Luan and Formulas: Vomiting and diarrhea together constitute Huo Luan. Fever, headache, body pain, aversion to cold, vomiting, and diarrhea also constitute Huo Luan. If self-induced vomiting and diarrhea cease but fever recurs, it indicates a new pathogenic factor. In Cold Damage with a faint and choppy pulse, originally Huo Luan, now Cold Damage, entering the Yin meridians after four or five days leads to diarrhea. If originally vomiting and diarrhea occurred, it is difficult to treat. If one feels the urge to defecate but passes gas instead without diarrhea, it belongs to Yangming; stool will be hard, resolving in thirteen days as the meridian cycle completes. After diarrhea, if stool becomes hard and the patient can eat, recovery is indicated. If the patient cannot eat initially but gains appetite after passing through another meridian day, recovery is expected; if not, it does not belong to Yangming. For Huo Luan with headache, fever, body pain, heat predominance, and thirst, Wuling San is indicated. For cold predominance with no thirst, Lizhong Tang is indicated. For vomiting, diarrhea, sweating, fever, aversion to cold, limb cramping, and cold extremities, Sini Tang is indicated. For simultaneous vomiting and diarrhea, free urination, profuse sweating, undigested food in stool, internal cold with external heat, and a faint pulse nearly disappearing, Sini Tang is indicated. For cessation of vomiting and diarrhea with sweating, cold extremities, unresolved limb cramping, and a faint pulse nearly disappearing, Tongmai Sini Tang with Porcine Bile is indicated. For aversion to cold, faint pulse, resolution of diarrhea, and blood depletion, Sini Tang with Ginseng is indicated. For cessation of vomiting and diarrhea with persistent body pain, harmonize the exterior gently with Guizhi Tang. For mild irritability after vomiting, diarrhea, and sweating with a normalized pulse, it is due to deficiency failing to handle grain qi.

Post-Illness Labor Relapse and Yin-Yang Exchange and Formulas: After major illness, frequent spitting that persists indicates stomach cold; warm with pills, preferably Lizhong Wan. After Cold Damage resolves, if emaciation, shortness of breath, and rebellious qi causing nausea occur, Zhuye Shigao Tang is indicated. If edema below the waist occurs after recovery, Muschi Zexie San is indicated. If fever recurs after Cold Damage recovery, Xiao Chaihu Tang is indicated; if the pulse is floating, use sweating; if deep and solid, use purgation. For labor relapse after major illness, Zhishi Zhizi Tang is indicated; add Dahuang if there is retained food. If the pulse has resolved but evening irritability occurs due to forced eating during recovery, reducing food intake resolves it. (Above: Post-illness labor relapse)

Yin-Yang Exchange in Cold Damage: Symptoms include heavy body, shortness of breath, lower abdominal urgency, possibly pulling pain in the genitalia, heat rushing to the chest, heavy head unwilling to lift, visual disturbances, and cramping in the knees and shins. Shaokui San is indicated. (Yin-Yang Exchange)
