Taiyin disease presents with warm hands and feet without cold extremities (Jue); Shaoyin disease presents with cold extremities that cannot be warmed; Jueyin disease's cold Jue requires differentiation of severity, while heat Jue requires differentiation of depth.
[Note] The Taiyin meridian does not present with Jue (cold extremities), but rather with naturally warm hands and feet. The Shaoyin meridian presents with cold Jue but not heat Jue. The Jueyin meridian presents with both cold and heat Jue. Cold Jue refers to symptoms characterized only by coldness without heat. Heat Jue refers to a condition where internal heat causes extremity coldness, or extremity coldness induces internal heat, with heat and Jue alternating incessantly. In such cases, one must differentiate the Yin-Yang status and the depth of the pathogen, treating with formulas such as Danggui Sini Tang (Angelica Sinensis Decoction for Cold Extremities) or Chengqi Tang (Purgative Decoctions). For details, refer to the Jueyin chapter.
Fullness in the lower abdomen with pain upon palpation, accompanied by cold extremities and clear, white urine, indicates cold binding in the Bladder; absence of cold extremities with free urination indicates blood stasis in the Bladder; difficulty in urination indicates water retention causing harm.
[Note] Lower abdominal fullness with pain on pressure, if accompanied by cold extremities and clear, white urine, indicates cold binding in the Bladder, suitable for Danggui Sini Tang plus Evodia and Fresh Ginger Decoction. If there is no cold extremities and urination is free, it indicates blood stasis in the Bladder, suitable for Taoren Chengqi Tang (Peach Kernel Purgative Decoction). If urination is difficult, it indicates water retention in the Bladder, suitable for Wuling San (Five-Ingredient Powder with Poria). If both stool and urine are obstructed, it indicates water-heat accumulation, suitable for Bazheng San (Eight-Ingredient Powder for Rectification).
Mental confusion due to Stomach heat, extreme Yang heat leading to mania, can be treated with Sanhuang Shigao Tang (Three-Yellow Gypsum Decoction), the three Chengqi Tangs, or Baihu Jiedu Tang (White Tiger Detoxifying Decoction). Blood stasis causing mania with free urination and hardness/pain in the lower abdomen belongs to the Taiyin/Sunyang category; Yangming blood stasis presents with black stool, mental restlessness akin to mania, and forgetfulness, treatable with Taoren Chengqi Tang or Didang Tang. One must recognize the phenomenon of sudden mania occurring just before sweating resolves the condition.
[Note] "Mental confusion due to Stomach heat" means that mental confusion arises from extreme heat in the Stomach meridian affecting the Heart. "Extreme Yang heat leading to mania" means that when heat enters the Yang aspect, it leads to madness. "Sanhuang" refers to Sanhuang Shigao Tang, used for mental confusion, mania, exterior excess with no sweat. "Three Chengqi" refers to the three Chengqi Tangs, used for mental confusion, mania, interior excess with constipation. "Baijie Tang" refers to Baihu Jiedu Tang, used for mental confusion, mania, with neither exterior nor interior signs but extreme heat. Taiyin blood stasis causing mania presents with hardness and pain in the lower abdomen and free urination. If urination is difficult, it is water-heat accumulation, not blood stasis. Yangming blood stasis causing mental restlessness presents with forgetfulness and black stool; if the stool is not black, it is extreme heat, not blood stasis. For mild blood stasis, use Taoren Chengqi Tang; for severe cases, use Didang Tang, choosing according to the situation. However, in mania syndromes, there are also cases where individuals with Yang excess and Yin deficiency experience sudden mania just as they are about to sweat and resolve the illness, followed by recovery after sweating. This must be recognized, and no medication is required.
