Pang Anshi stated: Whenever pathogenic factors have passed through one meridian without resolution, accompanied by delirium (incoherent speech and confusion of mind), the Qimen (LR14) acupoint should be punctured, applying the reducing method according to the excess condition. The method for puncturing Qimen requires waiting until the pulse is wiry or floating-tight; only then will puncture ensure recovery. If puncture under other pulse conditions fails to alleviate symptoms, it is because the root cause—pathogens in the Liver—was not addressed. The Qimen acupoint is located directly below the nipple, on the lateral abdomen near the costal margin; the needle insertion depth is one cun.
In cold damage with fever, aversion to cold with shivering, where the patient is extremely thirsty and desires water, the abdomen will inevitably become full. With dysuria and spontaneous sweating, if the disease is about to resolve, this indicates Liver Qi overacting on the Lung, termed "Zong" (Vertical Overaction); Qimen should be punctured.
Abdominal fullness accompanied by delirium, with a floating and tight pulse at the Cun Kou position, indicates Liver Qi overacting on the Spleen, termed "Heng" (Horizontal Overaction); Qimen should be punctured.
Hematochezia accompanied by delirium indicates heat entering the Blood Chamber (referring to the uterus or Chong and Ren meridians). If only sweat appears on the forehead, Qimen should be punctured, applying the reducing method according to the excess condition; subsequent generalized sweating will lead to recovery.
In women suffering from Wind Strike with fever and aversion to cold, if menstruation occurs coincidentally, and after seven to eight days the heat subsides, the pulse becomes slow, and the body turns cool, yet there is fullness in the chest and hypochondrium resembling Chest Binding (rigid fullness and pain in the epigastrium/chest), accompanied by delirium, this indicates heat entering the Blood Chamber. Qimen should be punctured, selecting points based on the excess condition.
When Taiyang and Shaoyang diseases occur simultaneously, presenting with headache or dizziness, sometimes feeling like Chest Binding, and hardness in the epigastric region, Feishu (BL13) and Dazhu (BL11) should be punctured and reduced. Sweating must be strictly avoided. Inducing sweating will cause delirium; delirium leads to a wiry pulse. If delirium persists for five days, Qimen should be punctured.
For throat obstruction (sore throat and swelling due to blockage) caused by cold damage, puncture the Hand-Shaoyin Heart Meridian. The artery behind the wrist on the ulnar side (near the small finger) is the location; insert three fen and apply tonification. (Additionally, Tongli (HT5), two points located one cun posterior to the wrist crease, belong to the Hand-Shaoyin Heart Meridian and treat heat-induced throat obstruction; insert three fen, moxibustion three moxa cones.)
Cold damage causing abdominal fullness in pregnant women, inability to urinate, and edema below the waist resembling Water-Qi disease, occurring at seven months of pregnancy when the Taiyin Spleen Meridian governs, is due to Excess Heart Qi. Laogong (PC8) and Guanyuan (CV4) should be punctured and reduced; recovery follows once urination becomes smooth. (Laogong is in the center of the palm, found by curling the ring finger; Guanyuan is three cun below the umbilicus.)
Excessive drinking of water during cold damage leading to abdominal distension and asthma should be treated by puncturing Zhongwan (CV12). (Located three cun below Jiutui (CV15).)
Cold damage with anhidrosis, spinal rigidity, throat closure with irritability and fullness, should be treated by puncturing Dazhu (BL11). (Dazhu is located bilaterally one cun five fen lateral to the spinous process of the first thoracic vertebra; insert one cun and reduce.)
Residual heat from cold damage that has not fully cleared, presenting with dry skin, should be treated by puncturing Quchi (LI11). (Located at the lateral end of the transverse cubital crease when the elbow is flexed; needle may penetrate downward, apply reduction.)
In heat diseases where sweating occurs but the pulse remains harmonious, indicating treatment via sweating is possible, select Yuji (LU10), Taiyuan (LU9), Dadu (SP2), and Taibai (SP3). Reduction eliminates heat; tonification induces sweating. If sweating is excessive, select the point at the transverse crease above the ankle to stop it. (Yuji is in the scattered vessels medial to the base of the thumb; Taiyuan is in the depression posterior to the wrist; Dadu is in the depression posterior to the base of the great toe; Taibai is in the depression inferior to the medial malleolus bone. The transverse crease above the ankle does not specify a name but refers to Fuliu (KI7), two cun above the medial malleolus, which treats chills/fever in bones and profuse sweating.)
In heat diseases lasting seven to eight days, with abnormal pulsation, wheezing, and dizziness, urgent puncture is required. As sweat is about to emerge naturally, shallowly puncture the space between the digits of the hand. (This is Hegu (LI4).)
Heat diseases initially presenting with chest and hypochondriac pain, and restlessness of the hands and feet, should be treated by puncturing the Foot-Shaoyang Gallbladder Meridian and tonifying the Foot-Taiyin Spleen Meridian. (According to cold damage theory, one must avoid patterns of Earth failing and Wood attacking. Here, Foot-Shaoyang Wood is pathogen-invaded and tends to overact on Spleen Earth; thus, it is appropriate to reduce Qiuxu (GB40) of the Foot-Shaoyang and tonify Taibai (SP3) of the Foot-Taiyin. *Su Wen* states: "Tonify Taiyin." This is correct. Quan Yuanqi's commentary in *Tai Su* suggests Hand-Taiyin Lung Meridian, arguing that since the Lung meridian emerges from the axilla, chest/hypochondriac fullness/pain results. Another view cites deficiency requiring seeking sinews in the Liver, not Metal, favoring Hand-Taiyin. However, having stated one should not seek Metal, why seek it again? This is unlikely. Since Wood has been reduced, tonifying Metal is logically inconsistent as Metal would further constrain Wood; therefore, tonifying Spleen Earth is undoubtedly correct.)
Heat diseases initially presenting with arm pain should be treated by puncturing the Hand-Yangming Large Intestine Meridian and Hand-Taiyin Lung Meridian; sweating stops the condition. (Lieque (LU7), the Luo-connecting point of Taiyin, is 2.5 cun above the wrist; insert seven fen. Shangyang (LI1), the Yangming Jing-well point, is at the radial corner of the nail of the index finger, distance from nail like a leek leaf width; insert three fen.)
Heat diseases beginning in the head should be treated by puncturing the Taiyang points in the neck to induce sweating. (Tianzhu (BL10) is in the depression lateral to the major tendons at the nape hairline; insert five fen, apply reduction.)
Heat diseases initially presenting with dizziness, fainting, and fever, with chest and hypochondriac fullness, should be treated by puncturing the Foot-Shaoyin Kidney Meridian and Foot-Shaoyang Gallbladder Meridian. (Yongquan (KI1) of Foot-Shaoyin is in the depression at the sole of the foot, in the hollow when the toes are curled; insert seven fen, apply reduction, ensuring no bleeding. Xiaxi (GB43) of Foot-Shaoyang is in the depression anterior to the joint between the fourth and fifth metatarsal bones.)
Heat diseases beginning with pain in the lower legs should first be treated by selecting Foot-Yangming Stomach Meridian points to induce sweating. (Xianggu (ST41) is in the depression posterior to the joint between the first and second toes; insert five fen, apply reduction.)
Heat diseases lasting three days without sweating, accompanied by fear, palpitations, chest and hypochondriac pain, inability to turn the body, hematochezia, hematuria, persistent epistaxis, rebellious Qi with vomiting and hiccups, irritability, thirst, and inability to eat or drink, should be treated by puncturing Laogong (PC8). (Located in the center of the palm; insert five fen, apply reduction.)
