There is a type of warm disease where the patient sweats, then immediately fever returns with a restless and rapid pulse. The sweating does not alleviate the pulse's intensity. Accompanied by delirious speech and inability to eat, this disease is named "Yin-Jiao" (Intersection of Yin and Yang). It presents three signs of death without showing any sign of life.
(In years of Yin, Shen, Si, Hai, Chen, Xu, Chou, and Wei, this syndrome occurs.) In warm diseases, if upon onset there is shortness of breath, slight chest oppression, consciousness remains relatively clear, but the pulse at Cun and Chi positions is reversed, it indicates death.
(In years of Zi, Wu, Mao, and You, this syndrome occurs.)
Generally, for warm disease patients after two or three days, if there is body heat, rapid pulse, headache, but normal diet, and the pulse is straight and rapid, death occurs in eight days; if after four or five days there is headache, rapid pulse, tendency to vomit, and the pulse is fine, death occurs in twelve days; this disease is difficult to treat. If after eight or nine days the pulse is not rapid, the body is not painful, eyes are not red, complexion unchanged, but diarrhea occurs instead, and the pulse comes rapidly (Die-Die), pressing on fingers yields no elastic rebound, occasionally becoming large, with hardness below the heart, death occurs in seventeen days. (If there is no hardness below the heart, survival is possible.)
Drinking alcohol causing Yin-Yang disharmony and relapse leads to certain death; consuming cooked noodle wine, leeks, mushrooms, eels, bean powder, dog meat, mutton, intestines, blood, raw fruits, oily and fatty foods will all cause fever, jaundice, and unrecoverable diarrhea. Various forms of labor can cause relapse, which may still be treatable, but sexual exhaustion often leads to death.
Pang Anshi said: The Dong Mai (Moving Pulse) appears above and below the Guan position, lacking head and tail, trembling violently, named "Dong". Yang movement causes sweating; Yin movement causes fever. The Guan position occupies six fen; the front three fen are Yang, the rear three fen are Yin. If Yang connects to Cun and moves while Yin remains static, according to the method, there should be sweating leading to resolution. Su Wen states: "Yang added to Yin is called sweat." If Yin connects to Chi and moves while Yang remains static, fever occurs; Su Wen states: "Roughness at Chi indicates internal heat." If after profuse sweating, the form is cold and aversion to cold exists, this is damage to the San Jiao (Triple Burner), indicating a fatal condition. If the pulse is soft and empty upon pressure, resolution occurs through chills and sweating; if the pulse is forceful upon pressure, resolution occurs through restless sweating; if the pulse is extremely weak, one must undergo sweating, vomiting, or purging therapies. Without body fluids to generate sweat, recovery depends on the self-harmonization of Yin and Yang.
Fuyang is located between the big toe and the second toe, ascending five cun, representing the Foot-Yangming Stomach Meridian pulse point. (Named Chongyang Acupoint.)
Shaoyin pulse is located behind the medial malleolus of the foot, in the depression of the arterial pulse above the calcaneus, representing the Foot-Shaoyin Kidney Meridian pulse point. (Named Taixi Acupoint.)
Zhang Zhongjing said: "Pressing Cun does not reach Chi, holding the hand does not reach the feet." This means physicians rely solely on Cun Kou pulse diagnosis and do not examine Chongyang and Taixi pulses.
Pang Anshi said: Cun Kou pulse floating and large; floating indicates deficiency. If the physician incorrectly uses purgation, it is blaming deficiency while seeking excess (mismanagement); large indicates lack of blood. If the physician gives cold water to induce profuse sweating, it is guarding emptiness while forcing blood. This causes Fuyang pulse to also become floating and empty. Empty cold struggling together results in dysphagia; if Fuyang pulse is not floating but merely slippery, it indicates stomach cold resulting in hiccups.
Pang Anshi said: Fu Qi Bing (Disease of Latent Pathogenic Qi) refers to sudden cold pathogen attacking the person out of season, with toxic qi lying latent in the Shaoyin Meridian. Initially asymptomatic, it manifests after ten days or a month. The pulse becomes weak. According to the method, throat pain resembling injury appears first, followed by diarrhea. Throat pain is treated primarily with Banxia Guizhi Gancao Tang (Pinellia, Cinnamon Twig, and Licorice Decoction); diarrhea with various syndromes is treated primarily with Tongmai Sini Tang (Unblocking the Pulse and Reversing Collapse Decoction). (The formula is found in the Shaoyin Syndrome section.) This disease improves within two or three days; ancient texts refer to this as Kidney Shanghan (Cold Damage to the Kidney).
Herbs: Banxia (Pinelliae Rhizoma), Guizhi (Cinnamomi Ramulus), Gancao (Glycyrrhizae Radix et Rhizoma), Shengjiang (Zingiberis Rhizoma Recens) (each 1 liang).
Chop them, add three sheng of water, boil to obtain one sheng. Take one cup per dose, sip slowly.
Pang Anshi said: Gentlemen nourish Yang in Spring and Summer, and nourish Yin in Autumn and Winter, conforming to the rigidity and flexibility of Heaven and Earth. When the climate is warm, regulation should employ cooling methods; when the climate is hot, regulation should employ cold methods. Cooling and cold must be appropriate, not excessive, thus supporting Yin Qi to nourish Yang Qi. When the climate is cool, regulation should employ warming methods; when the climate is cold, regulation should employ hot methods. Warming and hot must be appropriate, not excessive, thus supporting Yang Qi to nourish Yin Qi. When Yin and Yang mutually nourish, human Qi remains peaceful. Some who favor health preservation wear clothes to avoid wind even in intense summer heat and consume only hot food, leading to many cases of jaundice and blood depletion. During intense winter cold, when warmth is suitable, taking cold medicines leads to many cases of vomiting, diarrhea, and abdominal pain. These stem from vulgar rumors mistakenly believed as truth, often leading to premature death without realization, which is deeply sorrowful. This is the method for ordinary people to regulate their lives. However, those with chronic heat or entrenched cold must follow their specific nature.
