After the Taiyang disease was treated with sweating, the body remained burning hot; this is called Wind-Warmth (Fengwen). The pulse at both the Cun and Chi positions is floating, there is spontaneous sweating, heaviness of the body, somnolence, snoring, and difficulty in speaking. (Weirui Tang) In Spasm (Jingzheng), the jaw is clenched and speech is impossible. For Gang-Jing (rigid spasm), Gegen Tang is used; for Rou-Jing (flexible spasm), Guizhi Tang with Gualou is used. In Heat Disease (Rebing), aphonia occurs. If no sweat appears after three or four days, death ensues. Fire pathogen punishing Metal (Lung) causes hoarseness. (Huangqin, Huanglian, Gancao, Jiegeng, Zhimu, Huanglian, Maidong, Wuweizi) Exuberant Wind-Heat obstruction leads to cough and hoarse voice. (Jingjie, Fangfeng, Gancao, Jiegeng, Bohe, Hehua, Fenxin, Zhimu)
(This refers to a red face. Yang Qi is constrained on the exterior, hence the facial redness. Although it stems from heat, there are no indications for purgation in the Six Channels. There are also Yin-Cold patterns where extreme Water mimics Fire; these must be distinguished by other symptoms.)
Taiyang disease presents like malaria, with a faint pulse, aversion to cold, and an abnormal red-hot color on the face, accompanied by itching of the body. (Guizhi Mahuang Ban Ge Tang, administered in small doses to induce slight sweating) If the sweating treatment for Taiyang disease was incomplete, the condition may transfer to Yangming. There follows slight continuous sweating without aversion to cold. If the Taiyang symptoms have not ceased, purgation is contraindicated; instead, mild sweating should be induced. If the face is persistently bright red due to Yang Qi constrained on the exterior, sweating therapy is indicated. (Mahuang Tang) Facial redness in Yangming disease is not an indication for attack/purgation. (Gegen Tang) In Shaoyin disease, with slight diarrhea of undigested food, cold extremities, and a faint pulse, yet without aversion to cold, Tongmai Sini Tang with Congbai is used. A face red as brocade, sore throat, and expectoration of pus and blood indicate Yang Du (Yang Toxin). (Yangdu Shengma Tang)
Qi rushing upward causing heart pain, hunger without desire to eat, and vomiting of roundworms characterize Jueyin disease. (Guizhi Baizhu Fuling Tang, Lizhong Anhui San) When quiet but occasionally irritable, this indicates interior Cold. The roundworms ascend into the diaphragm, then stop after a moment. Eating induces vomiting and further irritability, as the worms emerge upon smelling food odor; vomiting of worms is expected. (Wumei Wan) If a patient with a Cold pattern undergoes repeated sweating, leading to Cold in the Stomach, vomiting of roundworms will certainly occur. (First administer Lizhong Tang, then Wumei Wan)
Note: Vomiting roundworms primarily indicates Stomach Cold, which is common knowledge. However, there are also cases belonging to Yang patterns, such as a洪大数实 (surging, large, rapid, and full) pulse, thirst, constipation, dark urine, or yellow coating, all of which respond to cold-natured agents. One must not adhere rigidly to one view. In cases of vomiting roundworms, Gancao should not be taken, nor should sweet foods be consumed. This is because roundworms become active with sweetness, calm with bitterness, stop with sourness, and subside with pungency.
