Use realgar (Realgar) processed by water levigation. For males, blow into the left nostril; for females, into the right nostril. Alternatively, take a piece of transparent realgar weighing five qian, wrap it in silk, and tie it to the top of the head (Baihui acupoint area); this is excellent.
Drink water infused with Cyatheae Spicam (Guanzhong). Soak Adzuki beans (Chixiaodou) together with glutinous rice in a water vat, and use this water daily.
Shanghan (Cold Damage) involves a dangerous and severe disease progression with numerous and complex symptoms. Without inquiring into the cause, examining the symptoms, and correctly naming the disease, misdiagnosis is inevitable. Upon arriving at the patient's home, one must first ascertain: whether they have just eaten a full meal, or if their constitution has always been deficient, or always excess, or if they have had other chronic conditions, or if they have never had other conditions. This constitutes the method of inquiring into the cause. The manifestations and symptoms of the Six Channels (Liu Jing) must be carefully examined for each. In Taiyang disease (Taiyang Disease), presenting with fever, aversion to cold, pain in the head and nape, stiffness in the waist and spine, nausea with tension, body ache, and bone pain, these are Taiyang exterior syndromes, representing the Biao (Branch/Manifestation) aspect. If there is internal heat, irritability, thirst, and difficult urination, these are Taiyang interior syndromes, representing the Ben (Root/Essence) aspect. A pulse that is floating, tight, and forceful indicates Shanghan (Cold Damage); floating, slow, and weak indicates Shangfeng (Wind Strike); a quiet pulse indicates no transmission; a restless and vigorous pulse indicates impending transmission. In Yangming disease (Yangming Disease), presenting with body heat, slight aversion to cold, pain in the forehead and eyes, dry nose, and insomnia, these are Yangming exterior syndromes, representing the Biao aspect. If there is irritability, thirst with desire to drink, sweating, and aversion to heat, these are Yangming interior syndromes, representing the Ben aspect. If there is tidal fever, spontaneous sweating, delirium, thirst, abdominal hardness and fullness, macules, jaundice, mania, and constipation, this is Zheng Yangming Stomach Excess, representing the Fu (Hollow Organ) disease. A slightly surging pulse indicates Biao; a surging and rapid pulse indicates Ben; a deep and rapid pulse indicates Excess. In Shaoyang disease (Shaoyang Disease), presenting with pain in the temples and dizziness, chest and hypochondriac pain, deafness, alternating chills and fever, vomiting, and bitter taste, this is Shaoyang Channel disease. Its channel is located between the exterior and interior, and its pulse is both wiry and rapid. [Note: Text says "Taiyang" here but context implies transmission to Taiyin] When strong fever, dry throat, or spontaneous diarrhea without thirst occur, this is Heat Evil from the Yang channels transmitting into Taiyin, representing the Biao aspect. If there is dryness, thirst, jaundice, red urine, and constipation, this is Taiyin Ben disease. If upon initial onset, there is no headache, thirst, or fever, but rather cold stool, abdominal distension, fullness, pain, vomiting, diarrhea, and hiccups, this is Taiyin Zhi Zhong (Direct Invasion) Ben disease. If initially there is no fever, only distension, belching, and pain, this is injury from cold and raw food. A deep and slow pulse indicates Biao; a deep and solid pulse indicates Ben; both Zhi Zhong and internal cold injuries present with deep and fine pulses. In Shaoyin disease (Shaoyin Disease), presenting with a dry tongue, dry mouth, delirium, thirst, and constipation, this is Heat Evil from Yang channels transmitting into Shaoyin, representing the Biao aspect. If there is body heat, flushed face, cold feet, and a deep pulse, this is Kidney Channel Jia Yin Shanghan (Cold Damage with Yin involvement), where both Ben and Biao are affected. Restlessness due to Yin excess, desiring to sit in a muddy well, wanting to drink but unable to retain fluids, with a flushed face, cold feet, and deep pulse, indicates Extreme Yin generating restlessness, which is Ben disease. If there is a flushed face, cold feet, irritability, desire to drink, removing clothes and blankets, with a rapid, large, and forceless pulse, this is Deficient Yang hidden within Yin, affecting both Biao and Ben. If upon initial onset, there is no headache, fever, or thirst, but rather cold limbs, curling up, pain in the umbilicus and abdomen, vomiting, diarrhea, and shivering, this is Kidney Channel Zhi Zhong Ben disease. If accompanied by severe colic in the lower abdomen, or vomiting and diarrhea, even to the point of tongue retraction and testicular retraction, this is Jia Yin Zhong Han (Cold invasion with Yin involvement), also Ben disease. A deep, solid, and forceful pulse indicates Yang Evil Biao disease; a deep, fine, and forceless pulse indicates Direct Invasion Cold syndrome; a rapid, large, and forceless pulse indicates Deficient Yang hidden within Yin; a deep pulse indicates Jia Yin Shanghan. In Jueyin disease (Jueyin Disease), alternating chills and fever resembling malaria indicates Yang Evil transmitting into Jueyin, representing the Biao aspect. If there is tongue retraction, testicular retraction, irritability, fullness, constipation, thirst, and hands and feet alternating between warm and cold, this is Yang Evil transmitting into Jueyin, representing the Ben aspect. If upon initial onset, there is no headache, fever, or thirst, but rather aversion to cold, cold limbs, pain in the abdomen and genital region, vomiting saliva, diarrhea, tongue retraction, and testicular retraction, this is Jueyin Zhi Zhong Ben disease. A floating and slow pulse indicates Biao; a deep and solid pulse indicates Ben; a fine and soft pulse indicates Direct Invasion. Headache with tension, body heat with aversion to cold, abdominal pain with vomiting, and both Qikou (Radial artery) and Renying (Carotid artery) pulses being vigorous indicate Jia Shi Shanghan (Cold Damage with Food Retention). Body heat with aversion to cold, headache with slight sweating, mental fatigue, and reluctance to speak indicate Laoli Shanghan (Cold Damage due to Overexertion). Body heat with aversion to cold, vague headache, wheezing, coughing, and irritability, with a tight and vigorous left pulse and a surging and slippery right pulse, indicate Jia Tan Shanghan (Cold Damage with Phlegm). Body heat with aversion to cold, pain in the head and hypochondrium, and Qi stagnation with discomfort indicate Jia Qi Shanghan (Cold Damage with Qi Stagnation). Chest, hypochondriac, and abdominal pain that does not move after pressing, headache, irritability, thirst, body heat, and aversion to cold indicate Xue Yu Shanghan (Cold Damage with Blood Stasis). Furthermore, Shang Shu (Summerheat Injury), Shang Shi (Dampness Injury), Wen Bing (Warm Diseases), Re Bing (Heat Diseases), Dong Wen (Winter Warmth), Feng Wen (Wind Warmth), Wen Du (Warm Toxicity), Wen Nue (Warm Malaria), Feng Shi (Wind-Dampness), Shi Wen (Damp-Warmth), Yi Bing (Epidemic Diseases), and Jing Bing (Spasm Disease) must all be distinguished accurately and named correctly. Treatment should be administered according to the name, thereby ensuring every action is appropriate. For those in the exterior, apply sweating and dispersing methods; for those in the interior, apply draining and purgative methods; for those between exterior and interior, apply harmonizing methods; for those in the upper jiao, apply emetic methods; for those sinking, apply lifting and raising methods; for Jia Yin cases, apply tonification; for Zhi Zhong cases, apply warming. Regarding exterior-interior, yin-yang, deficiency-excess, cold-heat, and root-branch, one must distinguish them as clearly as black and white, with absolutely no confusion. As symptoms change, treatment should not be rigid. Handling severity and urgency without error demonstrates true medical knowledge, worthy of wielding the power over life and death.
