胡(八)合黄连解毒汤(二十一)合服。诸小柴胡汤证后病不解,表里热势更甚,而心下急郁微烦;
Syndromes presenting with both exterior and interior manifestations: Syndromes where the pathogen is partly in the exterior and partly in the interior refer to the previous exterior and interior syndromes, where the pathological conditions are intermingled. In cases where one wishes to induce sweating but there are interior symptoms, or wishes to purge but the exterior syndrome has not yet resolved; inducing sweating is contraindicated, and inducing vomiting is also contraindicated; the treatment method should be harmonization. For wind-strike (Shangfeng), use Baihu Tang (White Tiger Decoction) (22); for cold damage (Shanghan), wind-strike (Zhongfeng), or simultaneous contraction of two meridians (Lianggan), use Xiaochaihu Tang (Minor Bupleurum Decoction) (9). One method applies regardless of wind-cold, wind-heat, summerheat, or dampness: combine Liangge San (Cooling the Diaphragm Powder) (23) and Tianshui San (Heavenly Water Powder) (52) into a single dose, decocted and administered. If exterior heat is predominant and interior heat is slight, use one part Tianshui San and half part Liangge San; if interior heat is predominant and exterior heat is slight, use one part Liangge San and half part Tianshui San. If exterior heat is extreme with only slight interior heat, accompanied by body pain, headache, dizziness, or vomiting, where sweating, vomiting, and purging are contraindicated, combine Tianshui San and Liangge San for harmonization. If harmonization fails to reduce the fever, use Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity); if both exterior and interior heat are mild, use Wuling San (Five-Ingredient Poria Pill) (24); if both exterior and interior heat are vigorous, use Daichaihu Tang (Greater Bupleurum Decoction) (8) with mild purging; for more severe cases, combine Daichaihu Tang with Dachengqi Tang (Major Order the Qi Decoction) (10) for purging to simultaneously eliminate exterior and interior heat. If the condition does not resolve after taking Shuangjie San (Double-Resolution Powder) (54), it indicates that the disease has transmitted; three days later, the pathogen is in the interior, and the treatment method should be purging. However, if purging is performed too early, exterior heat will enter the interior due to deficiency, leading to syndromes such as Jie Xiong (Chest Binding), Xu Pi (Empty Fullness), Ao Nao (Restlessness), and Huang Du (Jaundice). Mild cases will become critical, and critical cases will lead to death. It is advisable to use mild medications to disperse the exterior and harmonize the interior; the disease may resolve through sweating or through the harmonization of Qi without sweating. Use Xiaochaihu Tang (9), Liangge San (23), and Tianshui San (52) combined as the main treatment. For pathogens in the half-exterior half-interior stage, use Xiaochaihu Tang and Liangge San combined for harmonization, or combine Xiaochaihu Tang with Jiedu Tang (21). If the fever persists after administration, use Daichaihu Tang (8) combined with Sanyi Chengqi Tang (Three-One Order the Qi Decoction) (13). If exterior and interior heat are both mild, located in the half-exterior half-interior, and interior heat is slight, use Daichaihu Tang (8) combined with Huanglian Jiedu Tang (21). If the condition does not resolve after various Xiaochaihu Tang syndromes, with intensified exterior and interior heat, urgency and oppression in the epigastrium with slight irritability; or fever and sweating without resolution, epigastric fullness and hardness with vomiting and diarrhea; or Yangming disease with profuse sweating; or Shaoyin disease with watery diarrhea, epigastric pain, and dry mouth; or Taiyin disease with abdominal fullness and pain; or absence of exterior/interior syndromes but persistent fever for seven to eight days, where although the pulse is floating and rapid, it is solid, rapid, and slippery at the muscle level, all are suitable for Daichaihu Tang (8). By the seventh or eighth day, interior heat is intense while exterior heat is fading; even if the pulse is floating and rapid, use Sanyi Chengqi Tang (13) combined with Jiedu Tang for purging. If the indications for purging are not fully present and purging is contraindicated, use Baihu Tang (22) or Renshen Shigao Tang (Ginseng and Gypsum Decoction) (35). For a surging and restless pulse with slight interior heat where sweating is contraindicated, use Huanglian Jiedu Tang (21).
