If Wind-Warmth (Fengwen, a disease caused by warm pathogenic factors) is treated with purgation, it will inevitably lead to incontinence of urine. The patient will exhibit snoring, difficulty in speaking, and spontaneous sweating. (Zhang Zhongjing stated: In Wind-Warmth disease, the pulse is floating at all three positions (Cun, Guan, Chi); there is spontaneous sweating, heaviness of the body, excessive sleepiness, snoring nasal breathing, and difficulty in speech. If purgation is mistakenly applied, it leads to difficult urination, fixed gaze, and loss of control over urination and defecation.) In San-Yang combined disease (simultaneous involvement of the Taiyang, Yangming, and Shaoyang meridians), the body feels heavy, and involuntary enuresis may occur, which is a cause for concern. (Zhang Zhongjing stated: In San-Yang combined disease, symptoms include abdominal fullness, heaviness of the body, difficulty in turning the body, numbness of the mouth, facial greasiness, delirium, and enuresis. Mistaken diaphoresis worsens delirium; mistaken purgation causes sweating on the forehead and cold limbs. For cases with spontaneous sweating, Baihu Tang [White Tiger Decoction] is indicated.) Failure of the Lower Jiao (Lower Burner) Qi to return to its proper location also results in urinary incontinence. The above three conditions can be cured by treating according to the specific syndrome. (Zhang Zhongjing stated: When the Qi of the Lower Jiao does not reside in its proper region, urinary incontinence occurs. The aforementioned three syndromes can be resolved by treating them according to their respective presentations.) Sudden fixed gaze accompanied by manic speech indicates Kidney Exhaustion (Shen Jue); how long can life be sustained? (Zhang Zhongjing stated: Loss of control over urination and defecation, manic speech, and upward-turned fixed eyes indicate Kidney Exhaustion.)
Taiyin disease presents with abdominal fullness necessarily accompanied by intermittent pain. (Zhang Zhongjing stated: The manifestation of Taiyin disease includes abdominal fullness, vomiting, and inability to eat. Severe mistaken purgation leads to significant abdominal fullness and intermittent abdominal pain.) San-Yang combined disease also presents with abdominal fullness and heaviness of the body. (Zhang Zhongjing stated: In San-Yang combined disease, symptoms include abdominal fullness, heaviness of the body, and difficulty in turning the body.) Yangming disease presents with abdominal fullness, bitter taste in the mouth, and dry throat; if accompanied by slight asthma (wheezing), Xiaochaihu Tang [Minor Bupleurum Decoction] may be used. (Zhang Zhongjing stated: In Yangming Wind-Strike, symptoms include bitter taste, dry throat, abdominal fullness, slight asthma, fever, and a floating-tight pulse. Mistaken purgation leads to abdominal fullness and difficult urination.) During Gu-Dan (Grain Jaundice), the Stomach Qi should be regulated first. (Zhang Zhongjing stated: In Yangming disease, a slow pulse suggests impending Grain Jaundice. Mistaken purgation leads to abdominal fullness.) If tidal fever (Chao Re) is accompanied by constipation, do not use drastic purgation that would deplete the healthy Qi; instead, mildly harmonize the Stomach Fu organs using Chengqi Tang [Purgative Decoction]. (Zhang Zhongjing stated: In Yangming disease with a slow pulse, abdominal fullness, asthma, and tidal fever, Xiao Chengqi Tang [Minor Purgative Decoction] is indicated. It is also stated: For significant abdominal fullness without large bowel movement, Xiao Chengqi Tang is used to mildly harmonize Stomach Qi, avoiding drastic purgation.) After purgation, if restlessness and abdominal fullness occur, Zhizi Houpo Tang [Gardenia and Magnolia Bark Decoction] is appropriate. (Zhang Zhongjing stated: After purgation in Cold Damage disease, if restlessness, abdominal fullness, and restlessness in lying down or sitting up occur, Zhizi Houpo Tang is indicated.) After diaphoresis, Houpo (Magnolia Bark) preparations are most suitable. (Zhang Zhongjing stated: For abdominal distension after diaphoresis, Houpo Wuwu Tang [Five-Ingredient Decoction with Magnolia Bark] is indicated.) After emesis, Xiao Chengqi Tang should be used after careful differentiation. (Zhang Zhongjing stated: For abdominal fullness after emesis in Cold Damage disease, Xiao Chengqi Tang is indicated. This syndrome requires careful differentiation.) For Taiyin disease with abdominal fullness and intermittent pain, Guizhi Shaoyao Tang [Cinnamon Twig and Peony Root Decoction] is used; for severe excess pain, Dahuang Tang [Rhubarb Decoction] can treat it. (Zhang Zhongjing stated: In Taiyang disease, if the physician mistakenly applies purgation, leading to abdominal fullness and intermittent pain belonging to Taiyin, Guizhi Shaoyao Tang is used. For severe excess pain, a formula containing Dahuang [Rhubarb] is indicated.)
