I diagnosed this as *Ben Tun* (Panic Disease / Roaring Pig Syndrome). Because I wanted to test whether adding *Guizhi* (Cinnamon Twig) would be effective, I prescribed six qian of *Guizhi* and four qian of *Shaoyao* (White Peony Root), with other herbs proportioned accordingly. After one dose, there was no effect. After a second dose, there was still no effect. The patient's pain intensified, his body became more exhausted, and he could not even take rice water. I thought I could no longer use the patient as an experimental subject. Therefore, I reduced the *Guizhi* to four qian, making it equal in amount to the *Shaoyao*, and added five fen of top-grade *Rougui* (Cinnamon Bark). I instructed that the *Rougui* be stewed separately and taken mixed with the decoction. After one dose, the pain decreased significantly, the accumulated gas in the epigastrium and abdomen dispersed, and he belched frequently or passed foul flatus. After two more doses, the illness seemed to disappear. This experiment proved that *Guizhi* lacks this specific capability. Readers' doubts can now be resolved. Generally, *Guizhi* has a thin flavor and aroma, specializing in dispersing exterior pathogens. *Rougui* has a thick flavor and aroma, possessing great power to warm the interior. Using *Guizhi* to replace *Rougui* is of little benefit.
**Guizhi Jia Guizhi Tang (Cinnamon Twig Decoction Plus Cinnamon Twig)**: *Guizhi* (Cinnamon Twig) 6 qian, *Shaoyao* (White Peony Root) 4 qian, *Zhi Gancao* (Honey-fried Licorice Root) 3 qian, *Shengjiang* (Fresh Ginger) 2 qian, *Hongzao* (Jujube) 10 pieces. Note: In the original *Guizhi Tang*, the amounts of *Guizhi* and *Shaoyao* are equal. Here, *Shaoyao* is 4 qian and *Guizhi* is 6 qian; the extra 2 qian represents the increased dosage. Taking this for two doses yielded no effect.
**Guizhi Jia Rougui Tang (Cinnamon Twig Decoction Plus Cinnamon Bark)**: *Guizhi* (Cinnamon Twig) 4 qian, *Shaoyao* (White Peony Root) 4 qian, *Zhi Gancao* (Honey-fried Licorice Root) 3 qian, *Shang Rougui* (Top-grade Cinnamon Bark) 5 fen, *Shengjiang* (Fresh Ginger) 2 qian, *Hongzao* (Jujube) 10 pieces. Note: After one dose of this formula, the pain decreased significantly, and the upward rushing qi gradually calmed. After two more doses, full recovery was achieved.
**Case of Ben Tun in a Young Man with Weak Constitution**: A young man with a constitutionally weak body, sallow and dull complexion, occasionally suffered abdominal pain due to cold invasion. Initially, there was borborygmus in the abdomen and a sensation of moving qi; then dull pain, palpitations below the umbilicus, shifting left or right; finally, sudden upward attack, with qi rushing to the chest and epigastrium, intermittent. During attacks, hands and feet were cold, and sweat appeared on the forehead. He was treated with *Guizhi Tang* plus *Rougui*, recovering after four doses.
**Zhao Shuhua**, from Batou Bridge, Dongxiang, Huai'an, residing in Shanghai. His son, aged 19, had a constitutionally weak body, sallow and dull complexion resembling dirt-stained skin. His food intake was half that of normal people; he could not tolerate wind and cold, falling ill easily, and overeating also caused illness. His parents were very concerned. In July 1948, his son suddenly developed *Ben Tun* as described above. During attacks, he felt qi rising from below the umbilicus, rushing straight up to the chest, causing pain radiating to the chest and diaphragm. Severe cases involved gritting teeth, closing eyes, cessation of breathing, cold sweat on the forehead, cold hands and feet, and cyanosis of lips and nails, lasting 2-5 minutes. Then the accumulated gas would dissipate, breathing would normalize, limbs would warm, sweat would stop, and pain would vanish. These episodes occurred intermittently, 6-10 times daily, unresponsive to medicine. The family was panicked, considering it a strange disease.
Zhao consulted his relative, Mr. Li Xiyi, who referred him to me. Upon seeing the patient, I identified it as *Ben Tun*. I comforted the father and son gently, saying, "This condition is recorded in ancient medical texts. Modern doctors do not read these texts, hence their treatments fail." I promised a cure to bolster their confidence through psychological therapy. I prescribed **Guizhi Jia Rougui Tang** for them to take as directed. The next day, Zhao returned with his son, looking pleased, saying, "As you predicted, after one dose, symptoms lessened. Attacks were shorter and fewer." I advised continuing the same prescription. On the third day, I modified the original formula by removing *Guizhi* and adding *Fuling* (Poria) and *Baizhu* (Atractylodes Macrocephala Rhizome). Two doses brought stability.
**Guizhi Jia Rougui Tang**: *Guizhi* (Cinnamon Twig) 3 qian, *Shaoyao* (White Peony Root) 3 qian, *Zhi Gancao* (Honey-fried Licorice Root) 2 qian, *Shang Rougui* (Top-grade Cinnamon Bark) 5 fen, *Shengjiang* (Fresh Ginger) 1.5 qian, *Hongzao* (Jujube) 6 pieces.
**Formula Removing Guizhi and Adding Fuling and Baizhu**: *Shaoyao* (White Peony Root) 3 qian, *Zhi Gancao* (Honey-fried Licorice Root) 2 qian, *Shang Rougui* (Top-grade Cinnamon Bark) 5 fen, *Fuling* (Poria) 3 qian, *Baizhu* (Atractylodes Macrocephala Rhizome) 3 qian, *Shengjiang* (Fresh Ginger) 1.5 qian, *Hongzao* (Jujube) 6 pieces.
**Baihe Bing (Lily Disease) - Laborer's Lily Disease**: Laborers, after severe illness, become extremely exhausted, and righteous qi is hard to restore. Symptoms match those described in the *Jin Gui Yao Lue* (Essential Prescriptions from the Golden Cabinet) regarding *Baihe Bing*: "All vessels belong to one source, all suffer disease." Desire to eat but cannot; often silent; desire to sleep but cannot; desire to walk but cannot; appetite fluctuates between liking and disliking food odors; sensations of cold or heat without actual fever/chills; bitter taste in mouth, red urine; various medicines fail, and drugs cause severe vomiting and diarrhea; seems possessed by spirits; physical appearance is normal, pulse is slightly rapid. Additionally, there is hoarseness/inability to speak. Treated with *Baihe Dihuang Tang* (Lily and Rehmannia Decoction) modified. Recovery took four months.
**Patient Zhan Longchen**, from Gaoyou, a laborer in a factory, living at Jianmin Village, Zhongshan North Road. In Spring 1948, he contracted Shanghan (Cold Damage). Although the illness passed, his body remained extremely exhausted, and righteous qi recovered slowly. This resulted in sequelae of *Baihe Bing*, known in Western medicine as secondary neurasthenia. Previous treatments were ineffective. Factory doctors tested for hookworms, attributing weakness to infestation. Anthelmintics and purgatives were administered internally. However, after one dose, vitality worsened, and hoarseness developed immediately. Mr. Zhan was distressed, fearing death.
By June, Mr. Yang Xinghou, a relative, introduced me to treat him. I inquired about the history and examined current symptoms, finding them identical to *Baihe Bing* in the *Jin Gui Yao Lue*, except for the hoarseness, which is not mentioned in the classic. I realized this arose from using anthelmintic/purgative drugs. Given the chronic nature and deficiency, quick results were impossible. If the patient lacked confidence or patience, he might switch doctors midway, ruining progress ("He who travels a hundred li considers ninety as half"). I first used psychological therapy to strengthen his confidence, telling him full recovery required six months. I showed him the original text of the *Jin Gui Yao Lue*, commentaries, and my own *New Interpretations of Jin Gui Yao Lue* to solidify his trust. Following Zhang Zhongjing's methods, I prescribed the following. Four prescriptions were adjusted, primarily based on Zhang Zhongjing's *Baihe Dihuang Tang*, supplemented by modified herbs. After two months of continuous use, progress was evident, and voice improved.
In August, new lily bulbs were available. I instructed him to eat fresh lilies, boiled with white sugar, morning and evening, as a snack. Medication was taken for three days, followed by a two-day break. After four months, complete recovery was achieved.
The factory urged him to return to work upon hearing of his recovery, but he remained fearful and asked me. I advised resting for another one or two months. The factory suspected deception and sent him to the factory doctor for certification. The factory doctor still detected hookworms and ordered more anthelmintics and purgatives. The patient reluctantly complied. One dose caused four bowel movements, and previous symptoms recurred completely, including hoarseness. The factory sent him to Shanghai No. 11 People's Hospital. The physician suspected syphilis. The patient denied this and left angrily.
He sought my treatment again. The recurrence differed from before. Helplessly, I used psychological therapy again to remove his fear. I prescribed modifications of the previous formulas. Slowly, effects appeared. Three more adjustments led to gradual full recovery. By June 1955, his body and voice fully normalized. This difficult case demonstrates that without adapting Zhang Zhongjing's classical formulas, danger would have been imminent. Treating this illness is like repairing ruins after war: the focus is on comforting refugees and slowly rebuilding, not attacking enemies. There are no major disease enemies left in these ruins. The full case records are listed below, breaking the usual format of this book.
**Initial Treatment Record: Zhan Longchen, June 6, 1954**: Laborers injure qi; post-illness imbalance makes righteous qi hard to restore. Digestion affects stomach/intestines; panting affects lungs. Chronic course leads to neurasthenia, restlessness, and disrupted sleep/posture, matching *Baihe Bing* in *Jin Gui Yao Lue*. Misuse of purgatives caused loss of voice, indicating severe deficiency. Plan: Treat slowly, eliminate worry, accept fate, and find joy.
Steamed Baihe (Lily Bulb) 4 qian, Raw Dihuang (Rehmannia Root) 5 qian, Daipi Qi (Astragalus with Peel, Honey-fried) 3 qian, Huai Shanyao (Chinese Yam) 4 qian, Yun Fuling (Poria with Cinnabar Coating) 3 qian, Da Maidong (Ophiopogon Tuber) 3 qian, Hongzao (Jujube) 10 pieces, Chen Xiaomai (Old Wheat) 1 liang (decocted first).
**Second Visit: June 27, 1954**: Previous modified *Baihe Dihuang Tang* showed improvement. Appetite increased, voice slightly louder, complexion better. Stool remains dry due to unrecovered fluids, not worrying. Plan: Seek further progress, aid with diet therapy. Once fluids recover and bowels clear, symptoms will vanish.
Steamed Baihe (Lily Bulb) 4 qian, Shengdi (Prepared Rehmannia Root) 4 qian, Daipi Qi (Astragalus with Peel) 3 qian, Baizi Ren (Biota Seed) 3 qian, Huai Shanyao (Chinese Yam) 3 qian, Nan Shashen (Adenophora Root) 3 qian, Da Maidong (Ophiopogon Tuber) 3 qian, Chen Xiaomai (Old Wheat) 1 liang (decocted first).
**Third Visit: July 23, 1954**: Continued medication showed improvement. Complexion brighter, gait stronger, but voice not fully restored. Recent tongue coating slightly thick with turbid stagnation; stool still unsatisfactory. Plan: Seek further progress.
Nan Shashen (Adenophora Root) 3 qian, Daipi Ling (Poria with Peel) 3 qian, Fei Zhimu (Anemarrhena Rhizome) 3 qian, Huai Shanyao (Chinese Yam) 3 qian, Daipi Qi (Astragalus with Peel, Honey-fried) 2 qian, Huomaren (Hemp Seed) 3 qian, Tu Chao Baizhu (Atractylodes Stir-fried with Soil) 3 qian, Da Maidong (Ophiopogon Tuber) 3 qian, Xingren Ni (Apricot Kernel Paste) 3 qian. Note: Fresh lilies were available, so he ate boiled lilies with sugar twice daily; thus, steamed lily bulb was omitted from the formula.
**Fourth Visit: August 7, 1954**: Originally chronic neurasthenia, greatly improved by treatment. Recently, hot weather and lingering summer-heat pathogen caused chest irritability and stuffiness. Treatment should address the branch (symptoms) rather than the root.
Xiangru (Elsholtzia Herb) 2 qian, Zhi Banxia (Pinellia Tuber Processed) 2.5 qian, Binglang (Areca Nut) 3 qian, Shenqu (Medicated Leaven) 3 qian, Huoxiang (Patchouli Herb) 3 qian, Kourén (Cardamom Seed) 1.5 qian, Loupi (Trichosanthes Peel) 3 qian, Sugeng (Perilla Stem) 3 qian, Huafen (Trichosanthes Root) 3 qian, Shengjiang (Fresh Ginger) 2 slices, Zhuye (Bamboo Leaf) 40 pieces. Note: After three doses, new pathogenic factors were expelled. Continue with the third formula until early October, when illness cured and voice restored.
**Retreatment Record: Zhan Longchen, February 14, 1955**: Last year's severe *Baihe Bing* (neurasthenia) persisted. Modified *Baihe Dihuang Tang* led to gradual improvement, curing in about four months. However, chronic weakness meant righteous qi was not fully restored. Recently, another doctor diagnosed hookworms and used anthelmintics/purgatives, causing relapse and loss of voice. Plan: Adjust formula, but repeated errors make treatment harder.
Steamed Baihe (Lily Bulb) 4 qian, Shengdi (Prepared Rehmannia Root) 4 qian, Da Maidong (Ophiopogon Tuber) 3 qian, Yuanzhi Rou (Polygala Root Flesh) 2.5 qian, Zhu Fuling (Poria with Cinnabar) 3 qian, Daipi Qi (Astragalus with Peel, Honey-fried) 3 qian, Suanzaoren (Ziziphus Seed) 3 qian, Chen Xiaomai (Old Wheat) 1 liang (decocted first).
**Second Visit: February 29, 1955**: Medication stabilized sleep and appetite. Legs feel weak, stool soft/loose, voice not restored. Plan: Seek progress. Eliminate worry, find joy in everything for faster results.
Steamed Baihe (Lily Bulb) 4 qian, Shengdi (Prepared Rehmannia Root) 4 qian, Nan Shashen (Adenophora Root) 3 qian, Yuanzhi Rou (Polygala Root Flesh) 2.5 qian, Buguzhi (Psoralea Fruit) 3 qian, Dan Yuzhu (Polygonatum Rhizome) 3 qian, Suanzaoren (Ziziphus Seed) 3 qian, Zhu Fuling (Poria with Cinnabar) 3 qian, Daipi Qi (Astragalus with Peel, Honey-fried) 3 qian, Chen Xiaomai (Old Wheat) 1 liang (decocted first). Note: Taken until end of April, body recovered, voice strengthened. By mid-May, voice fully restored.
**Chronic Malaria (Chaotic Pattern)**: Patient initially had a cold, progressing to malaria. Five doctors failed to cure. Patient had opium addiction, worsening condition over six months. Condition became chaotic, irregular attacks. Over another six months, appetite decreased, emaciation worsened. I used **Shouwu Guzhi Tang** (Polygonum Multiflorum and Psoralea Decoction). Two doses reduced illness and increased appetite; six doses cured completely. Followed by **Buzhong Yiqi Tang** (Tonify the Middle and Augment the Qi Decoction) for health restoration.
In 1941, I served as Dean of Shanghai College of Traditional Chinese Medicine. Student Weng Guoying lived in Sanbeitang, Pudong. Her father had a cold turning into malaria. Local doctors failed after five changes. He lost faith in medicine. He had an opium addiction since youth, losing weight. Now over 50, suffering malaria for six months, his emaciation was evident.
After months of neglect, symptoms didn't worsen but pattern became chaotic: daily, every two days, every three days, every five-seven days, or multiple times a day. Attack times varied. Though attacks lightened, they didn't stop. Total duration: 1 year 7 months. Exhaustion increased, appetite dropped, surviving only on lotus paste and thin porridge.
During autumn break, Weng persuaded her father to come to Shanghai. He refused due to distrust. During winter break, she wrote urging him. After exams, they returned. Weng told me details beforehand. Clinical exam: emaciated, sunken abdomen, weak breathing, blue sclera, dry peeling skin. Tongue deep red, dull, black at root and center, indicating Kidney Yin deficiency exacerbated by opium burning fluids. Attack pattern matched previous description. I decided on **Shouwu Guzhi Tang**, two doses, then re-evaluate. Comforted him to build trust and spirit, aiding treatment. After two doses, only one mild, short attack in two days; appetite improved. Stool remained dry. Added Rongrong (Cistanche) and Danggui (Angelica Sinensis); replaced Rougui with Fupian (Prepared Aconite). Four doses cleared stool, increased appetite, and eliminated chills/fever. Decided to tonify heavily, prescribing **Buzhong Yiqi Tang** for ten doses to support righteous qi. Father and daughter were delighted, returning to Pudong for New Year.
**Shouwu Guzhi Tang**: Zhi Heshouwu (Prepared Polygonum Multiflorum Root) 6 qian, Pozhizi (Psoralea Fruit) 4 qian, Da Shudi (Prepared Rehmannia Root) 3 qian, Shufupian (Prepared Aconite Slice) 3 qian, Pao Jiangtan (Charred Ginger) 2 qian, Shanzhuyu (Cornus Fruit) 3 qian, Yun Fuling (Poria) 4 qian, Tu Chao Baizhu (Atractylodes Stir-fried with Soil) 4 qian, Ginger and Jujube as guides.
**Modified Shouwu Guzhi Tang**: Zhi Heshouwu (Prepared Polygonum Multiflorum Root) 6 qian, Pozhizi (Psoralea Fruit) 4 qian, Da Shudi (Prepared Rehmannia Root) 3 qian, Shang Rougui (Top-grade Cinnamon Bark) 8 fen, Pao Jiangtan (Charred Ginger) 2 qian, Shanzhuyu (Cornus Fruit) 3 qian, Dan Rongrong (Cistanche) 3 qian, Dangguishen (Angelica Sinensis Root Body) 3 qian, Yun Fuling (Poria) 4 qian, Tu Chao Baizhu (Atractylodes Stir-fried with Soil) 4 qian, Ginger and Jujube as guides.
**Buzhong Yiqi Tang**: Xidangshen (Codonopsis Root) 3 qian, Zhi Huangqi (Astragalus Root, Honey-fried) 3 qian, Tu Chao Baizhu (Atractylodes Stir-fried with Soil) 3 qian, Zhi Gancao (Honey-fried Licorice Root) 1.5 qian, Quandanggui (Whole Angelica Sinensis Root) 3 qian, Guang Chenpi (Tangerine Peel) 2.5 qian, Lv Shengma (Cimicifuga Rhizome) 1.5 qian, Chun Chaihu (Bupleurum Root) 2 qian, Shengjiang (Fresh Ginger) 3 slices, Hongzao (Jujube) 5 pieces.
**Hiccups (Qi Stagnation and Food Retention)**: A young man, arrogant and foolish, was beaten for a crime he didn't commit. While lunch was unfinished, he slept in anger. Food stagnated in the middle jiao, qi rebelled in the chest/diaphragm, causing hiccups (*Yue*). This is the *Yue* syndrome in *Shanghan Lun* and *Jin Gui Yao Lue*. Unresolved for five days, constipation noted. Prescribed **Tiaowei Chengqi Tang** (Regulate the Stomach and Purge the Yang Decoction) modified. One dose cleared bowels and stopped hiccups. Second dose relieved chest/sternum. Within three doses, sudden recovery.
**Wang Bingchen** from Dantu, living in a tiled house in Damuqiao, South Shanghai. Property owned by relative Mr. Jin. Wang had many children, poor, came to Shanghai to rely on Jin.
Mr. Jin let him live in a civilian village, collecting rent for a poor life. One day before the Anti-Japanese War, Wang placed a few yuan of rent on his desk drawer. After lunch, money was gone. Wife Peng and children claimed ignorance. Wang suspected eldest son Dahuai (18 years old) stole it. Dahuai denied it, saying father was busy or forgot elsewhere.
Son hadn't finished lunch. Wang, furious, beat him severely. Son slept in anger. Evening brought hiccups. Initially intermittent, then constant all day, stopping only when exhausted asleep. Woke every 1-2 hours due to sudden hiccups. Medical treatment failed. Patient suffered, father worried.
Consulted me. Wife Peng explained details, identifying Qi stagnation. Tongue tip white, root dry/yellow. Bowel movement absent for five days. Pulse slightly wiry-rapid. Skin slightly hot. Abdomen painful/tender/full. No other complaints. Reviewed previous prescriptions: Doctor A used Dingxiang (Clove) and Shidi (Persimmon Calyx); Doctor B used Daizhe (Magnetite) and Xuanshu (Inula Flower). All failed. I told Mr. Wang: "Previous failures were not due to pathogen, but Qi stagnation and stagnant food. Regulating Qi and descending rebellion was ineffective.
After eating, being scolded causes Liver Qi stagnation rebelling against Spleen. Spleen deficiency stalls transformation; Spleen Qi fails to assist Stomach. Stomach loses function, failing digestion. Treatment should soothe Liver, awaken Spleen, and regulate Stomach." Prescribed **Tiaowei Chengqi Tang** modified with Guang Yujin (Curcuma Rhizome), Chun Chaihu (Bupleurum Root), Jiao Baizhu (Charred Atractylodes), and Ji Neijin (Chicken Gizzard Lining). One dose cleared bowels, stopped hiccups, reduced bloating. Second dose continued bowel movements, relieved chest, frequent belching, hunger returned. Advised thin porridge for three days, careful diet to prevent relapse. Family complied, recovery achieved.
