Mr. Wenren stated: Varicella (smallpox) belongs to fire syndromes; a red complexion indicates a favorable course, while extreme cases indicate heat. If Liver Wood overacts on Spleen Earth, causing a greenish complexion, this is an unfavorable course. One should urgently use Sijunzi Tang (Four Gentlemen Decoction), Shengma (Cimicifugae Rhizoma), and Chaihu (Bupleuri Radix) to regulate and tonify the Spleen and Stomach; treatment for other symptoms should only proceed once the complexion returns to normal. I opine that if the aforementioned symptoms involve vomiting and convulsions due to food stagnation, or diarrhea and convulsions due to Spleen Qi injury, or cold limbs, these are precursors of Manpi Feng (Slow Spleen Wind). They should be treated with Renshen Lizhong Tang (Ginseng and Atractylodes Macrocephala Decoction) added with Chaihu and Gouteng钩 (Uncariae Ramulus cum Uncis). Any slight delay often leads to death. If there is varicella toxin,郁结 (stagnant) internally and externally, evaporating outwards, and encountering wind-cold which combats it, causing stagnation in the muscles and a greenish color all over the skin, Touji San (Muscle-Penetrating Powder) should be used. Injury to the Stomach generates wind and vomiting; injury to the Spleen generates wind and cold limbs; Wuyi Yigong San (Five-Ingredient Decoction for Extraordinary Efficacy) added with Tianma (Gastrodiae Rhizoma) should be used. If the sores are dense with intense heat, constipation, preference for cold drinks, and a red face, Xijiao Jiedu San (Rhinoceros Horn Detoxifying Powder) should be used. After entering the stage of pustule formation (guanjiang), if there is fever, irritability, thirst, and a red face, Danggui Buxue Tang (Angelica Sinensis and Astragalus Decoction) should be used. For hot feet, lower back pain, and red eyes, Dihuang Wan (Rehmannia Pill) should be used. These are all essential methods.
A child presented with red and vibrant smallpox lesions that were rising. Due to excessive eating, he experienced vomiting, diarrhea, lower back pain, a cyanotic lip and face, and cold hands and feet. This indicated deficiency-cold of the Spleen and Stomach being overacted upon. My father prescribed Liujunzi Tang (Six Gentlemen Decoction) with Shengjiang (Zingiberis Rhizoma Recens) and Rougui (Cinnamomi Cortex). After one dose, the preceding symptoms did not recede, and the smallpox color lacked redness. On the second dose, Fuzi (Aconiti Lateralis Radix Praeparata) was added (2 fen), and all symptoms immediately subsided. The next day, Shenqi Sixian San (Ginseng and Astragalus Four Sages Powder) was administered for two doses, leading to near recovery. Subsequently, within Bazhen Tang (Eight Treasure Decoction), Renshen (Ginseng Radix et Rhizoma) and Huangqi (Astragali Radix) were each increased to 5 qian. After four doses, the scabs fell off.
Another child contracted smallpox and consumed excessive cold drinks, resulting in abdominal pain, a greenish face, and cold hands and feet. This was deficiency-cold caused by cold pathogen injuring the Spleen. Fuzi Lizhong Tang (Aconite and Atractylodes Macrocephala Decoction) was used; pain ceased after one dose. Using 1 liang of Renshen and 1 qian of Shengjiang, pus filled the lesions after two doses. Then, Renshen decocted in water was given as tea; after more than a month, the scabs fell off.
Another child contracted smallpox, presenting with a greenish face, abdominal pain, and cold hands and feet. This indicated deficiency-cold of Spleen Qi. Wuyi Yigong San added with Muxiang (Aucklandiae Radix) and Rougui was used first. Then, Neituo San (Internal Support Powder) and Shenqi Sixian San were used until pus filled and scabs formed and fell off.
Another child contracted smallpox, presenting with a greenish face, abdominal pain, and cold hands and feet. This indicated deficiency-cold of Spleen Earth. First, Yihuang San (Yellow-Protecting Powder) powder (3 qian) was taken. Then, Liujunzi Tang with Muxiang stabilized the condition. Later, due to food stagnation, diarrhea occurred with a greenish face; Wuyi Yigong San was used for recovery.
Another child contracted smallpox, presenting with a greenish face and cold hands and feet. This indicated Cold Water insulting Earth (Kidney Water insulting Spleen Earth). Only Shierwei Yigong San (Twelve-Ingredient Decoction for Extraordinary Efficacy) could save him. The family did not believe this and instead administered dispersing and unblocking medicines. The child died with his entire body turning green; regret came too late.
