Postpartum edema, characterized by swelling of the hands and feet and shiny skin, is caused by Spleen deficiency failing to control water and Kidney deficiency failing to transform water. Treatment must prioritize tonifying Qi and Blood significantly, assisted by Cangzhu (Atractylodis Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Fuling (Poria) to tonify the Spleen; if accompanied by distension and fullness, use Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), and Xiangfu (Cyperi Rhizoma) to resolve it; for deficient constitutions, add Renshen (Ginseng Radix et Rhizoma) and Mutong (Akebiae Caulis); if there is heat, add Huangqin (Scutellariae Radix) and Maidong (Ophiopogonis Radix) to clear Lung Metal. Employ methods to invigorate the Spleen, promote diuresis, and tonify Middle Jiao Qi, selecting Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); if occurring after seven days postpartum, use Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) each 2 qian, Fuling (Poria) and Baishao (Paeoniae Radix Alba) each 1 qian, Chenpi (Citri Reticulatae Pericarpium) 0.5 qian, Mugua (Chaenomelis Fructus) 0.8 fen, Zisu (Perillae Folium), Mutong (Akebiae Caulis), Dafupi (Arecae Pericarpium), Cangzhu (Atractylodis Rhizoma), and Houpo (Magnoliae Officinalis Cortex) each 0.4 fen; if constipation occurs, add Yuliren (Prunus japonica seed) and Mahren (Cannabis Fructus) each 1 qian. If due to Cold pathogenic factor and dampness injuring the Spleen, resulting in anhidrosis and swelling, add Jiangpi (Zingiberis Recens Pericarpium), Banxia (Pinelliae Rhizoma), and Zisu Ye (Perillae Folium) to a Qi-tonifying formula to induce sweating and release the exterior.
Wupi San (Five Peel Powder): Treats postpartum wind-dampness invading and damaging the Spleen meridian, causing Qi and Blood stagnation, leading to puffy facial swelling, limb edema, and asthma.
Ingredients: Wujia Pi (Acanthopanacis Cortex), Digupi (Lycii Cortex), Dafupi (Arecae Pericarpium), Fuling Pi (Poria with Hostwood) (each 1 qian), Jiangpi (Zingiberis Recens Pericarpium) (1 qian), one Jujube (Ziziphi Jujubae Fructus). Decoct in water and take orally.
Another view states that if lochia remains unclean and stagnates in the uterine collaterals, causing edema, treating it as water qi disease by administering drugs such as Gansui (Kansui Radix) would be erroneous! One should merely administer Tiaojing San (Regulate the Menses Powder), so that blood circulation resumes and swelling subsides.
Tiaojing San (Regulate the Menses Powder):
Ingredients: Moyao (Myrrha), Hupo (Electrum) (both separately ground, each 1 qian), Rougui (Cinnamomi Cortex), Chishao (Paeoniae Radix Rubra), Danggui (Angelicae Sinensis Radix) (each 1 qian).
Grind into fine powder. Take 0.5 fen per dose, mixed with a small amount of Ginger Juice (Zingiberis Rhizoma Recens Succus) and wine.
Commentary: This formula can regulate menstruation and treat abdominal pain.
