To treat cough, nasal congestion, heavy voice, and aversion to cold caused by external contraction of wind-cold within seven days postpartum, do not use Ephedrae Herba (Mahuang, 麻黄) to induce sweating; if cough is accompanied by hypochondriac pain, do not use Chaihu Tang (Bupleurum Decoction); if cough produces sound but with scanty sputum and a red face, do not use cooling medicines. In cases of postpartum fire-cough or phlegm-cough, regulation must be maintained for half a month before cooling medicines can be used; they should not be used within the first half month.
Jiawei Shenghua Tang (Modified Birth-Expelling Decoction): Treats postpartum cough and nasal congestion with heavy voice due to external contraction of wind-cold.
Chuanxiong Rhizoma (Chuanxiong, 川芎) (1 qian), Danggui Radix (Danggui, 当归) (2 qian), Prunus Armeniaca Seed (Xingren, 杏仁) (10 grains), Platycodon Root (Jiegeng, 桔梗) (4 fen), Anemarrhena Rhizome (Zhimu, 知母) (8 fen).
If there is phlegm, add Fermented Pinellia Tuber (Banxiaqu, 半夏曲); if weak with sweating and cough, add Ginseng Radix (Renshen, 人参). In general, diaphoresis (inducing sweating) is contraindicated postpartum.
Commentary: Some editions list Zhimu (Anemarrhena Rhizome, 知母) as 4 fen.
Song: Postpartum seven days after contracting wind-cold, cough, nasal congestion, and aversion to cold occur.
Do not use Mahuang (Ephedrae Herba, 麻黄) or Chaihu Tang (Bupleurum Decoction, 柴胡汤); one must distinguish between fire-cough and phlegm-cough.
Wind-cold cough with heavy nasal voice responds best to Jiawei Shenghua Tang (Modified Birth-Expelling Decoction,
The formula contains Danggui Radix (Danggui, 当归), Paeonia Lactiflora Root (Baishao, 白芍), Anemarrhena Rhizome (Zhimu, 知母), Platycodon Root (Jiegeng, 桔梗), and Prunus Armeniaca Seed (Xingren, 杏仁); add Banxiaqu (Fermented Pinellia Tuber, 半夏曲) for phlegm, and Renshen (Ginseng Radix, 人参) for deficiency.
Jiaren Shen Anfei Shenghua Tang (Ginseng-Added Lung-Anchoring Birth-Expelling Decoction): Treats postpartum weakness where, within ten days, external contraction of wind-cold leads to cough, heavy voice, phlegm, fever, headache, or excessive sweating.
Chuanxiong Rhizoma (Chuanxiong, 川芎) (1 qian), Ginseng Radix (Renshen, 人参) (1 qian), Anemarrhena Rhizome (Zhimu, 知母) (1 qian), Morus Alba Root Bark (Sangbaipi, 桑白皮) (1 qian), Danggui Radix (Danggui, 当归) (2 qian), Prunus Armeniaca Seed (Xingren, 杏仁) (10 grains, skin and tip removed), Glycyrrhiza Uralensis Root (Gancao, 甘草) (4 fen), Platycodon Root (Jiegeng, 桔梗) (4 fen), Pinellia Ternata Tuber (Banxia, 半夏) (7 fen), Citrus Reticulata Pericarpium (Juhong, 橘红) (3 fen).
For those with deficiency and abundant phlegm, add Bamboo Sap (Zhuli, 竹沥) one cup and Ginger Juice (Jiangzhi, 姜汁) half a spoon.
Commentary: According to "Lun Cough" (Treatise on Cough), it is explicitly stated that even in fire-cough, cooling medicines should not be lightly used within the first half month, with strict warnings. However, both the first and second formulas contain Zhimu (Anemarrhena Rhizome, 知母); the notes mention "external contraction of wind-cold." This implies that after contracting wind-cold, pathogenic factors may transform into dryness-heat, necessitating the use of Zhimu (Anemarrhena Rhizome, 知母), though this was not clarified in the notes. If one assumes that such cooling substances are used immediately upon initial contraction, it would contradict previous discussions. Readers must grasp the subtle intent of predecessors to exercise judgment when applying ancient prescriptions.
Song: Ten days postpartum, contracting wind-cold, cough with heavy voice and phlegm occurs.
Body is weak and unable to resist pathogens, accompanied by fever, headache, and excessive sweating.
Use Jiaren Shen Anfei Shenghua Tang (Ginseng-Added Lung-Anchoring Birth-Expelling Decoction, 加参安肺生化汤), containing Danggui Radix (Danggui, 当归), Paeonia Lactiflora Root (Baishao, 白芍), Ginseng Radix (Renshen, 人参), Prunus Armeniaca Seed (Xingren, 杏仁), and Morus Alba Root Bark (Sangbaipi, 桑白皮).
Combine Anemarrhena Rhizome (Zhimu, 知母), Glycyrrhiza Uralensis Root (Gancao, 甘草), and Platycodon Root (Jiegeng, 桔梗); decoct together with Pinellia Ternata Tuber (Banxia, 半夏) and Citrus Reticulata Pericarpium (Juhong, 橘红).
Jiawei Siwu Tang (Modified Four Substances Decoction): Treats dry cough with sound but scanty sputum occurring after half a month.
Chuanxiong Rhizoma (Chuanxiong, 川芎), Paeonia Lactiflora Root (Baishao, 白芍), Anemarrhena Rhizome (Zhimu, 知母), Trichosanthes Semen (Gualouren, 栝蒌仁) (each 1 qian), Rehmannia Glutinosa Root (Shengdi, 生地), Danggui Radix (Danggui, 当归) (each 2 qian), Terminalia Chebula Fruit (Hezi, 诃子) (2 qian), Adenophora Root (Donghua, 冬花) (6 fen), Platycodon Root (Jiegeng, 桔梗) (4 fen), Glycyrrhiza Uralensis Root (Gancao, 甘草) (4 fen), Fritillaria Thunbergii Bulb (Dongling, 葱铃/兜铃) (4 fen), Zingiber Officinale Rhizome (Shengjiang, 生姜) (one large slice).
Song: Jiawei Siwu Tang (Modified Four Substances Decoction, 加味四物汤) consists of Danggui Radix (Danggui, 当归), Paeonia Lactiflora Root (Baishao, 白芍), and Chuanxiong Rhizoma (Chuanxiong, 川芎), plus Rehmannia Glutinosa Root (Shengdi, 生地), Anemarrhena Rhizome (Zhimu, 知母), and Platycodon Root (Jiegeng, 桔梗).
Also include Terminalia Chebula Fruit (Hezi, 诃子), Adenophora Root (Donghua, 冬花), Trichosanthes Semen (Gualouren, 栝蒌仁), Glycyrrhiza Uralensis Root (Gancao, 甘草), Zingiber Officinale Rhizome (Shengjiang, 生姜), and Fritillaria Thunbergii Bulb (Dongling, 兜铃).
