This type of hematemesis is commonly attributed by people to excessive Fire, leading to the use of cold-natured herbs to purge Fire. However, this results in even more vigorous Fire and increased bleeding; or if hemostatic agents are used without effect, who would realize that this is Blood failing to return to its vessels? The treatment method should employ Qi-tonifying herbs, assisted by those that guide Blood back into the vessels, achieving hemostasis without directly stopping blood. The prescription is as follows:
Ren Shen (Ginseng Radix) (5 qian), Dang Gui (Angelicae Sinensis Radix) (1 liang), Chao Dan Pi (Fried Moutan Cortex) (3 qian), Chao Jing Jie (Fried Schizonepetae Spica) (3 qian).
Decoct in water and take orally.
One dose is sufficient to stop all bleeding. The brilliance of this formula lies not in specifically tonifying Blood, but rather in tonifying Qi to nourish Blood; especially brilliant is that it does not directly stop bleeding, but achieves hemostasis through promoting Blood circulation. This is because Blood coagulates and stagnates when exposed to Cold, but returns to its vessels and does not rebel when dispersed. Saving lives at the critical moment, it indeed possesses miraculous efficacy.
In cases of hematemesis following severe anger, where blood gushes out like a pouring basin or shoots from the mouth, accompanied by sudden fainting and imminent death within moments. Treating with hemostatics causes Qi stagnation and discomfort; treating with Blood-tonics causes chest fullness and intolerance; some die due to various complications arising. Therefore, it is crucial to treat with the correct method. Use the Prescription for Resolving Blood and Calming Qi (Jie Xue Ping Qi Tang):
Bai Shao (White Peony Root) (2 liang), Dang Gui (Angelicae Sinensis Radix) (2 liang), Chao Jing Jie (Fried Schizonepetae Spica) (3 qian), Hei Zhi Zi (Black Gardenia Fruit) (3 qian), Chai Hu (Bupleuri Radix) (8 fen), Hong Hua (Carthami Flos) (2 qian), Gan Cao (Licorice Root) (1 qian).
Decoct in water and take orally.
After one dose, Qi becomes calm and comfortable; after two doses, bleeding stops; after three doses, the condition greatly improves. Anger injures the Liver, causing Liver Qi to become unbalanced, leading to hematemesis. If one does not first soothe the Qi but hastily stops bleeding, it will further agitate the Wood element of the Liver, inevitably making Qi more vigorous and bleeding worse. In this formula, Bai Shao is used in large quantities to pacify the Liver and soothe Qi; Jing Jie and Chai Hu are herbs that guide Blood back into the vessels; Dang Gui and Hong Hua generate new Blood and remove old stasis; how could recovery not be achieved?
Liuwei Dihuang Decoction (Six-Ingredient Rehmannia Pill/Decoction) added with Mai Dong (Ophiopogonis Radix) and Wu Wei Zi (Schisandrae Fructus) is most effective in tonifying the Kidney and nourishing the Liver. When Liver Wood is nourished, there is a vessel capable of storing Blood without leakage, making it highly suitable for blood disorders.
Recorded according to "Blood Treatment Methods" in *Shishi Milu*:
[Blood diseases most strictly prohibit cold-natured substances, as Cold causes stagnation and impedes flow, making it difficult for Blood to return to its vessels. Liuwei Dihuang Wan/Decoction is brilliant in being neither cold nor hot; it tonifies Kidney Water to nourish Liver Wood. Once Liver Wood is nourished, Blood has a vessel to reside in and naturally does not leak externally, let alone vomit upwards. The prescription uses Liuwei Dihuang Decoction added with Mai Dong and Wu Wei Zi:]
Shu Di Huang (Prepared Rehmanniae Radix) (5 qian), Shan Zhu Yu (Corni Fructus) (3 qian), Shan Yao (Dioscoreae Rhizoma) (2 qian), Dan Pi (Moutan Cortex) (3 qian), Ze Xie (Alismatis Rhizoma) (2 qian), Fu Ling (Poria) (2 qian), Mai Dong (Ophiopogonis Radix) (3 qian), Wu Wei Zi (Schisandrae Fructus) (1 qian).
Decoct in water and take orally.
With the addition of Mai Dong and Wu Wei Zi, Liuwei Dihuang Decoction can also clear and tonify Lung Metal, nourishing the skin and hair, strengthening pores, thereby preventing Blood from escaping externally. With Lung Metal undisturbed, it sufficiently controls Liver Wood downwards; fearing the control of Metal, Liver Wood would not attack the Lungs upwards. Thus, this formula is most suitable for blood disorders. Long-term use for three years without hematemesis allows one to celebrate rebirth; otherwise, one remains on the brink of life and death.
