When scrofula (tuberculosis of cervical lymph nodes) progresses into consumptive disease (Xu Lao), the principle is exactly the same as when hemorrhoidal fistulas progress into consumptive disease, differing only in the upper and lower locations of the lesions. (Zhuoxuan stated: It is not limited to scrofula and hemorrhoidal fistulas; any chronic ulcer that fails to heal and discharges excessive pus will lead to consumptive disease due to blood deficiency.)
During pregnancy, severe heat stagnation often leads to miscarriage, particularly with measles and epidemic toxins. This occurs because intense heat in the intestines and stomach steams and insults the uterus. Therefore, the use of Da Huang (Rhubarb Rhizome) and Mang Xiao (Mirabilite) is not contraindicated, and Ba Dou (Croton Seed) may also be used at times. This is known as "when there is a pathogen, there is no harm [to the healthy qi]." However, one must not neglect the timing for using purgative methods against epidemic toxins.
For patients with凝结 (coagulation/stagnation) in both hypochondriac regions, moxibustion directly on Zhangmen (LR13) may easily trigger agitation of Qi. Conversely, if Fengshi (GB31) is moxibusted first, it yields effective results. Those who are easily agitated by moxibustion should understand this principle well. For diseases located in the upper body, moxibustion should begin at the feet and gradually extend to the waist; this relieves symptoms in the upper part without causing agitation. Moxibusting only the local affected area often fails to produce full effects. This is analogous to encountering a pattern suitable for Da Chai Hu Tang (Major Bupleurum Decoction) but obstructing its use, instead administering Li Qi Tang (Qi-Regulating Decoction) to promote Qi flow. Once the Qi flows smoothly, agitation does not occur. The principles behind moxibustion and herbal medicine regarding disease treatment are fundamentally identical.
If damp-water accumulates in the lower jiao during pregnancy and persists postpartum, accompanied by lochia stasis and rapid, labored breathing, promoting urination to drain the water will also facilitate the discharge of lochia. The use of Xie Xin Tang (Drain the Epigastrium Decoction) or Zi Wan (Purple Pill) for treating mania aims to purge and unblock the chest and diaphragm. Mr. Dodo's use of Zi Wan to treat turtle chest and turtle back likely stems from this same intent. The use of Zi Wan for postpartum flaccidity and weakness of the legs and knees is also intended to unblock the chest and diaphragm Qi so it can descend.
When Pi accumulation (masses/lumps) causes rebellious Qi rushing to the epigastrium and hypochondriac regions, the patient's gaze will inevitably become abnormal (strabismus). Furthermore, depending on whether the Pi accumulation is on the left or right, the size of the pupils may spontaneously change.
For persistent vomiting during pregnancy with severe fluid disturbance, Xiao Ban Xia Jia Fu Ling Tang (Minor Pinellia Decoction plus Poria) combined with Jing Mi (Coix Seed/Rice), Shu Yu (Dioscorea Root), and Sheng Jiang (Fresh Ginger) yields remarkable results. If the Middle Qi is extremely deficient, Xiang Sha Liu Jun Zi Tang (Aucklandia and Amomum Six Gentlemen Decoction) with added Jing Mi, where all herbs are chopped and stir-fried until blackened, along with one fen of Yang Shen (American Ginseng) added separately, decocted in water and taken in small frequent doses, is effective.
In cases of sudden, incessant hematemesis or syncope due to blood loss, moxibustion on Jiutui (CV15) for several hundred moxa cones produces excellent results. (In severe blood loss, the priority is to restore Yuan Qi; one must not fear the heat of the moxa flame nor rely solely on cold-natured herbs, which would gradually deplete the Yuan Qi.)
For infants with incessant spitting up of milk, adding high-quality Moshepi (Musk Bark) to the syndrome-specific formula is effective. (For adults with vomiting unresponsive to various medicines, a powder of She Xiang (Musk) and Gui Xin (Cinnamon Heart) mixed and taken orally is effective.)
Ma Pi Feng (Horse Spleen Wind), measles, and erysipelas share similar treatment approaches. However, Ma Pi Feng has distinct treatments, such as Wu Jia San (Priceless Powder). (This view seems too crude, but upon careful reflection, it contains logic; those proficient in treatment will know this.)
I once read Mr.'s commentary *Shanghan Lun Zhengwen Jie* (Explanation of the Main Text of Treatise on Cold Damage) and deeply appreciated his insights surpassing those of ancient masters. I also read *Dao Shuo Suoyan* (Trivial Words on Guiding Water) and *Yang Ying Suoyan* (Trivial Words on Nurturing Infants), marveling at how his medical skills reached a divine level. However, his work *Fang Yi Jie* (Explanation of Formula Intentions) appears forced and speculative, sometimes contradicting the original intent of ancient formulas. Since formula theories originated with Cheng Wuji, later scholars like Wu Kun, Li Zhongzi, Ke Qin, and Wang Ang each offered their own interpretations. Yet, some selections were imprecise and explanations unclear, making the understanding of formula intentions difficult. This difficulty has existed since antiquity, not just in this book. (Zhuoxuan stated: *Fang Yi Jie* debates extensively. Ultimately, it is merely one school's opinion, and I cannot accept it.)
