Headache with swelling as large as a dou indicates an epidemic disease spreading through the air.
The upper half of the human body belongs to the Qi of Heaven; the lower half belongs to the Qi of Earth. Pathogenic heat lodges between the Heart and Lung, while the Fire of the Yangming and Shaoyang meridians flares up again. Additionally, one contracts the plague Qi of the four seasons of Heaven and Earth. Consequently, heat stagnates in the Upper Jiao and cannot dissipate, invading the clear orifices. Warm-heat rises to the vertex causing swelling; Wood carries Fire pathogen causing pain. It may even ulcerate and crack, and pus/blood infects others, hence called Yi Li (epidemic pestilence). Mild cases are named Fayiswelling (parotitis), occurring around both ears. Some use Chengqi Tang (Stomach Releasing Decoction) to purge excess heat from the Stomach, which is excessive punishment. The treatment method should not use rapid/strong drugs; if too fast, it exceeds the pathology, meaning "upper heat remains undissipated, middle cold arises," inevitably harming life. Use mild drugs, administer slowly and in small amounts. Observe where the swelling lies in terms of meridians and treat accordingly. If Yangming meridian is affected, the head swells greatly; if Shaoymeridian is affected, symptoms appear around the ears. Generally, Puji Xiaodu Yin (Universal Benefit Toxin-Resolving Decoction) is the main formula. Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix) are bitter, clearing Heat of Heart and Lung, serving as Jun (Emperor) herbs; Juhong (Citrus Reticulatae Pericarpium Rubrum) and Xuanshen (Scrophulariae Radix) are bitter-cold, Shenggancao (Raw Licorice Root) is sweet-cold, clearing fire and tonifying Qi, serving as Chen (Minister) herbs; Lianqiao (Forsythiae Fructum), Niubangzi (Arctii Fructus), Bohe (Menthae Haplocalycis Herba) are bitter-pungent-neutral, Banlangen (Isatidis Radix) is sweet-cold, Mapibo (unknown specific herb, likely local name) and Jiangcan (Bombyx Batryticatus) are bitter-neutral, dispersing swelling and detoxifying, serving as Zuo (Assistant) herbs; Shengma (Cimicifugae Rhizoma) and Chaihu (Bupleuri Radix) are bitter-neutral, moving stagnant Qi in Shaoyang and Yangming that cannot extend; Jiegeng (Platycodonis Radix) is pungent-warm, acting like a rudder preventing downward descent. Only by such regulation can life be preserved.
