各种积滞须分虚与实,虚者热微实者热多;

Niudanzhi Jiao Wan (Ox Bile Steamed Cake Pills), size of hemp seeds. Take five pills each time, administered with a decoction of Wumei (Mume Fruit), Shengjiang (Fresh Ginger), and Femi (Honey). If all five types of Gan syndrome appear, millet-grain-like rashes emerge on the body, and the complexion turns black, death is certain.
Gan Ke (Gan Syndrome with Thirst) alleviates slightly at night.
Gan Ke arises from pre-existing Gan Qi (pathogenic factor in Gan syndrome) in the viscera, compounded by the wet nurse's indulgence in the Five Pungents, roasted/fried foods, alcohol, and wheat products. This leads to stagnation and heat in the child's Heart and Lungs. During the day, there is irritability, intense thirst, and desire for water, with poor appetite for milk and food; at night, the thirst ceases. Lian Dan Wan (Coptis and Gallbladder Pill) is indicated. If drinking water does not stop and the tongue turns black, death ensues immediately.
Gan Lao (Gan Syndrome with Consumption) presents with tidal fever, night sweats, cough, and diarrhea.
Gan Lao involves bone-steaming fever (Yin deficiency with internal heat), tidal fever in the "Five Hearts" (both palms, both soles, and the chest), night sweats, cough, diarrhea with an abdomen hard as stone, and a silvery-white complexion. This condition is incurable. The ancient formula Ba Wu Tang (Eight-Ingredient Decoction), without Bai Zhu (Atractylodes Rhizome), plus Huang Qin (Scutellaria Root), Chai Hu (Bupleurum Root), Chen Pi (Tangerine Peel), Ban Xia (Pinellia Tuber), Shi Jun Zi (Quince Seed), Hamagui (Toad Ash), and Bie Jia (Turtle Shell), all in equal parts, is decocted with Sheng Jiang (Fresh Ginger) and Da Zao (Jujube). Alternatively, use Lian Dan Wan, Xiang Lian Zhu Du Wan (Costus, Coptis, and Pig Stomach Pills), or add Hamagui (Toad Ash) for rescue. If shortness of breath occurs, death is imminent.
Gan Xie (Gan Syndrome with Diarrhea) shows blue veins on the forehead.
Gan Xie is characterized by dry hair, white lips, blue veins on the forehead, abdominal distension with borborygmus, and passage of undigested food in stool. Hot herbs to stop diarrhea are contraindicated. Grind into powder, form pills with rice paste, size of millet seeds. Take five pills with rice drink. If slippery diarrhea with rectal prolapse or hiccups occur, death is rapid.
Gan Li (Gan Syndrome with Dysentery) is caused by entanglement of five-color damp-evil.
Gan Li occurs when existing Gan disease is compounded by food stagnation and exposure to cold/heat imbalance, leading to dysentery with five colors of discharge, tenesmus. Xiang Sha Wan (Costus and Amomum Pills) is indicated: Huang Lian (Coptis Rhizome) 3 qian, Mu Xiang (Aucklandiae Radix), Hou Po (Magnolia Bark), Ye Ming Sha (Trogopterus Dung), and Sha Ren (Cardamom Seed) 2 qian each, He Zi (Chebula Fruit) 1 qian. Grind into powder, form pills with glutinous rice paste, size of hemp seeds. Take fifteen pills with a decoction of Sheng Jiang (Fresh Ginger) and Ai Ye (Mugwort Leaf). If the area between the nose and upper lip (Ren Zhong) becomes flat and full, death is certain.
Gan Zhong (Gan Syndrome with Swelling) results from middle-jiao deficiency and toxin accumulation.
Gan swelling involves deficiency with stagnation, where accumulated toxins bind with Spleen Qi, causing abdominal tension. Subsequently, the Spleen is affected by dampness, leading to edema of the head, face, hands, and feet. Tui Huang Wan (Remove Yellowing Pills) and Fei Er Wan (Fatten Child Pills) are indicated. For severe distension, He Wan Zi (Brown Pills) is used: Luo Bo Zi (Radish Seed) 1 liang, Chen Pi (Tangerine Peel), Qing Pi (Green Tangerine Peel), Bing Lang (Areca Seed), Hei Qiu (Black Morning Glory Seed), Wu Ling Zhi (Trogopterus Dung), Chi Fu Ling (Red Poria), and E Zhu (Curcuma Rhizome) 5 qian each, Mu Xiang (Aucklandiae Radix) 2.5 qian. Grind into powder, form pills with flour paste, size of mung beans. Take fifteen pills with a decoction of Sang Bai Pi (Mulberry Root Bark) and Zi Su (Perilla Leaf), or Radish decoction. It treats infant indigestion, epigastric/fullness, vomiting, shortness of breath, borborygmus, diarrhea, cold pain in the abdomen, food stagnation, qi stagnation, intestinal masses, constipation or diarrhea, facial edema, and concurrently treats the Five Gans and Eight Dysenteries, emaciation with abdominal distension, with miraculous effects. Another prescription includes Hu Jiao (Pepper), Huang Lian (Coptis Rhizome), San Leng (Burreed Rhizome), and Ku Lei Gen (Melia Root Bark) 2.5 qian each. Hui Gan (Roundworm Gan Syndrome) is difficult to treat if worms exit.
Hui Gan arises from milk deficiency, premature introduction of porridge/meat, causing sweet/greasy substances to accumulate in the intestines/stomach and transform into roundworms. Symptoms include frequent crying, vomiting foam, abdominal pain, purple lips, and itching of the anus and teeth. Although roundworms feed on other parasites, they should not be forcibly moved; if they exit via the mouth or nose, treatment is difficult. In long-standing Gan stagnation, worms are invariably present. Yellow/white worms are treatable; black/blue worms indicate death.
Nao Gan (Brain Gan Syndrome) presents with fontanelle swelling and hair tufts.
Nao Gan stems from wind-heat carried in utero, excessive breastfeeding/feeding after birth, or parental intercourse during labor. Symptoms include crusty sores on the head, burning heat in the brain, hair forming tufts, swollen/elevated fontanelles, and generalized sweating. Long Dan Wan (Gentian Pill) is indicated: Long Dan Cao (Gentiana Root), Sheng Ma (Cimicifuga Rhizome), Ku Lei Gen (Melia Root Bark), Fang Feng (Saposhnikovia Root), Chi Fu Ling (Red Poria), Lu Hui (Aloe), You Fa Hui (Burnt Human Hair), Qing Dai (Indigo Naturalis), and Huang Lian (Coptis Rhizome) in equal parts. Wu Gu Gan (Innocent Gan Syndrome) presents with neck nodes like powder, possibly due to crow feathers.
Wu Gu Gan features movable, soft, non-tender nodes on the side of the head/neck containing worm-like particles resembling rice flour. If not promptly ruptured, worms disperse with heat flow, damaging viscera, leading to limb abscesses, purulent stools, high fever, emaciation, and prominent skull bones. Initially, puncture with a needle and apply plaster. Etiology may involve washing infant clothes hanging under eaves overnight, contaminated by female crow feathers; wearing such clothes allows worms to enter the skin. Burning the clothes prevents this. Yue Chan Wan (Moon Toad Pills) is indicated: One toad, killed and placed in a bucket, soaked in urine. Add one scoop of maggot, let maggots feed for one day/night. Remove, tie in a cloth bag, soak in running water overnight, dry on tiles. Add She Xiang (Musk) a "zi" amount. Grind into powder, form rice pills size of hemp seeds. Take thirty pills in the morning with rice drink. First dose reduces deficiency-irritability; second stops thirst; third stops diarrhea. Also treats various Gan syndromes.
Ding Xi Gan (Stilt Abdomen Gan Syndrome) features large abdomen and small limbs.
"Ding" refers to extremely small limbs and neck; "Xi" refers to large abdomen. Severe cases show elevated buttocks, wasted flesh, protruding umbilicus, crying with sunken chest, or "Grain Gan" craving raw rice.
Bu Lu Gan (Breast Leak Gan Syndrome) presents with vomiting, bone prominence.
Bu Lu Gan involves alternating deficiency-heat, separated cranial sutures, vomiting of food/worms, irritability/thirst, foul vomiting, emaciated ribs, and exposed form (death). Ding Xi and Bu Lu result from chronic Spleen/Stomach deficiency failing to digest food/grain, weakening Ying/Wei Qi, wasting muscles, depleting Kidney Qi, and susceptibility to wind-cold. Some arise from fetal toxicity or visceral blood deficiency. All belong to the Wu Gu category and are difficult to treat. Shi Quan Dan (Ten Complete Pills) is indicated.
Bao Tong Wan (Protect Child Pill) for digestion is universally applicable.
Wu Gan Bao Tong Wan (Five Gan Protect Child Pill): Man Li Tou (Eel Head), Chan Tou (Toad Head), Xiong Dan (Bear Gallbladder), She Xiang (Musk), Ye Ming Sha (Trogopterus Dung), Tian Jiang Zi (Calotropis Seed), Huang Lian (Coptis Rhizome), Long Dan Cao (Gentian Root), Qing Pi (Green Tangerine Peel), Wu Bei Zi (Gallnut), Ku Lei Gen (Melia Root Bark), Xiong Huang (Realgar), Qing Dai (Indigo Naturalis), Lu Hui (Aloe), Hu Huang Lian (Coptis Chinensis Rhizome) in equal parts. Single prescriptions praised for Ye Ming Sha.
Roasted Ye Ming Sha (Trogopterus Dung) powder added to food treats all Gans. Another condition arises if a pregnant woman is influenced by evil spirits affecting the fetus, causing infant diarrhea, alternating chills/fever, lusterless hair; or if a mother with an infant becomes pregnant again, causing this disease through breastfeeding. Qian Jin Long Dan Tang (Thousand Gold Gentian Decoction) is indicated. Also, wear Ye Ming Sha in a red gauze bag.
Zhu Ji (Various Accumulations)
Accumulations must be distinguished as Deficiency or Excess; Deficiency has mild heat, Excess has abundant heat.
Various accumulations cause abdominal distension/pain, potentially leading to masses, edema, jaundice, and the Eight Dysenteries. Deficiency type: mild whole-body heat or night heat, poor appetite, mental fatigue, appearing asleep when held. Excess type: high fever, especially abdominal heat, constipation, swollen cheeks, throat obstruction, profuse phlegm-rattle, heat-toxin sores. Mu Xiang Wan (Aucklandia Pills) is primary. Use less for Deficiency, double dose for Excess. Variations: black face/stool, chronic diarrhea with shortness of breath, sores on palms, emaciation/weakness are incurable.
Ru Ji (Milk Accumulation) vomiting/diarrhea is extremely foul.
Vomiting/diary milk with sour-foul odor results from crying while feeding, causing milk stagnation. Even without grain intake, masses form; this is Milk Accumulation.
Qi Ji (Qi Accumulation) crab-like stool and excessive crying.
Abdominal pain, crying, stool resembling crab feces, or fever, distended belly/emaciated body, food failing to nourish, result from Qi disturbance, Ying/Wei disharmony, and prolonged duration. This is Qi Accumulation.
Shi Ji (Food Accumulation) yellow face and hard abdomen.
Hard abdomen with heat, thirst/diarrhea, or vomiting, yellow face, result from overeating followed by sleep. This is Food Accumulation.

Severe cases use Xiao Ji Wan (Eliminate Accumulation Pill), Ying Gan Wan (Responsive Pill), or Hong Wan Zi (Red Pills) to purge.
Know that infant intestines/stomachs are soft; strictly avoid harsh drugs harming Yuan Qi.
Infants with accumulations have fragile intestines/stomachs. Avoid toxic attacking drugs; prolonged use causes Spleen deficiency, poor appetite, vomiting/diarrhea, and other complications. Method to remove accumulation: Harmonize Spleen/Stomach, attack gradually according to urgency. Do not damage Stomach Qi. Purging accumulation may injure Spleen, causing tidal fever or transforming into Slow Convulsion.
Pi Bing (Mass Disease) refusing food but drinking milk.
Infants refusing food but drinking milk, accompanied by cough/phlegm,必有 (must have) Pi masses in the abdomen.
Han Re Ru Nü (Chills/Fever like Malaria) due to retained water.
Only Pi masses cause tidal fever/chills. Cause: improper diet leading to water retention in Middle Jiao, unable to circulate, becoming stasis/phlegm, combated by cold Qi, knotting into Pi. Hence common in chronic malaria.
Hidden location in hypochondrium/abdomen with pain.
These are masses, similar to adult accumulations, hiding in hidden hypochondriac/abdominal areas, causing intermittent pain.
Qu Pi (Remove Mass) primarily uses Bao An Wan.

Abdominal distension due to middle-jiao deficiency and Qi; excess accumulation causes bad breath/asthma.
Distension with stifling sensation and asthma is Excess. Select Zi Shuang Wan, Bai Yu Bing, Xiao Ji Wan, or He Wan Zi to promote accumulation discharge. If shortness of breath/rapid breathing, use Fen Qi Zi Su Yin (Separate Qi Perilla Decoction) replacing Zi Su Tang.
No accumulation means no asthma; deficiency requires warming.
No accumulation-related asthma is Deficiency, treatable with warming dispersion. Do not misuse purgatives; avoid aromatic/dry/hot herbs.
Mistaken purging causes facial/hand-foot edema.
Mistaken purging sinks Spleen Qi; deficient Qi attaches to Lung and moves externally. Lung governs face/eyes; Spleen governs limbs. Thus, edema occurs.
Zhong Zhang (Swelling) generally uses Ta Qi Jiu.
Hu Jiao (Pepper) 1 liang, Xie Shao (Scorpion Tail) 5 qian. Grind, form flour paste pills size of millet seeds. Take 5-7 pills with rice drink. If abdomen large, add Luo Bo Zi (Radish Seed).

Wind-heat cyst with constipation: San Bai San (Three-White Powder). Prolonged crying causing Yin Qi sinking, water ducts blocked; or maternal excessive weeping causing fetal Small Intestine Qi closure, blood/water coagulation, Water assaulting Lung, leading to asthma preceding swelling/pain. Soft or woody hardness: Move Heart Qi, expel Kidney pathogen, benefit urination/defecation, no tonification. Tao Ren Wan (Peach Kernel Pill): Tao Ren (Peach Seed) 3 qian, La Gui (Cinnamon Twig), Da Huang (Rhubarb Root), Qian Niu (Morning Glory Seed), Ji Li (Tribulus Fruit), Mu Dan Pi (Moutan Cortex) 2 qian each. Heat in external genitals: Apply Ji Zi Qing (Egg White), or add Mu Li (Oyster Shell). See Major Section on Hernia for details.
Long-term distension/blockage obstructs chest; consider Qin Lian Zhi Geng.
Pi結 (Pi Knot) due to heat gathering in abdomen, blocking circulation, attacking chest/hypochondrium, painful on pressure, periodic high fever. Qin Lian Zhi Geng Tang (Scutellaria, Coptis, Aurantium, Platycodon Decoction): Zhi Ke (Aurantium Fructus), Jie Geng (Platycodon Root) 5 fen each, Ban Xia (Pinellia Tuber), Huang Qin (Scutellaria Root), Gua Lou Ren (Trichosanthes Seed), Huang Lian (Coptis Rhizome) 3 fen each, Sheng Jiang (Fresh Ginger), Mai Men Dong (Ophiopogon Tuber) decocted.
