Liuwei Tang (Six-Ingredient Pill, referring to Liuwei Dihuang Wan) (discussed under stroke-like disorders).
The eyes have five wheels corresponding to the five zang organs. The eye sockets belong to the Spleen, known as the Flesh Wheel. Red vessels belong to the Heart, known as the Blood Wheel. White sclera belongs to the Lung, known as the Qi Wheel. Greenish hue belongs to the Liver, known as the Wind Wheel. The pupil belongs to the Kidney, known as the Water Wheel. Thus, it is understood that the eyes are where the essence of the five zang organs converges. Eye disease specialists categorize them into seventy-two symptoms with Q&A formats, but there is much overlap; it is more accurate to distinguish between excess and deficiency. Acute redness, swelling, pain, photophobia, and aversion to light are termed Excess Syndrome of External Obstruction; chronic pain, blurred vision, and sunken small pupils are termed Deficiency Syndrome of Internal Obstruction. Excess arises from wind-heat, while deficiency arises from blood insufficiency. For excess, disperse wind and purge fire; for deficiency, nourish kidney water and yin. However, after dispersing wind, one must follow with blood nourishment, as the classics state: "The eyes see only when they receive blood." While nourishing yin, qi tonification should be added, as the classics state: "When qi is vigorous, it generates blood." To treat external obstruction, use Jili Tang or Chanhua Wubi San to disperse. If complicated by dietary injury, add digestive-promoting herbs. If constipation persists, administer Sishun Qingliang Yin to purge. To treat internal obstruction, use Xiaoyao San or Mingmu Dihuang Wan to tonify. If complicated by qi deficiency, prioritize Yiqi Congming Tang. Furthermore, for initial nebulae/opacities, oral medication alone suffices; avoid immediate topical drops lest the condition deepen. Use natural water, warm and frequent washing is recommended, as heat disperses wind and water controls fire. No herbs are added to the water because the eyes do not tolerate dust, and herbal juices cause discomfort. Knife pricking for bloodletting, flesh cutting, and application of arsenic are strictly contraindicated as reckless gambles. Pricking risks damaging muscle tissue; arsenic risks unceasing ulceration. Only Zhenzhu San (Pearl Powder) should be used topically, the supreme treasure among eye medicines. Moreover, when using dispersing powders, do not overdo it to avoid damaging blood; when using qi-tonifying drugs, do not overdo it to avoid aiding fire. Avoid excessive cold-cooling herbs which may congeal blood vessels and generate green-yellow membranes. Warmly nurture the liver and kidneys, maintain balance, and eye diseases will naturally heal.
