There are two types of postpartum lactation obstruction: one where blood and qi are abundant but stagnant, causing the milk vessels to be blocked; and another where blood and qi are deficient, resulting in very little milk. For deficiency, tonification is used, employing ingredients such as Milky Milkstone Powder, Pig Trotters, and Crucian Carp. For excess, promotion/circulation is used, employing ingredients such as Tetrapanax Pith, Arnebia Root, and Trichosanthes Root. The formulas are listed below (from *Chan Bao*):
**Lou Lu Tang (Arnebia Root Decoction)** [from *Chan Bao*]: Treats obese women with stagnant qi and vessels, resulting in lactation obstruction, or stagnation forming abscesses with swelling, pain, and impending suppuration.
Ingredients: Arnebia Root (2.5 liang), Snake Skin (roasted, one strip), Trichosanthes Root (ten roots, sliced, stir-fried until scorched).
Note: "Tu Gua" should be "Wang Gua" (Melon of the Month). As recorded in *Li Ji · Yue Ling*: "In the fourth month, Wang Gua grows." This refers to this plant. Its fruit resembles *Trichosanthes Kirilowii* fruit but is smaller, like a lychee; it is yellow, has vines, and round, intact leaves.

**Zhong Ru San (Milky Milkstone Powder)** [from *Chan Bao*]: Treats women with Qi deficiency and Blood insufficiency, sluggish pulse, and extremely scant milk.

Each dose is 2 qian, taken with strongly brewed Lou Lu Tang.
**Zhu Ma Ti Tang (Sow Trotter Decoction)** [from *Ju Fang*]: Treats lactation obstruction.
Ingredients: Sow Trotters (one), Tetrapanax Pith (4 liang).
Decoct with ten dou of water until four or five sheng remain. Take the juice. If milk does not flow, prepare and take another batch.
*(Dan Xi Huo Tao)* states: After recent delivery, Peony Root should not be used because its sour and cold nature can damage the vital energy of generation (forbidden within ten days). Instead, use Huang Qi Si Wu Tang (Astragalus Four Substances Decoction) as the essential formula for tonifying deficiency, replacing Peony Root with Astragalus Root. For Qi deficiency, add Ginseng, Atractylodes, Poria, and Licorice to the base formula. For fever, add Dried Ginger. For excessive spontaneous sweating, reduce the amount of Ligusticum Chuanxiong Rhizome, omit Poria, and double the amount of Honey-Roasted Astragalus Root. For thirst, add Schisandra Berry and Ophiopogon Tuber. For abdominal pain, Peony Root is indispensable; even in recent postpartum cases, it can be used if stir-fried with wine. If pain is caused by incomplete lochia discharge, decoct Si Wu Tang and mix in Cyperus Rhizome and Trogopterus Dung Powder. For severe cases, add 4–5 fen of Peach Kernel Paste. Pain caused by the uncontracted uterus in recent postpartum is called *Er Zhen Tong* (Child Pillow Pain) or *Jia Mu Kuai Tong*. Use Vine-Processed Peony Root, Poppy Shell, and Licorice, decocted in water with a small amount of Rice Vine. Alternatively, grind these three ingredients into powder and take with Vine Water. This utilizes the astringent property of sourness. If postpartum retained evil blood causes chills, fever, and formation of *jia* masses, use Si Wu Tang plus Burreed Tuber, Curcuma Zedoaria, Frankincense, Myrrh, Cyperus Rhizome, Trogopterus Dung, Dried Resin, Peach Kernel, and Safflower. For incessant postpartum abdominal pain, use Si Wu Tang plus Litsea Petiolata Fruit, Cyperus Rhizome, Cassia Twig Heart, Galangal, and Tangerine Peel, decocted with Child’s Urine and Vine; this is highly effective. For postpartum menorrhagia lasting over a month, use Si Wu Tang plus Angelica Dahurica Root and Cimicifuga Rhizome, mixed with Ash of Human Hair. For postpartum vulvar pain, use Si Wu Tang plus Lithospermum Root and Saposhnikovia Root. For generalized postpartum edema, use Si Wu Tang plus Frankincense, Myrrh, Cassia Twig Heart, Akebia Stem, Areca Peel, Galangal, Dragon’s Blood, Betel Nut, and Goldsand. For postpartum uterine inversion/prolapse, use Ba Wu Tang (Eight-Ingredient Decoction) plus Cimicifuga Rhizome and Saposhnikovia Root, with Wine-Processed Astragalus Root as the sovereign drug. For postpartum wind-stroke with facial paralysis (crooked mouth and eyes), use Ba Wu Tang plus Aconitum Lateralis Radix Praeparata and Schizonepeta Spike, with a small addition of Saposhnikovia Root and Notopterygium Rhizome, decocted and taken.
