[Background Information]
Book: Medical Outline (Yixue Gangmu) Author: Lou Ying Dynasty: Ming Chapter: Postpartum Jaundice / Dietary and Labor-induced Kan Yellow Category: Medical Discussion
[Ding] Treated a woman who developed jaundice after giving birth during the fourth month of pregnancy. She presented with fatigue in the four limbs, poor appetite, amenorrhea, intermittent fever, and a wiry pulse.
Baizhu (1 liang), Renshen, Qinjiao, Danpi, Shengdi, Mutong, Chaihu, Shaoyao (each 0.5 liang), Chuanxiong, Huangqin, Gange (each 1 qian), Gancao.
Divided into twelve doses. Decoct in water and take hot before meals.
This condition is also known as "Huang Pang" (Yellow Distension). "Huang Dan" (Jaundice caused by damp-heat accumulation in the liver and gallbladder leading to bile overflow onto the skin) is an acute disease; therefore, Zhang Zhongjing set a prognosis period of eighteen days. "Shilao Huang" (Chronic jaundice caused by food stagnation and overexertion) is a chronic condition, sometimes persisting for a long time without cure, so it should be classified under a separate chapter heading.
[Ding] Da Wenzhong Wan (Great Warm Middle Pill) treats Huang Pang. (A formula finalized by Mr. Zhu in his later years.)
Xiangfu (1 jin, soaked in urine from young boys, one night in spring/summer, three nights in autumn/winter), Gancao (2 liang), Zhen Sha (stir-fried until red, quenched in vinegar three times, 1 jin), Kushen (2 liang in spring/summer, 1 liang in autumn/winter), Houpo (processed with ginger juice, stir-fried black, 5 liang), Shaoyao (5 liang), Chenpi (3 liang), Shanzha (5 liang), Cangzhu (5 liang, soaked in rice washing water), Baizhu, Fuling (each 3 liang), Qingpi (6 liang).
All ground into fine powder, mixed with vinegar paste to form pills the size of paulownia seeds. For those with dark complexion, exposed tendons and bones, and excess qi, take 50-60 pills with rice drink. For those with puffy white complexion and deficient, emaciated qi, take 30-40 pills with Baizhu decoction. Avoid all raw, cold, greasy foods, chicken, goose, mutton, duck, raw/hard items, and zongzi (sticky rice dumplings) which are hard to digest. After seven days of taking the medicine, one will feel coolness in the palms and a red flush inside the lips; recovery occurs after half a month of regulation.
For men suffering from Huang Pang, there is old food accumulation in the stomach, bland taste in the mouth, sore legs, and shortness of breath.
Zhen Sha (washed, stir-fried clean, quenched in vinegar, taken as fine powder, 3 liang), Cangzhu, Xiangfu, Sanleng (each 1 liang), Chenpi, Huanglian, Zhuyu, Renshen (each 0.5 liang), Fuling (7 qian).
Grind into powder, mix with Shenqu and vinegar paste to form pills. Take 50 pills per dose. If feeling full, add 10 Baohe Wan pills, taken with plain boiled water.
Also, Xiao Wenzhong Wan (Small Warm Middle Pill) treats Huang Pang. (Suitable for poor people in rural areas, as their diet lacks accumulation, requiring only yin nourishment and dampness drying.)
Zhen Sha (1 jin, stir-fried with vinegar into powder); add glutinous rice stir-fried until very yellow into powder (also 1 jin), mix with vinegar paste to form pills the size of paulownia seeds. Take 40-50 pills with rice drink per dose. Avoidances same as above. Mild cases take 5 liang, severe cases no more than 7 liang for cure.
The sixty-year-old sister-in-law has a yellow face, bitter taste in the mouth, and thirst; this is damp-heat generated by food stagnation.
Baizhu (1.5 qian), Lianqiao (1 qian), Qianghuo, Tongcao.
Decoct until ready, swallow Gu Dan Wan and Baohe Wan (40 pills each), and Awei Wan (5 pills).
Nuanzhong Wan treats Huang Pang, suppresses liver pathogenic factors, and soothes spleen qi. Not suitable for deficiency patterns.
Chenpi, Cangzhu, Houpo (processed), Sanleng, Baizhu, Qingpi (each 5 qian), Xiangfu (1 jin), Gancao (2 liang), Zhen Sha (10 liang, stir-fried red with vinegar).
Grind into powder, mix with vinegar paste to form pills. Take 50 pills on an empty stomach with salt and ginger decoction; taking with wine before evening meal is also acceptable. Avoid dog meat.
In the above five formulas, Fuzi, Houpo, Shanzha, Qingpi, Sanleng, and Penglang are used to grind away accumulations; Zhen Sha and sour vinegar are used to drain the liver; Baizhu and Cangzhu are used to tonify the spleen. These are heavy agents to remove fearfulness. Only Wenzhong Wan lacks tonifying properties; do not use for deficiency.
[Shi] Treats Huang Pang. Lvfan (Green Vitriol, 6 liang), boiled seven times in rice vinegar in an iron pot until dry, placed on the ground to eliminate fire qi, then ground into powder. Nanxing powder (1 liang), stir-fried flour (1 liang), large Zaojiao (Giant Condensed Milk Vine Pod, 2 jin), boiled in water until soft, kneaded to extract thick gum, skins and dregs removed, filtered and boiled again, add 6 liang of Jujube, steamed until cooked, skins and pits removed, boiled into thick paste, mixed evenly with previous powders to form pills the size of paulownia seeds. Take 5 pills per dose, morning and evening with ginger decoction. Avoid greasy, fried, and roasted foods. If red spots appear on the body, immediately decoct jujube soup to drink; the spots will heal spontaneously.
[Ben] Treats men and women suffering from dietary and qi labor, generalized yellow swelling, impending hydrops (edema), and long-standing Pi masses (abdominal lumps), where sudden yellowing of the eyes, face, small intestine, and bladder occurs. It is advisable to take Zijin Dan.
Danfan (3 qian), Huangla (Beeswax, 2 liang), Dazao (Jujube, 50 pieces).
Place in a clay or silver/ceramic pot with 3 sheng of good vinegar. First add Fan and Zao, simmer slowly for half a day. Remove Zao, peel and pit, then add La, simmer slowly for another 1-2 hours. When the paste is ready, add 2 liang of Laccha (Old Tea), mix well to form pills the size of paulownia seeds. Take 20-30 pills per dose, with tea or wine as desired. For long-standing Changfeng hemorrhoids, take with aged rice drink. Three times daily, effective within one day. Zhao Yancai, a member of the royal clan, suffered from bloody stool, waxy complexion, and inability to eat, due to alcohol disease. Given this formula, bleeding stopped upon completion of the course, complexion became fresh and moist, and appetite doubled. A scholar from Xin'an had similar symptoms; given 300 pills divided into ten doses, he recovered immediately.
[Luo] Zaofan Wan treats dietary/labor jaundice, yellow eyes and body.
Zaofan (Black Vitriol, amount unspecified), place in a clay pot, heat until red-hot, sprinkle with rice vinegar, heat using charcoal.
Grind into powder, mix with jujube flesh to form pills. Take 20-30 pills per dose, with ginger decoction after meals. Another method uses Baifan (White Vitriol) instead of Zaofan.
[Shi] Zaofan Wan treats Huang Pang.
Zaofan (5 liang, processed), Jujube flesh (2 liang), Zhengfen (Steamed Powder, 3 liang).
Grind into powder, mix with ginger juice to form pills. Take 20-30 pills per dose, twice daily, with rice drink before meals.
The above three formulas use the acidity of Fan and vinegar to drain the liver, and the sweetness of jujube flesh to tonify the spleen. For deficiency-type yellowing, select from the formulas listed under the sections of "Urine flows freely" and "Spontaneous sweating deficiency jaundice" in the preceding Dan section.
[Ding] A thirty-year-old woman has a yellow face, weak and sore legs, bitter taste in the mouth, preference for tea, irregular menstruation, and frequent fatigue.
Huangqi (3 fen), Baizhu (1 fen), Chao Bai (Stir-fried Phellodendron/Coptis?, 2 fen), Qinjiao (2 fen), Gancao (3 fen), Mutong (5 fen), Chenpi (5 fen), Shaoyao, Renshen (each 1 qian), Danggui (1 qian).
A sixty-year-old woman has a yellow face, severe fatigue, sore legs, bitter taste in the mouth, and a scattered, large pulse; this is dampness injuring qi.
Baizhu (0.5 liang), Chenpi (4 qian), Cangzhu, Mutong, Huangqin (each 3 qian), Sharen, Renshen, Chuanxiong (each 2 qian), Huangbai (stir-fried, 1 qian), Gancao (roasted, 5 fen).
Divided into six doses. Decoct in water and take before meals.
Official Wang, after suffering from Pi (fullness/stagnation), has a yellow face, weak and sore legs, fatigue, shortness of breath after eating, and dizziness.
Huangqi (2 fen), Cangzhu (1 qian), Baizhu (1.5 qian), Mutong (2 fen), Gancao (roasted, 2 fen), Huangbai (stir-fried, 3 fen), Houpo (processed, 1 qian), Chenpi (1 qian).
Decoct in water and take.
Cheng Geng, an official of the fifth rank, has a yellow face, weak and powerless legs, indigestion, a deficient and slightly wiry pulse, bitter taste in the mouth, and abdominal distension; tonification is appropriate.
Cangzhu (0.5 qian), Mutong (3 fen), Baizhu (1.5 qian), Danggui (5 fen), Shaoyao, Chenpi (each 5 fen), Gancao (roasted, 2 fen), Chuanxiong (5 fen), Renshen (3 fen).
Decoct in water and take with 40 Baohe Wan pills.
[Jing] Treats Shi Qi (Food Qi), generalized Huang Pang, asthma, inability to eat, and chest/heart thirst and oppression. Use non-moth-eaten Zaojiao, remove skin and seeds, coat with good vinegar, roast until scorched, grind into 1 qian powder. Add 7 Baidu beans (Castor Beans), removing oil membrane. Grind both ingredients together with dilute vinegar and good ink to form pills the size of hemp seeds. Take 3 pills per dose, with Chenpi decoction after meals. Three times daily, increase by one pill every other day, stopping when bowel movement occurs. Regular use eliminates alcohol and food stagnation.
[Zihe] A man who married into a family (matrilocal marriage) accidentally contracted jaundice. He ate well but grew thin, could not lift his four limbs, had a yellow face, and lacked strength. His father-in-law wanted to discard him, but his wife refused. The father-in-law was a farmer who brought the son-in-law home intending for him to work; seeing his severe illness, he abused him daily. Someone advised him to take Lafan Wan and Sanleng Wan, but neither was effective. Dai Ren (Zihe) saw him, treated him without diagnosis, and administered a vomiting agent, expelling one dou of accumulated phlegm and old water. Then used Xieshui Wan and Tongjing San, causing diarrhea forty to fifty times continuously. Dai Ren ordered him to drink one cup of ice water, which stopped the diarrhea immediately. Subsequently, he took Pingwei San, interspersed with 50 Binglang Wan pills. Within seven days, the jaundice receded and strength returned. This is because jaundice is caused by damp-heat and old grain, commonly known as Shilao Huang.
[Shi] Likes eating dry tea.
Shigao, Baishaoyao, Huangqin.
Decoct in water and take. Refer to the Cha Dan (Tea Jaundice) section under the Ji Kuai (Accumulated Masses) chapter for combined use.
[Shan] Wine and Noodle Jaundice. Burn Loofah (including skin and seeds) to ash. If the disease is caused by noodles, mix with noodle water to take. If caused by wine, mix with wine to take.
