The Classics state: The Foot-Yangming Stomach Meridian, if Qi is excessive, causes heat throughout the anterior aspect of the body. (Acupoints such as Zusanli should be selected. See full text in Needling and Moxibustion section.)
[Suwen] The Yellow Emperor asked: Some people have hot limbs, and upon encountering wind and cold, it feels as if being roasted by fire. Why? Qibo answered: Such individuals have deficient Yin Qi and abundant Yang Qi. The limbs belong to Yang.
When two Yangs combine while Yin Qi is deficient and scanty, it is like a small amount of water failing to extinguish a roaring fire, leading to uncontrolled Yang. Uncontrolled Yang cannot foster growth; it merely prevails alone. Encountering wind feels like roasting because the person's flesh will waste away. (See "Ni Tiao Lun", where 'shuo' means wasting/emaciation.)
[Zhang Zhongjing] Sanwu Huangqin Tang (Three-Ingredient Scutellaria Decoction): Treats vexatious heat in the limbs of women. (Formula see Postpartum Vexatious Heat section.)
[Zhu Danxi] To treat heat in the palms and soles, use Dongyuan's Huoyu Tang (Fire-Stagnation Decoction). (Formula see Fire-Stagnant Heat section below.)
This formula for treating heat in the palms and soles is miraculous.
Ingredients: Zhizi (Fructus Gardeniae), Xiangfu (Rhizoma Cyperi), Cangzhu (Rhizoma Atractylodis Lanceae), Baizhi (Radix Angelicae Dahuricae), Banxia (Rhizoma Pinelliae), Chuanxiong (Rhizoma Chuanxiong).
Preparation: Make pills using dough made from Shenqu (Massa Medicata Fermentata).
[Li Dongyuan] Discussion on Five Differences of Body Heat: When pathogens affect the five Zang organs, they all cause body heat, but the manifestations differ. There are three methods of palpation:
1. If heat is felt with light touch but disappears with heavy pressure, the heat is in the skin, hair, and blood vessels.
2. If heat is intensely steaming when pressing deeply to the sinews and bones, but not felt with light touch, the heat is between the sinews and bones.
3. If heat is not felt with light touch nor heavy pressure, but is felt with moderate pressure, the heat is above the sinews/bones and below the skin/hair vessels, i.e., in the muscles.
These are the three methods; Sanhuan Wan (Three-Yellow Pill) can be used to treat them generally. Subdivided, the five Zang organs have distinct patterns.
Lung Heat: Felt with light touch, absent with slight pressure, faintly visible on the skin surface because the Lung governs the skin and hair. Worse in the evening; this is heat in the skin/hair. Symptoms must include panting/coughing and aversion to cold/fever. Mild cases use Xiebai San (Drain White Powder); severe cases use Liangge San (Cool the Diaphragm Powder), Baihu Tang (White Tiger Decoction), etc., or Dihuang Digupi San (Rehmannia and Bark of Dogwood Root Powder).
Heart Heat: The Heart governs blood vessels. Slight pressure to below the skin and above the muscles reveals heat with light touch; heat is felt slightly below the skin but disappears with more force, indicating heat in the blood vessels. Worst at noon; this is Heart heat. Symptoms include vexation, heart pain, heat in the palms, and hiccups. Treat with Huanglian Xiexin Tang (Coptis Decoction to Drain the Heart), Daochi San (Direct Red Powder), Zhusha Wan (Cinnabar Pill), Anshen Wan (Calming Spirit Pill), Qingliang San (Clearing Cooling Powder), etc.
Spleen Heat: Not felt with light touch, not felt with heavy pressure to bones; felt moderately between light and heavy pressure. This heat is in the muscles, worse at night. Symptoms include lethargy, desire to lie down, inability to hold the limbs, and lack of energy to move. For excess heat, use Xiehuang San (Drain Yellow Powder) or Tiaoweichengqi Tang (Regulate the Stomach and Order the Qi Decoction). For deficiency heat in the middle, use Renshen Huangqi San (Ginseng and Astragalus Powder) or Buzhong Yiwang Tang (Tonify the Middle and Benefit the Stomach Decoction).
Liver Heat: Pressing below the muscle to above the bone reveals Liver heat. Worst during Yin-Mao hours (3-7 AM); pulse is wiry. Symptoms include fullness/stuffiness in limbs, difficult defecation with tendon spasms, irritability,惊惧 (fright), heat in limbs, tendon flaccidity unable to rise from bed. Treat with Xieqing Wan (Drain Green Pill) or Chaihu Yin (Bupleurum Drink). Wiry pulses in both hands or onset during Yin-Shen hours indicate Liver heat; these formulas apply.
Kidney Heat: Light pressure yields no heat; deep pressure to the bone reveals steaming heat like fire. The person feels bone soreness as if insects are eating the bones, overwhelmed by heat, unable to rise from bed. Treat primarily with Zishen Wan (Nourish Kidney Pill) or Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). (Regarding pulse: Light pressure like weight of three beans, corresponding to skin and rapid, indicates Lung heat. Six beans, corresponding to vessels and rapid, indicates Heart heat. Nine beans, corresponding to flesh and rapid, indicates Spleen heat. Twelve beans, corresponding to sinews and rapid, indicates Liver heat. Pressure to bone, rapid return and rapid pulse, indicates Kidney heat.)
Huanglian (Coptidis Rhizoma) drains Heart Fire; Mutong drains Small Intestine Fire; Huangqin (Scutellariae Radix) drains Lung Fire and Large Intestine Fire; Chaihu (Bupleuri Radix) drains Liver Fire (assisted by Huanglian) and Gallbladder Fire (also assisted by Huanglian); Baishaoyao (Paeoniae Radix Alba) drains Spleen Fire; Shigao (Gypsum Fibrosum) drains Stomach Fire; Zhimu (Anemarrhenae Rhizoma) drains Kidney Fire; Huangbai (Phellodendri Cortex) drains Bladder Fire (also said to drain Dragon Fire).
[Wang Haizheng] Skin feels like burning, but heavy pressure does not feel very hot; this is Lung heat. White of eyes red, vexation, thirst, drink only Huangqin single herb decoction.
[Zhu Danxi] Qingjin Wan (Clear Metal Pill) attacks Spleen and Lung Fire.

Formulas for treating Kidney Heat (see Lao Zhai/Labor Disease section).
[Lingshu] Heat in the Stomach digests food quickly, causing a hanging feeling in the heart and hunger; skin above the umbilicus is hot. Heat in the Intestines causes yellow, porridge-like stool; skin below the umbilicus is hot. (Shichuan Chapter: Stomach is above umbilicus, so Stomach heat makes upper abdomen hot. Intestines are below, so Intestine heat makes lower abdomen hot. Liver/Gallbladder are in hypochondrium, so their heat makes hypochondrium hot. Lungs are in chest/back, so their heat makes chest/back hot. Kidneys are in waist, so their heat makes waist hot. Infer by analogy.)
[Luo Tianyi] Sudden heat indicates disease in Heart and Lungs. Accumulated heat indicates disease in Kidneys and Liver. Sudden heat: use Ju Fang Xionghuang Jiedu Wan (Realgar Detoxification Pill). Accumulated heat: use Ju Fang Miaoxiang Wan (Wonderful Incense Pill). Sudden type involves upper panting, disease in Heart/Lungs, called High Panting; use Muxiang Jinlingzi San. Upper Jiao heat with vexation: use Niuhuang San (Calculus Bovis Powder). Chronic emaciation with fever and night sweats is called "Five Zangs All Damaged," a condition of Heat Labor and Bone Steaming. Emaciation, deficiency, vexation/panting, intestinal rumbling, and bloody stool are all Steaming Labor. Treatment should nourish Blood and benefit Yin; heat will retreat naturally. This is "treating without treating"; use Qian's Dihuang Wan.
Xionghuang Jiedu Wan: Treats upper diaphragm congestion-heat, obstructed phlegm-saliva, sore throat/swelling, red eyes/carbuncles. (Formula see Throat Bi section.)
[Baoming Ji] Muxiang Jinling San: Treats sudden Heart/Lung heat, continuous upper panting.

Mix uniformly. Use 6 liang pure yellow wax and 7.5 qian Baijian (Alumen) refined with honey into pills. Each liang makes 30 pills. Swallow with rice drink. To accelerate action, prick one eye of the pill, soak in cold water briefly, then take.
Chronic heat belongs to Lao Zhai (Consumption/TB). Treatment see Zhai Men.
Heat syndromes analyzed separately involve differences in Qi/Blood, Exterior/Interior, Upper/Lower, and Five Zangs. Analyzed collectively: Qi Heat, Exterior Heat, Upper Jiao Heat, Heart/Lung Heat all belong to Yang and share similar treatments; Blood Heat, Interior Heat, Lower Jiao Heat, Kidney/Liver Heat all belong to Yin and share similar treatments. Middle Jiao and Spleen Heat are the same.
[Luo Tianyi] General treatment for Heat Qi: Use Huanglian Jiedu Tang (Coptis Detoxification Decoction) for severe heat, vexation, delirium, and insomnia.
Huanglian (7.5 qian), Huangbai, Shanzhi (each 0.5 liang), Huangqin (1 liang).
Cut into pieces. Take 5 qian, decoct in water, remove dregs, take hot. Repeat if ineffective.
[Wang Haizheng] Add Fangfeng (Saposhnikoviae Radix) and Lianqiao (Forsythiae Fructus) to the above formula to make Jinhua Wan (Golden Flower Pill) for Wind-Heat. Add Chaihu for childhood tidal fever. Combine half with Siwu Tang for female tidal fever.
[Li Dongyuan] Sanhuang Wan (Three-Yellow Pill): Treats accumulated heat in the Triple Burner for men and women. Upper Jiao heat attacking upward causes red/swollen eyes, painful head/neck, mouth/tongue ulcers. Middle Jiao heat causes chest/vexation, poor appetite. Lower Jiao heat causes red/dysuric urine, constipation. Heat in all five Zangs causes carbuncles/furuncles/sores. Also treats five types of hemorrhoids, anal swelling/pain, or fresh blood in stool. (Danxi says: This is a Triple Burner formula.)
Huanglian (cleaned), Huangqin (de-radiated), Dahuang (each 10 liang).
Powder finely, honey pills, size of paulownia seeds. Take 30 pills with cooked water. Adjust dosage based on Zang-Fu deficiency/excess. Suitable for children with accumulated heat.

(Danxi says: This enters Heart, Spleen, Liver, Kidney, Stomach meridians.)
Hanshuishi (Halloysitum Rubrum), Shigao, Cishi (Magnetitum), Huashi (each 3 jin, powdered, decoct in 1 dan water to 5 dou, strain, add following herbs): Xuanshen (Scrophulariae Radix, 1 jin, washed, dried, cut), Chenxiang (Aquilariae Lignum), Lingyangjiao (Saigae Tataricae Cornu), Xijiao (Rhinocerotis Cornu - *Note: Substitute with Buffalo Horn in modern practice*), Qingmuxiang (Aucklandiae Radix), Shengma (Cimicifugae Rhizoma) (each 5 liang), Dingxiang (Caryophylli Flos, 1 liang), Gancao (Licorice Root, 8 liang. These 8 herbs powdered, added to previous decoction, boiled to 1.5 dan, strain, add next two items): Xiaoyan (Mirabilite), Xiaoshi (Natrii Sulfas) (good quality 2 jin. Added to decoction, simmer gently, stir constantly with willow stick until ~7-8 sheng remains, pour into cup, wait half day until semi-solid, then add last two items): Chensha (Cinnabaris, 3 liang, finely ground), Shexiang (Moschus, genuine 1.2 qian, milk-ground. Added to semi-solid mass, mix evenly).
Store in cellar for 2 days. Take 1-2 qian with cold water after meals. Adjust carefully for adults/children. (Ju Fang adds 100 liang Gold.)
[Li Dongyuan] Yin covers Yang, Fire cannot extend; induce sweating. Classics say: Body like burning charcoal, sweat disperses it. Pulse wiry and rapid indicates Yin Qi; use wind herbs to raise Yang and emit Fire Stagnation, then rapid pulse will recede. For this disease, if pulse is rapid, use Huangbai with small amounts of Huanglian, Chaihu, Cangzhu, Huangqi, Gancao, plus Shengma. Sweating causes pulse to drop, illustrating "emit Fire Stagnation."
Chaihu Shengyang Tang (Bupleurum Raising-Yang Decoction): Treats limb/muscle/sinus/bone marrow heat in men/women, burning sensation, touching feels hot. Limbs governed by Spleen (Earth). Heat hidden in earth; often due to Blood deficiency. Also due to Stomach deficiency from excessive cold/raw food/ice water, suppressing Yang Qi in Spleen Earth. Classics: "Emit Fire Stagnation."
Shengma, Gegen (Puerariae Radix), Duhuo (Angelicae Pubescentis Radix), Qianghuo (Notopterygii Rhizoma et Radix) (each 0.5 liang), Fangfeng (Saposhnikoviae Radix, 2.5 qian), Gancao (Raw, 2 qian), Chaihu (5 qian), Zhigancao (Roasted Licorice, 2 qian), Renshen (Panax Ginseng, 0.5 liang), Baishaoyao (Paeoniae Radix Alba, 0.5 liang).
Cut into pieces. Take 0.5 liang, 3 large bowls water, boil to 1 bowl, strain, take slightly warm. Avoid cold/ice for over a month.
[Li Dongyuan] Huoyu Tang (Fire Stagnation Decoction): Treats Five Centers Vexatious Heat, Fire stagnant in earth. Limbs are Earth; Heart Fire sinks into Spleen Earth, so raise/emit Fire Stagnation. (Danxi: Use Dongyuan's Huoyu Tang for palm/sole heat. See Palm/Sole Heat section above.)
Shengma, Gegen, Fangfeng, Chaihugen (Bupleuri Radix), Zhigancao, Baishaoyao (each 5 qian).
Cut into pieces. Take 3-4 qian, 2 large bowls water, add 3 cun green onion white with root, boil, strain, take slightly warm.
Climate/Yun Heat has three types:
1. Fire Heat assists Heart Excess causing heat. Classics: Year of excessive Fire, hot summer spreads, severe case body heat, bone pain, spreading sores. Also: Shaoyang arrival causes sores/rashes/body heat. Also: Shaoyin ruling Heaven, guest victory causes headache. Shaoyang heat treated with Cold herbs.
2. Cold Evil attacks Heart Deficiency causing heat. Classics: Year of excessive Water, cold spreads, people suffer body heat, vexation/palpitations, Yin collapse. Also: Year of insufficient Metal,炎火 (flames/fire) acts, Cold returns causing Yin collapse, Gridlock Yang rises upward causing heat. Also: Shaoyin in泉 (subterranean), Master victory causes heat rising upward, vomiting from Gridlock heat. Master victory means Winter Cold defeats Shaoyang Guest Heat; treat with Hot herbs.
3. Wind fans Fire and Cold-Damp stagnates Fire causing heat. Classics: Taiyang ruling Heaven, First Qi, air becomes very warm, people suffer body heat/headache; this is Wind fanning Yang heat, treat with Clearing. Also: Fourth Qi, Wind-Damp contend, people suffer great heat/lack of Qi. Also: Shaoyang ruling Heaven, Second Qi, Fire reversely stagnates, white haze rises, people suffer headache/body heat. This is Cold-Damp stagnating Fire; treat according to degree of Cold/Heat.
[Lingshu] Yellow Emperor: Acupuncture chapters mention Cheyi (Removing Clothes), Master says pierce all odd acupoints of Yang meridians, no fixed location, wish to hear fully? Qibo: Yang Qi excess, Yin Qi deficiency. Yin deficiency causes internal heat; Yang excess causes external heat. Internal/External heat clash, hotter than holding charcoal. Fear cotton/silk near body, cannot lie on mats. Pores closed, no sweat, dry tongue/lips, parched throat, indifferent to food taste. Emperor: Good. How to treat? Qibo: Pierce Tianfu, Dazhu (3 points), pierce Zhonglü to remove heat, tonify Hand/Foot Taiyin to promote sweating. Heat gone, sweat rare, faster than removing clothes. (Cijie Zhenxie Chapter: La, pronounced si. Fire heat above, push down; from below going up, guide away.)
[Jiayi Jing] Body heat, chest/hypochondriac pain, inability to turn side: Luxi governs. Desire to lie down, body heat, immobility, dampness: Sanyangluo governs.
[Wang Haizheng] Hands too hot is Gujue (Bone Jue), feeling like in fire. Moxa Yongquan (KI1) 3 moxa cones for immediate relief.
[Cuyao] Palm heat: Yuji (LU10, needle horizontal backward 0.3 cun), Shaofu (HT8, 0.3 cun). Sole heat, damp/itch: Shugu (BL65, drain then tonify, moxa). Plantar heat, foot node pain: Gongsun (SP4, 0.5 cun drain).
[Suwen] Pulse floating and choppy, choppy with body heat indicates death. (Tongping Xushi Lun) Heat with quiet pulse is difficult to treat. (Yuji Zhenzang Lun) Pulse vigorous, sweat does not resolve indicates death. Pulse deficient, heat continues indicates death. (See detailed Shanghan Fever.)
