Prolonged fever (Chronic fever). Gan heat (Heat caused by gan syndrome) is also prolonged, but accompanied by yellowish complexion, pica for charcoal or soil, and erosion under the nose.
[Wu] In pediatric accumulated heat syndrome, there is heat in both the exterior and interior, with generalized body heat, flushed cheeks, dry mouth, dark red urine, and scorched yellow stool. Initially, use Sishun Yin (Four Compliance Decoction) and Qingliang Yinzi (Cooling Drink) to purge the zang-fu organs; once the heat is expelled through bowel movements and urination, the fever will subside. If the fever recurs after subsiding, it indicates that the interior heat has been resolved, but the exterior heat remains unresolved. In this case, Xingxing San (Xingxing Powder), Hongmian San (Red Silk Powder), plus Mahuang (Ephedrae Herba) should be used to induce mild sweating, so that the exterior heat may be eliminated.
Why does fever recur after the elimination of exterior heat? Physicians of the world often fail to comprehend this stage. Some administer cooling medicines again, some attempt to release the exterior again, while others deem it incurable, leading to many cases of mismanagement and premature death. This occurs because both the exterior and interior are deficient, Qi fails to return to its origin (Yuan), and Yang floats outward. Therefore, the recurrent fever is not a true heat syndrome. One should only use Liushen San (Six Deities Powder) with Jingmi (Oryza Sativa Semen, Rice Grain) decocted together to harmonize Stomach Qi, thereby drawing floating Yang back inside, resulting in the cooling of the body. For severe heat, Yinbai San (Silver White Powder) is used.
Qingliang Yinzi: Take 2 qian of Bohe (Menthae Haplocalycis Herba, 3 qian), decoct together. (Formula see section on treating chills and fever.) Xingxing San (Formula see section on smallpox and rashes.)
Hongmian San:
Baijiangcan (Bombyx Batryticatus, stir-fried, 2 liang), Tianma (Gastrodiae Rhizoma, raw, 1 liang), Tiannanxing (Arisomatis Tuberis Preparata, sliced thinly, oil-soaked until yellow, 2 liang), Sumujie (Sappan Lignum node, separately ground, 2.5 liang).
Grind all into fine powder. Take 1 qian per dose, with one small cup of water, add a little Hongmian (Red Silk), decoct together until reduced to 6 fen, and take warm. It is suitable for any pediatric wind-heat causing unclear head and eyes. If there is exterior heat due to Shanghan (Cold Damage) with exterior signs, add 5 fen of Mahuang (Ephedrae Herba, without nodes) powder per dose; if there is interior heat with dry mouth and thirst, add 0.5 qian of Huashi (Talcum) powder per dose, decoct together and take.
Liushen San:
Renshen (Ginseng Radix et Rhizoma), Baifuling (Poria, white part), Ganshanyao (Dioscoreae Hypoglaucae Rhizoma, dried), Baizhu (Atractylodis Macrocephalae Rhizoma), Baibiandou (Phaseoli Multiflori Semen), Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried). Each in equal proportions.
Grind into fine powder. Take one large qian per dose, with one small cup of water, one jujube, and two slices of ginger, decoct together until reduced to 5 fen, and take. This formula has many applications: add Fuzi (Aconiti Lateralis Radix Preparata) for cold stomach, add Tianma (Gastrodiae Rhizoma) for wind syndromes, add Yingsuke (Papaveris Pericarpium) for dysentery.
Yinbai San:
Ganshanyao (Dioscoreae Hypoglaucae Rhizoma, dried), Baizhu (Atractylodis Macrocephalae Rhizoma), Baifuling (Poria, white part) (each 0.5 liang), Renshen (Ginseng Radix et Rhizoma), Baibiandou (Phaseoli Multiflori Semen), Zhimu (Anemarrhenae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried), Shengma (Cimicifugae Rhizoma) (each in equal proportions).
