[From *Ling Shu*] The Yellow Emperor said: Among various diseases, most manifest as lucidity in the morning, tranquility during the day, exacerbation in the evening, and severity at night. Why is this? Qibo replied: In the morning, the *Zhen Qi* (Upright Qi) begins to generate, and the pathogenic qi declines; therefore, there is lucidity in the morning. At noon, the *Zhen Qi* grows strong; when strong, it overcomes the evil; therefore, there is tranquility. In the evening, the *Zhen Qi* begins to decline, and the pathogenic qi begins to arise; therefore, there is exacerbation. At midnight, the *Zhen Qi* enters the Zang organs, leaving the pathogenic qi alone in the body; therefore, there is severity. The Yellow Emperor asked: What if there are cases contrary to this pattern? Qibo replied: This occurs when the condition does not follow the Qi of the Four Seasons, but is solely governed by the Zang organs. In such cases, the disease becomes severe during the time element that the Zang organ cannot overcome (Sheng); it improves during the time element that the Zang organ can overcome (Ke). (From "Chapter on Dividing One Day into Four Times")
[From *Su Wen*] The Liver governs Spring, with the Foot Jueyin (Liver) and Foot Shaoyang (Gallbladder) meridians taking charge, corresponding to the Jia and Yi days. When disease is in the Liver, it heals in Summer. If it does not heal in Summer, it exacerbates in Autumn. If it does not lead to death in Autumn, it stabilizes in Winter. It rises/recovers in Spring; wind exposure should be avoided. For Liver disease, recovery occurs on Bing and Ding days. If not recovered on Bing and Ding days, it exacerbates on Geng and Xin days. If it does not lead to death on Geng and Xin days, it stabilizes on Ren and Gui days, and rises/recovers on Jia and Yi days. Patients with Liver disease feel lucid at dawn, severe in the late afternoon, and quiet at midnight. The Heart governs Summer, with the Hand Shaoyin (Heart) and Hand Taiyang (Small Intestine) meridians taking charge, corresponding to the Bing and Ding days. When disease is in the Heart, it heals in Late Summer. If it does not heal in Late Summer, it exacerbates in Winter. If it does not lead to death in Winter, it stabilizes in Spring. It rises/recovers in Summer; warm food and hot clothing should be prohibited. For Heart disease, recovery occurs on Wu and Ji days. If not recovered on Wu and Ji days, it exacerbates on Ren and Gui days. If it does not lead to death on Ren and Gui days, it stabilizes on Jia and Yi days, and rises/recovers on Bing and Ding days. Patients with Heart disease feel lucid at noon, severe at midnight, and quiet at dawn. The Spleen governs Late Summer, with the Foot Taiyin (Spleen) and Foot Yangming (Stomach) meridians taking charge, corresponding to the Wu and Ji days. When disease is in the Spleen, it heals in Autumn. If it does not heal in Autumn, it exacerbates in Spring. If it does not lead to death in Spring, it stabilizes in Summer. It rises/recovers in Late Summer; warm food, overeating, damp places, and wet clothing should be prohibited. For Spleen disease, recovery occurs on Geng and Xin days. If not recovered on Geng and Xin days, it exacerbates on Jia and Yi days. If it does not lead to death on Jia and Yi days, it stabilizes on Bing and Ding days, and rises/recovers on Wu and Ji days. Patients with Spleen disease feel lucid during the day, severe at sunrise, and quiet in the late afternoon. The Lung governs Autumn, with the Hand Taiyin (Lung) and Hand Yangming (Large Intestine) meridians taking charge, corresponding to the Geng and Xin days. When disease is in the Lung, it heals in Winter. If it does not heal in Winter, it exacerbates in Summer. If it does not lead to death in Summer, it stabilizes in Late Summer, and rises/recovers in Autumn; cold food and cold clothing should be prohibited. For Lung disease, recovery occurs on Ren and Gui days. If not recovered on Ren and Gui days, it exacerbates on Bing and Ding days. If it does not lead to death on Bing and Ding days, it stabilizes on Wu and Ji days, and rises/recovers on Geng and Xin days. Patients with Lung disease feel lucid in the late afternoon, severe at noon, and quiet at midnight. The Kidney governs Winter, with the Foot Shaoyin (Kidney) and Foot Taiyang (Bladder) meridians taking charge, corresponding to the Ren and Gui days. When disease is in the Kidney, it heals in Spring. If it does not heal in Spring, it exacerbates in Late Summer. If it does not lead to death in Late Summer, it stabilizes in Autumn. It rises/recovers in Winter; hot food and clothes heated by moxibustion/fire should be avoided. For Kidney disease, recovery occurs on Jia and Yi days. If not recovered on Jia and Yi days, it exacerbates on Wu and Ji days. If it does not lead to death on Wu and Ji days, it stabilizes on Geng and Xin days, and rises/recovers on Ren and Gui days. Patients with Kidney disease feel lucid at midnight, severe at the end of each season (Chen, Xu, Chou, Wei), and quiet in the late afternoon. Generally, when pathogenic Qi invades the body, it acts according to the cycle of overcoming (Ke). It heals when reaching the generating (Sheng) phase, becomes severe when reaching the overcoming phase, stabilizes when reaching the generating phase again, and recovers when returning to its own position. One must first determine the pulse of the Five Zang organs to discuss the times of remission and exacerbation, as well as the periods of life and death. (From "Chapter on the Methods and Times of Zang Organs")
The above methods for predicting disease recovery and exacerbation based on the Five Zang organs and time/day are extremely accurate. Qian Zhongyang (Qian Yi) deeply understood this principle. Students of medicine should study it carefully and expand upon it.
[From *Ling Shu*] When the disease is in the Zang organs, a pulse that is deep and large indicates the disease will easily heal; a small pulse is adverse. When the disease is in the Fu organs, a pulse that is floating and large indicates the disease will easily heal. A Cun Kou (Radial artery) pulse that is slippery, small, tight, and deep indicates the disease is worsening, located internally. a Ren Ying (Carotid artery) pulse that is large, tight, and floating indicates the disease is worsening, located externally. A Cun Kou pulse that is slippery and deep indicates the disease is progressing daily, located internally. A Ren Ying pulse that is slippery, vigorous, and floating indicates the disease is progressing daily, located externally. A Cun Kou pulse that is floating and slippery indicates the disease is progressing daily. A Ren Ying pulse that is deep and slippery indicates the disease is diminishing daily.
