If food intake is excessive, the Spleen and Stomach will be damaged. Consequently, Stomach Qi cannot perform its transformation and transportation functions, and Spleen Qi cannot distribute to all parts of the body. Therefore, dampness flows downward, invading the positions of the Liver and Kidney, and stagnating in the lower legs (tibia). Added to this, if sexual activity is not regulated, it leads to an excess of Yin and deficiency of Yang, eventually forming a chronic disease. Sun Simiao said: In ancient times, few people suffered from this disease. Since the southward migration during the Yongjia era, many scholars and officials have suffered from it; this is also the reason.
Danggui Niantong Decoction (Angelica Sinensis Decoction for Painful Dampness): Treats diseases caused by damp-heat, characterized by annoying pain in limbs and joints, heaviness in the shoulders and back, obstruction in the chest and diaphragm, accompanied by whole-body pain, with dampness flowing into the hands and feet, causing unbearable swelling and pain in the lower legs.
Ingredients: Qianghuo (Notopterygii Rhizoma et Radix), Gancao (Licorice Root, honey-fried), Huangqin (Scutellariae Radix, wine-fried), Yichen (Artemisiae Scopariae Herba, each half tael, wine-fried); Renshen (Ginseng Radix), Kushen (Sophorae Flavescentis Radix, wine-washed), Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix); Cangzhu (Atractylodis Rhizoma), Dangguishen (Angelicae Sinensis Radix, each two qian); Zhimu (Anemarrhenae Rhizoma, wine-washed, three qian); Zexie (Alismatis Rhizoma, three qian), Zhuling (Polypori Porria, three qian), Baizhu (Atractylodis Macrocephalae Rhizoma, one and a half qian), Fangfeng (Saposhnikoviae Radix, three qian).
Usage: Decoct in water, take at any time.
The *Bencao Shiji* (Ten Categories of Materia Medica) states: "Xuan" (dispersing) can remove obstruction, "Tong" (unblocking) can remove stagnation. The *Neijing* states: "When dampness prevails excessively, treat it with bitter-warm herbs." Qianghuo is bitter and acrid, penetrating the joints and overcoming dampness; Fangfeng and Gancao are warm and dispersing, clearing dampness retained in the meridians; thus, they serve as the Sovereign (Jun) herbs. Water nature is moist and tends to flow downward. Shengma and Gegen are bitter, acrid, and neutral, with thin flavors, representing Yang within Yin, guiding the medicinal action upward through bitter dispersion. Baizhu is bitter, sweet, and warm, harmonious and peaceful, removing dampness; Cangzhu is light and floating in texture, vigorous in qi, removing dampness between the skin and pores; thus, they serve as the Minister (Chen) herbs. When blood is obstructed and does not flow, pain occurs. Dangguishen is acrid and warm, dispersing stasis, ensuring that blood and qi return to their proper places. Renshen and Gancao are sweet and warm, tonifying the Spleen and nourishing righteous Qi, preventing bitter-cold herbs from damaging the Spleen and Stomach. Zhang Zhongjing stated: "When damp-heat combines, it causes annoying pain in limbs and joints." Kushen, Huangqin, Zhimu, and Yichen are bitter and cold, using bitterness to drain heat. All are processed with wine or fried to utilize their specific properties. Treating dampness without promoting urination is incorrect treatment. Zhuling is sweet, warm, and neutral; Zexie is salty and neutral; using bland flavors to渗 (percolate/drain) dampness, they also guide retained fluid; thus, they serve as the Assistant (Zuo) herbs. The combination of flavors allows the upper and lower diversion of dampness, enabling the stagnant Qi to be dispersed and unblocked.
Crush the above herbs. Take one tael per dose, add two and a half cups of water, soak briefly, decoct until one cup remains, strain, and take warm. Take on an empty stomach, wait a while, then eat a good meal to settle the medicine. Take one dose before bed, no need for food to settle it.
He Zhanyou, Secretary of the Secretariat, was forty-one years old, with a robust physique. In the spring of Bingchen year, he campaigned to Yangzhou. Suddenly, Hongjiaojiao (podagra/gout-like condition) attacked. His whole body and limbs were slightly swollen. The pain was so severe that he could not bear touch, especially in the lower legs. He could not wear shoes, rode barefoot, controlled the stirrups with his hands, and placed his feet in a bamboo container due to the urgency. He sought treatment. I recalled the *Neijing*: "Fluid retention inside causes swelling above." It also says: "All pain is excess; where blood is excess, one should puncture to bleed." I used a Sanleng needle (Triangular needle) to prick the swollen area several times. Dark blood gushed out, rising more than two chi high, then flowing like a thread onto the ground, about half a sheng in volume, black-purple in color. Soon after, the swelling subsided and pain decreased. I administered Danggui Niantong Decoction, one and a half taels. He slept that night and was cured after taking another dose the next day.
The *Zhouhou Beiji Fang* states: For Fengdu Hongjiaojiao (Wind-Toxin Podagra), if the lower legs are full and pressing leaves an indentation, simply drink two to three sheng of black bull urine frequently to promote urination, and the swelling will gradually subside. It is best to use fresh urine collected in a copper vessel. Pure yellow urine can also be used.
Li Gao recorded: Yang Dashou said: Since ancient times, the treatment for Hongjiaojiao has favored purgative methods because the disease is caused by obstruction. However, one must not overdo it; excessive purging damages the Spleen and Stomach, preventing the transporting Qi from ascending, which instead sinks downward causing Hongjiaojiao. Nor should one under-treat it; insufficient treatment fails to dissipate the obstructive Qi. Hereafter, I present three formulas to be used after detailed differentiation of deficiency and excess.
Qianghuo Daozhi Decoction (Notopterygium Decoction to Guide Stagnation): Treats early-stage Hongjiaojiao with whole-body pain or swelling and pain in limbs and joints, and obstruction of urination and defecation. Use this formula first to guide the pathogen, then use Danggui Niantong Decoction to eliminate it.
Ingredients: Qianghuo, Duohuo (Angelicae Pubescentis Radix, each half tael); Fangji (Stephaniae Tetrandrae Radix, three qian); Dahuang (Rhei Radix et Rhizoma, wine-baked, one tael); Danggui (Angelicae Sinensis Radix, three qian); Zhishi (Aurantii Fructus Immaturus, flour-fried, two qian).
Usage: Crush. Take five or seven qian per dose, add two cups of water, decoct until seventy percent remains, take warm. Stop when slight diarrhea occurs; adjust dosage according to deficiency or excess.
Kaijie Daoyin Pills (Unbinding and Guiding Fluid Pills): Treats indigestion and epigastric fullness/stuffiness.
Ingredients: Chenpi (Citri Reticulatae Pericarpium), Baizhu, Zexie, Fuling (Poria), Shenqu (Massa Medicata Fermentata), Maiya (Hordei Fructus Germinatus), Banxia (Pinelliae Rhizoma, prepared, each one tael); Zhishi, Qingpi (Citri Reticulatae Pericarpium Viride), Ganjiang (Zingiberis Rhizoma, each half tael).
If there are accumulations, add Badoushuang (Crotonis Semen Pulvis, one and a half qian).
Usage: Grind into powder. Make pills using rice dough steamed cakes, size of paulownia seeds. Take thirty to fifty or up to seventy pills per dose, with warm water, well after meals.
Another Formula: Treats damp-heat and various damp pathogens attacking, causing heavy and painful waist and knees, and swollen lower legs.
Ingredients: Binglang (Arecae Semen), Gansui (Kansui Radix), Chishao (Paeoniae Radix Rubra), Weilingxian (Sinomenii Caulis), Zexie, Tingli (Lepidii Semen), Xurong (Frankincense, ground, each two liang); Moyao (Myrrha, ground, one liang); Qianniu (Pharbitidis Semen, half liang); Daji (Euphorbiae Pekinensis Radix, fried, three liang); Chenpi (four liang).
Usage: Grind into powder. Make pills using wheat paste, size of paulownia seeds. Take fifty pills per dose, increasing to seventy or eighty, with warm water before meals. Change clothes after taking, then stop further doses. Avoid alcohol for two days prior to medication, and avoid noodles and Gancao for three days. Eat warm, bland congee to nourish the Stomach.
In the spring of Yisi year, Lian Pingzhang was thirty-eight years old, obese. Hongjiaojiao began to attack. His head, face, and whole body limbs were slightly swollen, all red. His lower legs were red and swollen, pain unbearable. He dared not lean on a cane; touching the skin increased the pain. He rose and lay down repeatedly, suffering day and night indescribably. He sought my treatment. Pingzhang lived in the cold north and drank much wine, damaging the Spleen over time, failing to transform and transport, leading to dietary fluids sinking downward. I prescribed Danggui Niantong Decoction, one tael and two qian. The pain halved. After another dose, swelling and pain completely disappeared. Only the tip of his right finger remained slightly red and swollen. I pricked the nail end with a Sanleng needle, releasing much dark blood, and the redness and swelling vanished. Not long after, eating wet noodles caused limb pain again. I treated him with Zhishi Dahuang Tang (Aurantium and Rhubarb Decoction).
Ingredients: Qianghuo (one and a half qian), Danggui (one qian), Zhishi (five fen), Dahuang (wine-baked, three qian).
Hongjiaojiao is caused by water and dampness. Noodles generate dampness, blood becomes obstructed and does not flow, causing annoying pain in limbs and joints. The *Neijing* says: "Wind overcomes dampness." Qianghuo is acrid and warm, penetrating joints and removing dampness, so it serves as the Principal herb. Blood stagnation causes pain; Danggui is acrid and warm, dispersing obstruction and stopping pain. Zhishi is bitter and cold, treating fullness and digesting food, so it serves as the Minister herb. Dahuang is bitter and cold, guiding the damp-heat from noodles and treating old blood stasis; taken for its swift and powerful action, it serves as the Envoy herb.
Crush. Prepare one dose only. Add one and a half cups of water, decoct until one cup remains, strain, and take warm. Take on an empty stomach before meals. After two bowel movements, the pain stops.
Worldly physicians record: Hongjiaojiao. Kongxian Dan (Controling Phlegm Pill) plus Yanzhi (Vermilion Powder, one qian), Binglang (Arecae Semen, one liang), Mugua (Chaenomelis Fructus, one liang), Juanbai (Selaginella Tamariscina, half liang). First boil in salt water for half a day, then in plain water for half a day the next day. Mix with the above herbs to make pills. Take thirty pills initially, increasing to forty or fifty. Expelling foul matter brings immediate effect. (See Kongxian Dan formula under Wandering Pain.)
*Yanyi* records: Someone addicted to wine, drinking fifty to seventy cups daily, later suffered severely from Hongjiaojiao. Someone taught him to use Bajitian (Morinda Officinalis Radix, half liang) stir-fried with glutinous rice until the rice changes color slightly. Discard the rice. Use Dahuang (Rhei Radix et Rhizoma, one liang), chopped and stir-fried. Grind together into powder. Make pills with mature honey. Take fifty to seventy pills with warm water. Continue abstaining from wine, and he was cured.
*Bencao Gangmu* records: Treating wind-damp accumulation forming Hongjiaojiao, often feeling slight swelling, sometimes painful upon onset. Use Yinyu Wan (Ilex Rotunda Pill).
Ingredients: Yinyu (Ilicis Rotundae Folium, fried), Yiyiren (Coicis Semen, each half liang); Yuliren (Pruni Semen, one liang); Qianniu Zi (Pharbitidis Semen, three liang, raw powder one and a half liang).
Usage: Grind finely. Make pills with refined honey, size of paulownia seeds. Take twenty pills with Ginger-Jujube Decoction. If no bowel movement, increase to thirty pills. Three times daily, stop when bowel movement is smooth. Supplement with white congee.
Formula for Kidney Wind attacking the feet and knees.
Lianzhu (Pearl Cinnabar? Text implies specific mineral/herb form), Gansui (one liang), Miebzi (Momordicae Semen, one male, one female, shell removed).
Usage: Grind into powder. Take two pig kidneys, slice open. Sprinkle one qian of medicine evenly inside. Wrap in wet paper multiple layers. Roast over fire until cooked. Let cool to warm. Chew thoroughly at dawn (Wu-geng time) and swallow with rice drink. More water retention leads to more diarrhea; less retention leads to less diarrhea. Consume soft rice for recovery. If only one foot is affected, check left/right: use left kidney for left foot, right kidney for right foot. Medicine amount limited to one qian.
In the Renzi year in Piling, a surnamed Ma sold wine. We hadn't seen each other for a long time. I asked his son, who said: "He has a chronic Kidney Wind condition. Now one leg is swollen like a gourd. From waist down, thickness is uniform. Pain is unbearable. To turn or shift position, two people must hold him. Some suggested using a Pi Dao (scalpel) to cut and drain." I said: "No. I have medicine to prepare and give you." Follow the above method. Between Chen and Si hours (7-11 AM), he excreted several sheng of pus/water. Pain stopped instantly, swelling receded. One month later, he still walked with a cane. I gave him Chiwu San (Red Bird Powder) to apply to his knee. Only then was he cured. Ten years later, I went to Piling. He led his sons to bow in gratitude, saying: "The previous foot ailment has never returned. Although accumulated years of Kidney Wind have disappeared, I now walk vigorously as usual."
*Cao* records: Someone critically ill, unable to put feet on the ground for ten years. Good doctors exhausted their skills but could not cure him. Relatives placed him by the road seeking help. A wandering monk saw him and said: "This disease can be cured with one herb, but I don't know if it exists here." He went into the mountains to search and found it: Weilingxian (Sinomenii Caulis). Having him take it for a few days allowed him to walk. Later, Deng Siji, a mountain dweller, learned of this method and recorded it: Find Weilingxian, dry in shade for over a month, grind and sieve. Mix two qian bi with warm clear wine, take on an empty stomach. Due to its strong nature, dosage can be increased to six qian bi. If diarrhea exceeds twice, reduce dosage. Stop when disease is cured. Its nature is very mild, does not conflict with other herbs, but dislikes tea and noodle soup. Gancao and Zhizi can replace drinks.
*Jiyan Fang* records: Treating Kidney Wind obstruction and accumulation, heavy waist and knees. Weilingxian powder, mixed with honey into pills, size of paulownia seeds. Initial dose: eighty pills with wine. At dawn, slight diarrhea expels foul matter like green pus or peach gum; this is wind-toxin accumulation. If no movement, take one hundred pills again at night. After expulsion, eat congee to supplement for one month. Continue taking warm-tonifying medicines.
Dairen (Zhang Congzheng) records: Xi Shuai had heavy and painful waist and thighs, inconvenient walking and riding. Some treated as Hongjiaojiao cold-damp, some as deficiency. Wu Tou (Aconiti Radix), Fu Zi (Aconiti Lateralis Radix Praeparata), Xurong (Frankincense), Moyao (Myrrha)—blood-invigorating and tendon-strengthening drugs—were all used. After sixty-plus days, eyes turned red, upper body felt hot, urination and defecation became difficult, waist/thigh condition unchanged. Dairen palpated both wrists: pulse deep and slow. Deep indicates interior; should purge. One dose of Zhouche Wan (Boat Cart Pill) and Junchuan San (Clear River Powder) expelled over twenty loads of accumulated water. By morning, ate one or two bowls of pickled vegetable white congee. Put him on horseback; he was already vigorous.
Li Shibalang from Tangxi suffered from inability to extend waist and legs, stooping and limping for years. Medicines failed; stopping them helped. Relapse occurred after wading in water in late autumn. Doctor prescribed Sijin Wan (Four Jin Pill). His father, Li Zhongan, begged medicine from Dairen. Dairen asked: "What did you take recently?" Zhongan: "Sijin Wan." Dairen: "Are your eyes dim and red?" Father shocked: "Eyes suddenly flared up." Dairen: "Come quickly, or you will go blind." Initially came leaning on a cane, eyes swollen and blind. Dairen first induced vomiting/sweating (Yong Fa). Medicine caused twenty bowel movements. Eyes cleared instantly; cane discarded. After repeated purging, he could read calendars. Regulated for one month, took Danggui Wan, returned walking vigorously.
*Shanghan Lun* records: Sour taste injures tendons; tendon injury causes flaccidity, named Xie (Leakage). Salty taste injures bones; bone injury causes weakness, named Ku (Wither). Wither and Leakage combine, named Duanxie (Severance). Ying Qi (Nutritive Qi) blocked, Wei Qi (Defensive Qi) cannot move alone. Ying and Wei both weak, Sanjiao (Triple Burner) lacks nourishment, four extremities severed, body emaciated, only feet swollen, yellow sweat, cold calves. If fever attacks, it becomes Lijie (Wandering Joint) Bi syndrome. (Treat with Guizhi Jia Huangqi Tang [Cinnamon Twig Decoction plus Astragalus] etc.)
Zhu Danxi records: Xun's soles felt like cone stabbing, sometimes swollen. Wrists also painful and swollen. Diarrhea slippery, tenesmus. This is blood deficiency combined with dampness affecting the Lower Jiao Blood aspect. Treat with Jianbu Wan (Walking Strongly Pill). (Note: "Wrists" likely refers to ankles).
Ingredients: Shengdi (Rehmanniae Radix, one and a half liang), Cangzhu, Shaoyao (Paeoniae Radix), Chenpi (each one liang); Dafuzi (Arecae Pericarpium, three qian); Niuxi (Achyranthis Bidentatae Radix, half liang); Guiwei (Angelicae Sinensis Radix tail), Zhuyu (Corni Fructus), Huangqin (each one liang); Guizhi (Cinnamomi Ramulus, three qian).

Usage: Powder. Honey pills, paulownia seed size. Take fifty pills, with warm wine and salt water.
*Bencao Gangmu* records: Treating Liver-Kidney wind-deficiency and weak Qi, feet unable to touch ground, waist/spine pain, wind-toxin flowing down meridians, difficulty walking/standing, dribbling urine. This medicine tonifies internal injuries of Five Zang organs, regulates middle, boosts Qi, cools blood, strengthens bones/tendons, benefits intelligence, lightens body, delays aging. Use Xuduan Wan (Teasel Pill).
Ingredients: Sixianmu (Eucommia/Duzhong, five liang), Wujiaopi (Acanthopanacis Cortex), Fangfeng, Yiyiren, Qianghuo, Chuanduan (Dipsaci Radix, each three liang); Bibai (four liang), Shengdi (five liang), Niuxi (wine-soaked, three liang).
Usage: Powder. Use three sheng good wine, dissolve three liang green salt. Use half jin papaya (Mugua), peel/remove seeds, boil in salt-wine to paste. Mix and pound into pills, paulownia seed size. Take thirty to fifty pills, empty stomach before meals, with warm wine-salt water.
*Bencao Gangmu* records: Treating men's waist-leg Bi syndrome, stiffness/looseness, unstable walking. Use Bibai twenty-four parts, Duzhong eight parts. Grind/sieve. Take three qian bi daily with warm wine, increase to five qian. Avoid beef.

Drink one or half cup warm on empty stomach daily, three to four times. Keep a slight buzz. Long-term use feels like hundreds of insects crawling under skin; this indicates wind-damp dispersing.
*Bencao Gangmu* records: Dispelling wind, tonifying blood, boosting Qi, strengthening bones/tendons, strengthening legs. Use Hugu Jiu (Tiger Bone Wine).
Ingredients: Hujingggu (Tiger Tibia Bone, authentic), Bibai, Xianlingpi (Epimedii Folium), Yiyiren, Niuxi, Shudi (each two liang).

Usage: Divide into two decoctions.
Hongjiaojiao obstruction/swelling, urinary/fecal blockage, epigastric fullness, indigestion. Should mildly purge. (See Internal Injury Damp-Heat section for formula.)
Hongjiaojiao mostly belongs to Lung Qi Excess. Classics say: "Lung disease causes sweating; pain in buttocks, yin, thighs, knees, hips, and feet." Thus, Dairen uses Yong Fa (Inducing Vomiting/Sweating) for Hongjiaojiao, rightly so. Also, *Qianjin Fang* uses sweating methods, which is also draining Lung method. Below are formulas to vent Lung Qi.
Dairen records: Wei Dexin, traveling for winter selection, caught cold. True Yuan Qi declined. Sat/laid in cold damp. Irregular diet. Winter exposure caused Gu Bi (Bone Bi). Bones belong to Kidney. High bones of waist destroyed/useless. Hips feel broken. Face black as charcoal. Front/back pain. Wei Jue (flaccidity/cold limbs) and drowsiness. Asked many doctors, all treated as Kidney Deficiency. I first used Linglong Zao (Spirit Furnace) moxibustion/steaming for days. Then used bitter agent to vomit two to three sheng of cold phlegm. Upper excess, lower deficiency clearly visible. Then used bland agent: Baizhu removes Spleen damp, Fuling nourishes Kidney water. Also pierced Shenshu (BL23) and Taixi (KI3) points, once every two days. Within one month, recovered fully.
Sun Shaofu Han Yanzheng suddenly fell ill, hands/feet unable to lift. Doctors thought it was Wind, needled hands/feet, patient felt no pain. Summoned Sun Simiao. Sun said: "This is Hongjiaojiao." Used Binglang powder (three qian), Ginger (three slices), Dry Perilla (seven leaves), Chenpi (three qian). One cup water, decoct to seventy percent, strain, take hot. Cured after several doses.
For foot swelling, use Binglang San (Arecae Powder).

Usage: Boil briefly. Mix in two qian Binglang powder. Take after meals.
*Shengji Zonglu* records: Treating Hongjiaojiao and wind-cold-damp, limb spasms, feet too painful to touch ground. Use Perilla (two liang), crush. Three sheng water, extract juice. Cook two he of japonica rice porridge with Perilla juice. Eat with scallion, fermented soybean, pepper, ginger. Stops immediately.
Above are examples of Binglang and Perilla.
Worldly physicians record: Miracle formula for all Hongjiaojiao. Use front two feet of Pangolin (Chuan Shan Jia), burn to retain nature, grind fine. Add little Moshe Xiangdangzi (Musk). Dosage varies. To take, drop other matters. Do not eat dinner. Take at late night when empty. Sit/lie as desired. At cockcrow, take another dose. Pain stops immediately. Walk normally in one or two days. Extremely effective.
*Qianjin Fang* records: Treating Hongjiaojiao Twelve Winds Bi unable to walk. Use Pine Needle Wine. Sixty jin pine needles, finely chop. Four shi water, boil to four dou nine sheng. Brew five dou rice normally. Separate pine juice to soak rice and mix with rice. Seal with mud. Seven days later, clarify and drink. Get drunk. Many benefit from this wine power.
*Bencao Gangmu* records: Hugu Jiu (Tiger Bone Wine): Treats waist/leg pain, spasms, inability to bend/extend, cold/numb legs/knees.
