Treats Jueyin disease (diseases of the Foot-Jueyin Liver meridian) presenting with excessive thirst, sensation of Qi rushing up to strike the heart, pain and burning heat in the heart region, hunger without desire to eat, and vomiting of roundworms after eating. It also primarily treats chronic dysentery.
Wumei (Mume Fruit) 300 pieces, Xixin (Asarum Root) 6 liang, Ganjiang (Dried Ginger) 10 liang, Huanglian (Coptis Rhizome) 1 jin, Danggui (Angelica Sinensis Root) 4 liang, Fuzi (Prepared Aconite Root) 6 liang, Shujiao (Sichuan Pepper, remove oil/sweat) 4 liang, Guizhi (Cinnamon Twig) 6 liang, Renshen (Ginseng Root) 6 liang, Huangbai (Phellodendron Bark) 6 liang. The above ten ingredients are separately pounded and sifted into powder, then mixed thoroughly. Soak Wumei in vinegar for one night, remove the pits, and steam them. Place them under five sheng of rice, utilize the heat from the cooked rice to pound them into a paste. Mix this paste evenly with the herbal powders, place in a mortar, add honey, and pound two thousand times until pills the size of tung tree seeds are formed. Take ten pills before meals, three times daily, gradually increasing to twenty pills. Avoid raw, cold, slippery, and foul-smelling foods.
[Collected Annotations] Ke Qin stated: Among the six Yin syndromes, Jueyin is the most difficult to treat. Its root is Yin, its branch manifestation is Heat, its substance is Wood, and its function is Fire. One must suppress its dominant pathology and address its etiology first; whether by astringing, dispersing, counteracting, or following, treatment should proceed according to what is beneficial. Regulate the Qi of the Middle Jiao to restore harmony; this is the method for treating Jueyin. Jueyin occurs when two Yins reach their end, also known as the "extreme Yang of Yin," theoretically devoid of Heat. However, its principle follows the cycle of dark and light; as Yin begins to exhaust, Yang begins to generate. Thus, the presence of Heat in Jueyin disease is caused by Shaoyang. When Fire is excessive, Water becomes deficient, leading to excessive thirst, Qi rushing up to strike the heart, and pain/burning heat in the heart region; excess Qi transforms into Fire. When Wood overacts, it controls Earth, leading to hunger but no desire to eat. Worms arise from Wind transformation; when hungry, the Stomach is empty, and roundworms emerge upon smelling food odor, resulting in vomiting worms. Zhang Zhongjing's formulas generally use Sweet, Bitter, Pungent, and Sour herbs as sovereign agents, avoiding sour astringents. This formula uses them because Jueyin governs the Liver. The *Hongfan* states: "Wood is pliant and straight, producing Sour." The *Neijing* states: "Wood generates Sour, and Sour enters the Liver." Using the great Sourness of Wumei as the sovereign agent suppresses the dominant pathology. Pairing with Huanglian (Coptis Rhizome) to drain Heart Fire eliminates pain; assisting with Huangbai (Phellodendron Bark) to nourish Kidney Yin eliminates thirst; this addresses the etiology. Lianbai (Huanglian and Huangbai) are sufficient for Jueyin Yang pathogens but insufficient for Yin pathogens. Shujiao (Sichuan Pepper), Fuzi (Prepared Aconite Root), Xixin (Asarum Root), and Ganjiang (Dried Ginger) are all large Pungent-Heat herbs used together. They not only treat Jueyin Yin pathogens but also disperse the Liver, which desires dispersion, using Pungent herbs to do so. Adding Guizhi (Cinnamon Twig) and Danggui (Angelica Sinensis Root) benefits the Blood of the Liver, seeking its associated organ. Mixing Cold and Heat herbs causes disharmony of flavors; thus, Renshen (Ginseng Root) is added to regulate Middle Jiao Qi. Soaking Wumei in vinegar seeks similar Qi; steaming under rice utilizes Grain Qi. Making pills with Honey and administering slowly while gradually increasing dosage treats the root condition slowly. Roundworms are insects; raw/cold substances combine with Damp-Heat, so the medicine uses both Cold and Heat. With chest irritability and vomiting worms, Huanglian and Huangbai employ "treating heat with heat" methodology. Roundworms become quiet with Sour, dormant with Pungent, and descend with Bitter; this is indeed an excellent formula for treating parasites. Chronic dysentery leads to deficiency; regulating Cold and Heat and using Sour to astringe will naturally stop the dysentery.
