Treats chronic dysentery that does not cease, deficiency and collapse of the Large Intestine (Large Intestine), where taking Lizhong Wan (Regulate the Middle Pill) results in increased diarrhea.
Chishizhi (Kaolin, crushed) one jin, Yuyuliang (Magnetitum, crushed) one jin. The above two ingredients are boiled in six sheng of water to obtain two sheng of decoction. The dregs are removed, and the liquid is divided into three warm doses.
[Collected Annotations] Ke Qin stated: Ganjiang (Dried Ginger), Renshen (Ginseng Root), and Baizhu (Atractylodis Rhizoma) can tonify the deficiency of Yuanqi (Primal Qi) in the Middle Jiao (Middle Burner), but they are insufficient to secure the slippage of essence and fluids in the Lower Jiao (Lower Burner). Since this dysentery is located in the Lower Jiao, Li Zhong Tang-type formulas cannot achieve success. However, the insecurity of the Large Intestine is still attributed to the Stomach; the failure of the "gate" to close is still attributed to the Spleen. These two mineral drugs are formed from the refined qi of Earth; they solidify the Stomach and astringe the Intestines. Urgently treating the branch symptoms of the Lower Jiao actually serves to cultivate the root of the Middle Jiao. It must be understood that this syndrome involves deficiency of Earth but not deficiency of Fire; therefore, Ganjiang (Dried Ginger) and Fuzi (Prepared Aconite Lateral Root) are unsuitable. If dampness is severe and deficiency is not significant, and dysentery persists, then promoting urination should be employed.
Treats Jueyin Heat Dysentery characterized by tenesmus (heavy sensation in the rectum), a deep and wiry pulse, and thirst with desire to drink water.
Baitouweng (Pulsatillae Radix) three liang, Huanglian (Coptidis Rhizoma) three liang, Huangbai (Phellodendri Cortex) three liang, Qinpi (Fraxini Cortex) three liang. The above four ingredients are boiled in seven sheng of water to obtain three sheng of decoction. The dregs are removed, and one sheng is taken warm. If the illness persists, another sheng is taken.
[Annotation] All three Yin meridians present with downward discharge syndromes. Spontaneous diarrhea without thirst belongs to Taiyin Syndrome; spontaneous diarrhea with thirst belongs to Shaoyin Syndrome. Only Jueyin downward discharge presents as cold syndrome, characterized by cold extremities, absence of thirst, and passage of undigested food; or as heat syndrome, characterized by excessive thirst, tenesmus, and passage of pus and blood. This heat dysentery with tenesmus arises from stagnant fire steaming dampness, causing turbid qi to rush violently through the Changchang (Broad Intestine) and Pomen (Anus), resulting in heaviness, stagnation, and difficulty in expulsion. As stated in the *Neijing* (*Inner Canon*), this corresponds to "sudden purging with urgent straining." Baitouweng (Pulsatillae Radix) is used as the Sovereign herb for its cold nature and bitter-pungent taste; Qinpi (Fraxini Cortex) is used as the Minister herb for its cold nature and bitter-astringent taste. Cold nature overcomes heat; bitter nature dries dampness; pungent nature disperses stagnant fire; astringent nature secures against tenesmic diarrhea. Coptidis Rhizoma (Huanglian) assists by clearing fire in the Upper Jiao, thereby stopping thirst. Phellodendri Cortex (Huangbai) directs by draining heat in the Lower Jiao, thereby eliminating dysentery. There are two approaches to treating Jueyin Heat Dysentery: for initial dysentery, this formula is used to dry with bitterness, disperse with pungency, and secure with astringency, which is the method of treating heat with cold; for chronic dysentery, Wumei Wan (Mume Pill) is used, employing sour flavors to gather fire, assisted by bitter-cold herbs, and mixed with warm-tonifying herbs. This represents treating contrary and compliant aspects, adapting to the progression of the disease, and harmonizing Qi to restore balance.
