12. Closure of the vaginal opening (Chanmen) fails due to deficiency of Qi and Blood. In first-time childbirth, injury inevitably causes swelling and pain. For deficiency, treat with Shiquan Dabu Tang (Ten Complete Tonifying Decoction); for swelling and pain caused by injury, wash with thickly decocted Gancao Tang (Licorice Decoction), and the swelling and injury will naturally subside.
[Note] Whenever the Yumen (vulva/vaginal introitus) remains open after childbirth, it is mostly caused by deficiency of Qi and Blood. There are also cases where the injury from first-time childbirth is severe, inevitably resulting in swelling and pain. For those with deficiency of Qi and Blood, use Shiquan Dabu Tang (Ten Complete Tonifying Decoction) for treatment; for those with swelling and pain due to injury, wash with thickly decocted Gancao Tang (Licorice Decoction), and the swelling and injury will naturally subside.
Shiquan Dabu Tang (Ten Complete Tonifying Decoction) Formula is found in the first volume.
Qinghun San
13. Postpartum Xueyun (Blood Fainting/Syncope) with scant Lochia (postpartum discharge) and redness of the face and lips indicates stagnant blood stasis. If Lochia has been excessive and the lips are pale white, it belongs to Blood Collapse, without doubt. For deficiency, use Qinghun San, composed of Jingjie Sui (Schizonepeta Spike), Renshen (Ginseng Root), Chuanxiong (Szechwan Lovage Rhizome), Gancao (Licorice Root), and Zelan Ye (Agastache Leaf). For abdominal pain due to stagnant blood stasis, use Foshou San; for both conditions, it is suitable to frequently burn Ganshi (Dry Lacquer) or quench a hot iron nail in Cu (Vinegar) to fumigate intermittently.
[Note] Postpartum Xueyun (Blood Fainting/Syncope): In cases where Lochia (postpartum discharge) is scant, internal stagnant blood stasis attacks upward causing coma and dizziness, the face and lips must be red. In cases where blood loss is excessive leading to syncope due to Blood Collapse, the face and lips must be pale white. For Blood Deficiency, Qinghun San is appropriate, consisting of Jingjie Sui (Schizonepeta Spike), Renshen (Ginseng Root), Chuanxiong (Szechwan Lovage Rhizome), Gancao (Licorice Root), and Zelan Ye (Agastache Leaf). If there is stagnant blood stasis with abdominal pain, use Foshou San. Both conditions are suitable for frequent fumigation using burned Dry Lacquer (Ganshi) or vinegar-quenched hot iron nails.
Foshou San Formula is found in the first volume.
14. What is the cause of Lochia (postpartum discharge) not descending? It is due to Wind-Cold interfering, Qi stagnation, and Blood coagulation. If it does not descend due to lack of Blood, the complexion will be yellowish-white without distension or pain. For Wind-Cold causing Blood coagulation, use Shixiao San (Laughing Out Powder); for excessive loss requiring tonification and promotion of flow, use Shengyu Tang.
[Note] Postpartum Lochia (postpartum discharge) not descending: In cases where Wind-Cold interferes, causing Qi stagnation and Blood coagulation preventing descent, there must be abdominal distension and pain. In cases where too much blood was lost during delivery, leaving no Blood to descend, the complexion must be yellowish-white, and the abdomen/heart region will not hurt; distinguish based on this. For Blood coagulation, use Shixiao San (Laughing Out Powder) to dispel stasis and promote flow; for lack of Blood, use Shengyu Tang to tonify and promote flow.
Shixiao San (Laughing Out Powder) and Shengyu Tang Formulas are both found in the first volume.
