10. After conception, it is advisable to live separately and rest quietly; one should strictly guard against dietary flavors to ensure the peace of the Spleen and Stomach, regulate emotions to prevent fear and fright, and be cautious in daily life to avoid wind and cold.
[Note] After conception, living separately and resting quietly prevents the disturbance of Ministerial Fire (a type of Yang Qi residing in the Liver and Kidney with warming functions), which might otherwise generate fetal toxins. Strictly guarding against the five flavors (Sour, Bitter, Sweet, Pungent, Salty) in diet harmonizes the Spleen and Stomach, facilitating the generation of maternal Qi and Blood, thereby ensuring the proper formation and development of the fetus. Internally regulating the Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright) and externally avoiding wind and cold, maintaining a smooth daily routine, avoiding heavy labor, excessive sleep, or climbing heights and dangerous places, ensures that the mother remains healthy and the fetus stays secure.
11. There are two methods for stabilizing the fetus: one must carefully distinguish whether the disease originates from the mother or the fetus. If the mother's illness causes fetal movement, treat the mother; if the fetus's condition causes maternal symptoms, examine the cause related to the fetus.
[Note] The approach to stabilizing the fetus involves two methods, requiring careful distinction between maternal diseases and fetal diseases for appropriate treatment. Whenever fetal movement is caused by the mother's illness, treating the mother alone will stabilize the fetus once the mother recovers. Conversely, if maternal illness is triggered by a fetal disorder, stabilizing the fetus alone will lead to the mother's recovery once the fetus is secured.
12. In early pregnancy, clearing heat and nourishing Blood are primary, while regulating the Spleen and soothing Qi are secondary adjuncts. Sweating, purging, and promoting urination are three prohibited therapies; treatment must be tailored according to the syndrome's exterior/interior, deficiency/excess, and cold/heat patterns.
[Note] Zhu Danxi stated that during pregnancy, clearing heat and nourishing Blood should be the main focus to avoid damaging Yin and Blood. Regulating the Spleen strengthens its transportation function, facilitating the generation of Qi and Blood; soothing Qi ensures its smooth flow, harmonizing Qi and Blood. By regulating the Spleen and soothing Qi, combined with clearing heat and nourishing Blood, the fetus becomes stable. The "Three Prohibitions" refer to sweating, purging, and promoting urination, as excessive sweating may deplete Yang Qi and damage Qi, excessive purging may deplete Yin fluids and damage Blood, and promoting urination may injure body fluids. However, treatment must still be administered based on a detailed examination of the syndrome's exterior/interior, deficiency/excess, and cold/heat characteristics, avoiding overly harsh medications.
13. For individuals with a thin physique, overheating herbs should be avoided; for those with a robust physique, tonifying Qi may risk disturbing Phlegm. Huangqin (Scutellariae Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma) are essential herbs for stabilizing the fetus, with other herbs added or subtracted as needed. Increase Huangqin for excess Fire, increase Baizhu for excess Phlegm, use Siwu Tang (Four Substances Decoction) for Blood deficiency, use Sijunzi Tang (Four Gentlemen Decoction) for Qi deficiency. Duzhong (Eucommiae Cortex), Xuduan (Dipsaci Radix), Ejiao (Colla Corii Asini), and Aye (Folium Artemisiae Argyi) are used for unstable fetus; Zisu (Perillae Folium), Dafupi (Arecae Pericarpium), Zhike (Aurantii Fructus Immaturus), Sharen (Amomi Fructus), and Chenpi (Citri Reticulatae Pericarpium) are used for Qi excess causing abdominal distension.
[Note] Thin individuals tend to have internal Heat; using too many warming herbs damages Yin and Blood. Robust individuals tend to have Phlegm; excessive Qi tonification may stagnate Qi and stir Phlegm. Baizhu eliminates Phlegm and strengthens the Spleen; Tiaohuangqin (prepared Scutellariae Radix) clears heat and nourishes Yin; these two are key herbs for stabilizing the fetus. If other syndromes exist, auxiliary herbs are added, adjusting the ratio of Baizhu and Tiaohuangqin as necessary. For excess Fire, increase Huangqin to clear Heat; for excess Phlegm, double Baizhu to resolve Phlegm; for Blood deficiency, combine with Siwu Tang (Four Substances Decoction) to tonify Blood; for Qi deficiency, combine with Sijunzi Tang (Four Gentlemen Decoction) to tonify Qi. For an unstable fetus, add Duzhong, Xuduan, Ejiao, and Aye to stabilize it; for Qi excess causing abdominal fullness, add Zisu, Dafupi, Zhike, Sharen, and Chenpi to soothe Qi.
The formulas for Siwu Tang (Four Substances Decoction) and Sijunzi Tang (Four Gentlemen Decoction) are found in the collected formulas in Volume One.
