[Verse] Leprosy (Mafen) generally results from the formation of toxic pestilential qi. Its etiology involves three aspects: damage to the five zang organs by wind, the appearance of five death signs indicating a true malignant prognosis, and the possibility of survival if treatment is adhered to strictly in the early stage.
[Note] This condition was formerly known as "Li Feng" (Leprosy/Pestilential Wind), which refers to wind pathogenic factors possessing toxicity. The Classics state: "Wind entering the vessels becomes Li." It also states: "In Li, Ying Qi (Nutritive Qi) is hot and corrupt, and its qi is not clear." Consequently, this leads to the collapse of the nasal bridge, deterioration of complexion, and ulceration of the skin. When toxic wind lodges in the vessels and does not depart, it is named Li Feng, now commonly called Leprosy (Da Ma Feng). There are three causes: first, it arises from local climatic and geographical conditions; it is rare in Central China but common in miasma-prone areas; second, it is contagious, occurring through contact with individuals suffering from leprosy, or transmission among parents, spouses, or family members, or due to lack of hygiene outdoors, or uncleanliness in latrines, houses, beds, bedding, or clothing; third, it results from improper self-care, such as bathing while exposed to cool air for temporary comfort, sleeping exposed to wind, or lying on damp ground, allowing toxic wind to invade the blood vessels. Although these causes have different names, they all belong to the pestilential qi of heaven and earth. After infection, one may not notice it immediately; if not vented promptly, it accumulates over time before manifesting. Symptoms include numbness all over the body, followed by the appearance of white scales and red macules, spreading like petechiae, resembling snake skin, and shedding in patches. At onset, progression from top to bottom is considered favorable (Shun), while from bottom to top is unfavorable (Ni); gradual onset is treatable, whereas sudden onset is difficult to cure. When wind-toxin enters the interior and transforms into worms, these worms erode the five zang organs, leading to five types of physical damage: Lung involvement causes eyebrow loss first; Liver involvement causes purple blisters on the face; Kidney involvement causes perforation in the soles of the feet first; Spleen involvement causes the whole body to resemble tinea; Heart involvement damages the eyes first. These are critical conditions. There are also five types of death signs: numbness and insensibility indicate "Skin Death"; inability to feel pain upon cutting indicates "Flesh Death"; ulceration without pus indicates "Blood Death"; detachment of hands and feet indicates "Sinew Death"; collapse of the nasal bridge, rupture of eyelids, eversion of lips, and hoarseness indicate "Bone Death". If any one of the five death signs appears, it signifies a deteriorating, incurable prognosis. Upon initial sensation of the disease, administer Wan Ling Dan (Ten Thousand Spirit Pill) to induce sweating. Subsequently, administer Shen Ying Xiao Feng San (Divine Response Wind-Expelling Powder), Zhui Feng San (Wind-Chasing Powder), and Mo Feng Wan (Wind-Grinding Pills) in sequence. For bleeding gums, rinse with a decoction of equal parts Huang Lian (Coptidis Rhizoma) and Guan Zhong (Cyatheae Spicosa). Externally apply Lei Ju Qu Feng San (Gathered Wind-Expelling Powder), combined with a bath/fumigation using Di Gu Pi (Lycii Cortex), Jing Jie (Schizonepetae Herba), Ku Shen (Sophorae Flavescentis Radix), and Xi Xin (Asari Radix), each two liang, decocted in river water. For cases involving tissue destruction, if located in the upper body, use Zui Xian San (Intoxicating Immortal Powder); if in the lower body, use Tong Tian Zai Zao San (Heaven-Penetrating Rebirth Powder). If the nasal bridge has collapsed, use Huan Ji San (Muscle-Replacing Powder) internally. If the patient shows slight deficiency signs, administer Bu Qi Xie Rong Tang (Tonify Qi and Drain Ying Decoction) and drink He Shou Wu Jiu (Polygoni Multiflori Wine). If one can cultivate mental clarity and reduce desires, observe dietary restrictions, and seek early treatment, there may be hope for recovery. However, if one cannot maintain a bland diet or abstain from sexual activity, recurrence is inevitable even after apparent recovery, ultimately leading to death.
