Raw Rhinoceros Horn (sliced) (Rhinocerotis Cornu), Raw Rehmannia Root (Rehmanniae Radix), White Peony Root (Paeoniae Radix Alba), Moutan Cortex (Moutan Cortex)
For cases of hematemesis and epistaxis caused by mental exertion leading to hyperactive heart fire, this formula is indicated.
The Heart belongs to Fire and governs the vessels. Excessive mental exertion causes the Heart fire to move recklessly, leading to surging blood that overflows from orifices, resulting in the tendency for hematemesis and epistaxis. The Classics state: "In treating disease, one must seek the root." Therefore, herbs that cool the Heart are used as the principal agents. Raw Rhinoceros Horn can resolve Heart heat; Raw Rehmannia Root can cool Blood. White Peony Root and Moutan Cortex are sour and cold substances. Sour flavors enter the Liver, and cold properties overcome heat. The reason why Heart disease is treated via the Liver is that the Liver is the Mother of the Heart; Wood generates Fire. Thus, treating from the Liver constitutes an "intercepting" strategy (Ying Duo Zhi Bing).
Liang Rice (Setaria italica / Foxtail Millet), Rice Oryza sativa, Shu Mi (Panicum miliaceum / Broomcorn Millet) (each 1 sheng), Beeswax (size of a bullet, added later, melted until dissolved)
For cases of hematemesis and epistaxis due to Heart exhaustion, where prolonged use of cold and cooling agents has damaged the Spleen and Stomach, this formula is indicated.
The Heart is the Mother of the Spleen, and the Spleen is the Child of the Heart. If the Spleen is damaged by cold and cooling substances, it will inevitably steal Qi from its Mother (the Heart) to nourish itself, thereby worsening the condition of the Heart. How could such a situation not be perilous? Therefore, it is appropriate to regulate the Spleen and benefit the Stomach. To regulate the Spleen, nothing is better than Grain Qi; thus, Rice, Liang Rice, and Shu Mi are used. Beeswax is additionally employed for its ability to thicken and nourish the intestines and stomach.
Prepared Pinellia Tuber (Pinelliae Rhizoma Preparatum), Aged Ginger (Zingiberis Rhizoma), Poria (Poria), Atractylodes Macrocephala Rhizome (Atractylodis Macrocephalae Rhizoma, stir-fried with soil), Apricot Seed (Armeniacae Semen), Tangerine Peel (Citri Reticulatae Pericarpium, white pith removed)
For cases of Spleen exhaustion manifesting as inability to use the four limbs, dysfunction of the five Zang organs, abdominal distension, shoulder breathing (dyspnea with shrugging shoulders), stiffness at the base of the tongue, and inability to swallow saliva, this formula is indicated.
The Spleen governs the digestion and transformation of food and fluids. After physical fatigue or during recovery from illness, if one consumes palatable food excessively, the weak Spleen cannot digest it properly. The labor of transportation and transformation eventually leads to Spleen exhaustion. Since the Spleen governs the four limbs, this results in the inability to use them. The Five Zang organs all receive Qi from the Spleen; when the Spleen is exhausted and injured, the Five Zang organs lack the source of Qi, leading to dysfunction and loss of normal physiological states. The Classics state: "The Spleen distributes Qi to the Three Yin meridians." When the Spleen is exhausted, the Qi of the Three Yin meridians becomes stagnant and fails to flow, causing abdominal distension. The Qi of the Three Yin meridians ascends to the chest and then returns, causing shoulder breathing. The meridian of the Spleen ascends through the diaphragm, flanks the pharynx, connects to the root of the tongue, and spreads under the tongue, causing stiffness at the base of the tongue and difficulty swallowing saliva. Pinellia Tuber is sweet and acrid; sweetness benefits the Spleen, while acridity disperses stagnation. Aged Ginger assists in awakening the Spleen and counteracting the toxicity of Pinellia. The Spleen prefers dryness and fears dampness; therefore, Atractylodes Macrocephala Rhizome is used to dry the Spleen, and Poria is used to leach out dampness. The Spleen prefers free flow and dislikes obstruction; therefore, Apricot Seed is used to promote Qi movement, and Tangerine Peel is used to vent Qi. Bamboo Leaf has a pure aroma and can clear fire from the Earth element. Peony Root is sour and can drain Wood from the Earth element. Jujube Fruit is used for its sweetness to benefit the Spleen.
