Colloquially known as *xin cao* (Heart Clamor). The symptoms resemble hunger, with an urgent desire to eat, a restless and unsettled feeling in the heart, sensations of sourness or spiciness, and a sense of panic. This arises from Liver Fire invading the Spleen and Stomach; due to Spleen Earth deficiency failing to withstand Liver Wood's restraint, one feels disturbed and uneasy. When Fire is excessive, grains digest easily; after eating, hunger returns quickly. Eating provides temporary relief, but soon hunger and clamor recur. This is classified as Fire Clamor (*Huo Cao*), which should be treated by clearing Fire. If Phlegm-Fluids are stagnant and accumulated, there is a sensation resembling hunger but not true hunger, an inability to eat much despite wanting to, and a slippery pulse; this is Phlegm Clamor (*Tan Cao*), which should be treated by resolving Phlegm. If accompanied by belching or vomiting acid water, it is caused by both Phlegm-Fluids and Fire, requiring simultaneous treatment to clear Fire and eliminate Fluids. The general principle is to tonify Earth (Spleen/Stomach), primarily using Liu Jun Zi Tang (Six Gentlemen Decoction) with modifications. For chronic cases that do not heal, nourish Blood, as excessive Fire depletes Blood. Therefore, for those with Blood deficiency due to overthinking who experience clamor at dawn, and for women suffering from clamor, Si Wu Tang (Four Substance Decoction) is frequently used with modifications.
Sleep belongs to Yin and stillness. Yin deficiency with Fire causes agitation, leading to restlessness and insomnia. The *Classic* states: "Wei Qi circulates in the Yang meridians during the day, resulting in wakefulness; it circulates in the Yin meridians at night, resulting in sleep." If rebellious Qi (i.e., counterflow Qi. All upward counterflow symptoms belong to Fire, specifically Yin Fire) lodges in the Zang-Fu organs, Wei Qi cannot enter Yin, causing the eyes to remain open. It also states: "If the Stomach is disharmonious (whether due to Heat or Phlegm), sleep will be unrestful." Both passages convey this meaning. For insomnia due to deficiency exhaustion and empty heat restlessness, use Suan Zao Ren Tang. For bone-steaming fever, restlessness, and insomnia, take 1 liang of Suan Zao Ren (Ziziphi Spinosae Semen), add 1.5 large bowls of water, grind and extract the juice, add 2 he of rice, and cook into porridge. Once cooked, add 1 he of Di Huang Zhi (Rehmanniae Radix Juice), boil again, and consume at any time. (Above: Method for nourishing Blood.) Wen Dan Tang (Warm the Gallbladder Decoction) treats insomnia with empty heat restlessness following severe illness. Take 8 sheng of flowing water from a thousand miles away, splash it ten thousand times, collect five sheng of the clarified water, and boil using reed fire. When boiling, add 1 sheng of Shu Mi (Panicomi Fructus, known in the North as yellow millet, used for brewing wine) and 5 he of Zhi Ban Xia (Pinelliae Rhizoma Preparatum). Steam until cooked, strain, and drink one small cup three times a day. (Above: Method for eliminating Phlegm.) Insomnia has two types: one is empty heat restlessness after severe illness, and insomnia in the elderly due to Yin deficiency and isolated Yang; the other is insomnia caused by Phlegm lodging in the Heart Meridian, preventing the Shen (Spirit) from returning to its residence. For deficiency patterns, use Liu Jun Zi Tang (Six Gentlemen Decoction) plus 1 qian of Chao Suan Zao Ren (Fried Ziziphi Spinosae Semen) and 1 qian of Zhi Huang Qi (Prepared Astragali Radix). For Phlegm patterns, use Wen Dan Tang (Warm the Gallbladder Decoction) with half the amount of Zhu Ru (Bambusae Caulis in Taenia) reduced, plus 1 qian of Nan Xing (Arisaematis Cum Bile Rhizoma) and 1 qian of Suan Zao Ren (Ziziphi Spinosae Semen), taken with Qing Ling Dan. Alternatively, use Dao Tan Tang (Guide Out Phlegm Decoction) plus 0.5 fen of Shi Chang Pu (Acori Tatarinowii Rhizoma). For dyspnea preventing lying down, treat the dyspnea. For Jue (Cold/Heat extremity reversal) preventing lying down, treat the Jue. For coughing preventing lying down, if on the left it belongs to Liver distension, clear the Liver; if on the right it belongs to Lung distension, clear the Lung.
