[Background Information]
Book: Yi Shu (Records of Medicine)
Author: Cheng Xingxuan
Dynasty: Yuan (Note: Historically Qing, but transcribed as per original text)
Chapter: Neijing Fanglun (Commentary on Formulas from the Inner Canon)
Category: Medical Discourse
The "Nei" refers to the fundamental principles of life and destiny; the "Jing" is the book that records these principles. This work was compiled through extensive discussion among the Yellow Emperor and his six ministers: Qibo, Guiyuzhou, Bogao, Shaoshi, Shaoyu, and Leigong. The formulas contained therein are lofty, concise, profound, and subtle, making them difficult to comprehend and master. Few people today can effectively utilize them. However, since they are the divine prescriptions of ancient sages, it is imperative to record them at the beginning of this volume. (From *Chengfang Qieyong*)
**Zeshu Mixian San (Alisma, Atractylodes, and Daphne Decoction Powder)**
There is a condition characterized by fever, physical exhaustion and flaccidity, profuse sweating resembling a bath, aversion to wind, and shortness of breath. This disease is named "Jiu Feng" (Wine Wind). It is treated with Zexie (Alismatis Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma), each in ten parts, and Mixian (Daphnes Genkwa Cortex) in five parts. These are mixed and taken as a powder, measured by three fingers ("San Zhi Cuo"), administered after meals. (From *Suwen*)
"Jiu Feng" refers to what is described in the *Feng Lun* (Discourse on Wind): contracting wind after drinking alcohol results in "Lou Feng" (Leaking Wind). The injury lies in the Spleen and Stomach. Symptoms include fever, lethargy, profuse sweating, aversion to wind, and qi deficiency. Internally, the damage is caused by dampness from alcohol, causing pathogens to linger in the muscles; externally, wind pathogens injure the Defensive Qi (Wei Qi), leading to the dissipation of Yang Qi, laxity of the interstices (Cou Li), opening of the Yuan Gates (sweat pores), and flaccidity of the sinews. This explains the origin of the syndrome. Mixian expels wind from the exterior; Zexie leaches out dampness from the interior; Baizhu assists Spleen and Stomach Qi to expel pathogens. "San Zhi Cuo" indicates the preparation as a powder, intended for slow action in the Middle Jiao to resolve the disease. "Hou Fan" (after meal) corresponds to Zhang Zhongjing's instruction: after taking medicine, drink hot thin porridge to assist the medicinal power. (From *Gufang Xuanzhu*)
**Lancao Tang (Orchid Herb Decoction)**
There is a condition where the mouth tastes sweet, named "Pi Dan" (Spleen Heat/Excess). This arises from excessive consumption of rich and fatty foods. Fatty foods generate internal heat; sweet foods cause abdominal fullness. The qi overflows upward, transmitting into Xiao Ke (Wasting and Thirsting Syndrome). It is treated with Lancao (Eupatorii Herba) to eliminate stale turbid qi. (From *Suwen*)
Shentou Cao (Peilan, Eupatorium fortunei), with fragrant flowers and leaves, maintains its drying-dampness efficacy. "Pi Dan" is an excess syndrome, typically occurring in individuals who frequently consume sweet, delicious, and fatty foods. Only such clearing vegetables can remove the long-standing accumulated heat in the Spleen meridian. Its flavor is pungent and nature cold, primarily opening knots, benefiting orifices, resolving heat, and quenching thirst. Therefore, used singly, it can thoroughly cleanse the qi of undigested rich and sweet foods, revealing its potent and penetrating nature. If the condition is not caused by rich food-induced heat, this formula is not suitable. (From *Gufang Xuanzhu*)
**Jishi Li (Chicken Dropping Sweet Wine)**
There is a condition with fullness in the heart and abdomen, where one can eat in the morning but cannot eat in the evening, named "Gu Zhang" (Drum Distension/Tympanites). How should it be treated? Treat with Jishi Li. One dose brings improvement; two doses bring recovery. (From *Suwen*)
"Li" refers to sweet wine, made with a small amount of yeast starter and large amounts of rice, brewed overnight for best results. Chickens lack a separate anterior yin orifice; urine and feces share the same passage. Combined with Taoren (Persicae Semen) and Dahuang (Rhei Radix et Rhizoma) to mildly promote bowel movement, water-dampness exits via the stool. Clear wine represents the qi of cooked grains, reaching the skin and hair, circulating in the collaterals, and descending to unblock the water pathways. Jishi (Gallus Gallus Domesticus Brisson Faeces) combined with wine enables water-dampness to exit via urination. With both bowels and bladder unblocked, abdominal distension gradually dissipates. (From *Gufang Xuanzhu*)
**Si Wuie Gu Yi Ru Wan (Four Parts Cuttlebone and One Part Rubia Pill)**
There is a condition with support-like fullness in the chest and hypochondrium, interfering with eating. Upon onset, the patient first smells a fishy odor, secretes clear fluid, expectorates blood, experiences cold limbs, dizziness, and frequent bleeding from the anterior and posterior orifices. This disease is named "Xue Ku" (Blood Depletion). It originates from significant blood loss in youth, or sexual activity while intoxicated, leading to exhaustion of Middle Qi and Liver injury. Liver injury causes menstrual scarcity or amenorrhea. Treatment uses four parts Wuwie Gu (Sepiae Endoconcha) and one part Ru (Rubiae Radix et Rhizoma), formed into pills using Quelian (Quail Egg), sized like small beans. Take five pills after meals, washed down with Baoyu Zhi (Abalone Juice) to benefit the intestines. (From *Suwen*)
Wuweigu Wan consists of "flesh-and-blood" substances. Since blood is depleted and qi is lost, only living substances can retain and nourish blood and qi. Wuwie Gu is salty and warm, descending in nature, astringent to stop leakage; long-term use promotes fertility, demonstrating its ability to consolidate qi and benefit essence. Ru is salty and sour, entering the Liver, activating blood, unblocking meridians, regulating qi, and repairing injury. Quilian strengthens Yang and benefits blood. Taking medicine followed by food, then Baoyu Zhi, suppresses the downward action of the herbs, benefits the intestines, and reconnects severed vessels. Taking five pills aligns with the Classic's statement that blood loss and sexual excess injure the Liver due to Middle Qi exhaustion; thus, the medicine is intended to stay in the Middle Jiao and be transported downward via the Spleen and Stomach. (From *Gufang Xuanzhu*)
**Shengtie Luo Yin (Raw Iron Drop Drink)**
There is a condition of anger and mania, arising from Yang. Yang Qi suffers sudden obstruction and difficulty in resolution. This disease is named "Yang Jue" (Yang Reversal); depriving the patient of food cures it. Food enters the Yin and generates Yang Qi; therefore, administering Shengtie Luo Yin works because it rapidly descends qi. (From *Suwen*)
For Jue Yang causing anger and mania, treatment does not rely on herbal medicines, as the Classic states: mineral drugs may induce epilepsy, and aromatic herbs may induce mania. Thus, Shengtie Luo (Raw Iron Drops) is ground in water and drunk. Raw iron has a cold qi and pungent taste, with a direct descending nature, rapidly descending qi. Using the hammered fragments covers the outward path of the collaterals, resolving knots within Wood and Fire, thereby stopping mania and anger. (From *Gufang Xuanzhu*)
**Yiyao Fang (Moxibustion/Warming Compress Formula)**
The Yellow Emperor asked: How to introduce heat when treating Cold Bi with acupuncture? Bogao answered: For commoners, use fire-quenching (moxibustion). For nobles, use warming compresses. Use twenty jin of pure wine, one sheng of Shujiao (Sichuan Pepper), one jin of Jiangjiang (Dried Ginger), and one jin of Guixin (Cinnamon Twig Heart). All four ingredients are chopped and soaked in wine. Add one jin of cotton wadding and forty zhang of fine white cloth, immersing them together. Place in a pit burning dry horse dung, seal and plaster tightly to prevent leakage. After five days and nights, remove the cloth and cotton, sun-dry the cotton, soak again until all juice is extracted. Each soaking must last seven days. Remove and mix the dregs with cotton, wrapping in cloth to make strips six to seven chi long, creating six or seven compresses. Heat the compresses with raw mulberry charcoal and apply to the acupoints treated for Cold Bi, ensuring heat penetrates to the lesion site. When cool, reheat and apply. Repeat thirty times. Wipe sweat with the cloth, also thirty times. Walk indoors immediately after, avoiding wind. Apply compress after every acupuncture session. Thus, the disease will heal. (From *Lingxu*)
"Ju" means chopping ingredients to the size of soybeans before decocting to extract clear liquid. "Zi" means soaking. "Ma Shi Wei Zhong" means simmering over burning dry horse dung. "Tu" means sealing with salt mud. "Xun" means a week. "Fu Bu Wei Jin" refers to modern pouches used to hold cotton and herb dregs. "Zi" refers to dregs. Heating cloth with raw mulberry charcoal utilizes Mulberry's ability to benefit joints and expel wind-cold-damp Bi. Nobles have clear and smooth blood and qi; therefore, before and after acupuncture, warming compresses alone can unblock meridians, induce sweating, and eliminate Cold Bi. Walking in a sealed room post-acupuncture promotes qi and blood circulation while avoiding wind-cold. (From *Chengfang Qieyong*)
**Shi Gao (Pork Fat Ointment)**
An abscess developing in the throat is named "Meng Ju" (Fierce Jar). If untreated, Meng Ju turns into pus. If pus is not drained, blocking the throat leads to death within half a day. For those who have turned to pus, draining it and applying Shi Gao (Porci Adeps) while cold, eaten cold, cures it in three days. (From *Lingxu*)
"Meng Ju" describes the urgency of the harm. "Shi Gao" refers to refined pig fat. (From *Chengfang Qieyong*)
**Qiaocao Gen Tang (Forsythia Root Decoction)**
An abscess developing in the hypochondrium is named "Bai Ci". "Bai Ci" is a disease affecting women. Treat with moxibustion. If it becomes a large abscess with pus, treat it. Inside, there may be new flesh growth, sized like a red adzuki bean. Chop Conggen (Scallion Root) and Qiaocao Gen (Forsythiae Caulis/Radix) each one sheng. Boil in one dou six sheng of water until reduced to three sheng. Force the patient to drink it thickly, wrap warmly, sit over a pot to steam, inducing sweating down to the feet. The disease resolves. (From *Lingxu*)
"Ji" refers to Water Caltrop. "Qiao" refers to Forsythia. The roots of both herbs detoxify. Ma Yuantai stated: "Ji" is now known as Forsythia. (From *Chengfang Qieyong*)
**Jiafa Jiu (Hair Wine)**
Pathogenic factors invade the collaterals of Hand-Shaoyin, Foot-Shaoyin, Hand-Taiyin, Foot-Taiyin, and Foot-Yangming. These five collaterals converge in the ear and ascend to connect with the left corner of the head. If all five collaterals are exhausted, the body's pulses move, but the form remains unconscious. The state resembles a corpse, named "Shi Jue" (Corpse Reversal). Shave one square cun of hair from the left corner, burn it, and administer with a cup of fine wine. If unable to drink, force-feed; recovery is immediate. (From *Lingxu*)
When pathogenic factors invade the collaterals of the Four Zang and One Fu, causing Shi Jue, it is because Heart and Kidney are the Water-Fire collaterals; Lung and Spleen are Heaven-Earth collaterals; Stomach is the Central Earth collateral. These five collaterals converge in the ear and ascend to the left head corner. If Yin and Yang separate and fail to intersect, pulses move while consciousness is lost, resembling a corpse. Acupuncture the Jing-Well points of the five collaterals. If ineffective, first blow air into the ears with a bamboo tube to open the convergence of the five collaterals. Then shave the left corner hair, burn it into Xuè Yú Tàn (Burnt Hair Ash), enter the collaterals to transform stasis, and drink fine wine to allow Collateral Qi to communicate with Wei Qi, harmonizing Yang and reversing Jue. (From *Gufang Xuanzhu*)
**Banxia Shumi Tang (Pinellia and Millet Decoction)**
Jue Qi lodges in the Five Zang and Six Fu, causing Wei Qi to guard only the exterior, circulating in Yang without entering Yin. Circulating in Yang makes Yang Qi abundant; abundant Yang Qi causes the Yang Qiao vessel to sink/stagnate. Inability to enter Yin leads to Yin deficiency, resulting in insomnia (eyes unable to close). Tonify deficiency, sedate excess, regulate emptiness and fullness, unblock the pathway, and expel pathogens. Administer Banxia Tang; once Yin and Yang are regulated, sleep returns immediately. (From *Lingxu*)
Ancient treatments for the Extraordinary Vessels relied solely on acupuncture, lacking herbal formulas. Now, with Jue Qi lodging in the Zang-Fu, Wei Qi circulates only in Yang, Yang Qiao Qi is abundant, and it cannot enter Yin, causing Yin deficiency and insomnia. Banxia Shumi Tang is used because herbs cannot directly enter the Yang Qiao vessel; thus, treating the Stomach drains Wei Qi. Banxia is pungent and warm, entering the Stomach Qi level; Shumi (Glutinous Millet) is sweet and sour, entering the Stomach Blood level. Water from a thousand miles stirred ten thousand times is light and ascending, not aiding Yin pathogens. Cooking with reed fuel involves strong fire initially, then gentle simmering to reduce, embodying ascending-descending methods. Ascending via Banxia opens Yang to vent Wei and expel pathogens; descending via Shumi enters Yin to nourish Ying and tonify deficiency. With Yin and Yang connected, sleep returns immediately. **Magao Shensang Guijiu (Horse Fat, Raw Mulberry, and Cinnamon Wine)**
Spring Bi (spastic paralysis) is treated with Magao (Horse Fat). For severe spasticity, apply Baijiu (White Spirit) mixed with Guixin (Cinnamon Twig) to the affected area; for flaccidity, use a Mulberry Hook to stretch it. Place raw mulberry ash in a pit, adjusted to sitting height. Apply fat to the spasmodic cheek, drink fine wine, eat roasted meat. Non-drinkers should still eat meat. Pat lightly three times. (From *Lingxu*)
"Jichun Bi" refers to true wind-stroke in northern regions. Pathogens strike the Yangming collaterals, causing sudden facial deviation. Spasticity prevents eye closure; heat causes sinus flaccidity and inability to open eyes. Due to the dry, windy climate of the north, pungent dispersants are insufficient. External application of warming and moistening methods is required. Magao is sweet, pungent, soft, and slow, massaging spasticity, moistening blood vessels, and unblocking Bi. Flaccid sinews cannot contract, so Guixin's pungent heat and wine's meridian-unblocking property are applied to the flaccid side to harmonize Ying and Wei and unblock blood collaterals. The Mulberry Hook stretches the cheek. "Kan" refers to the depression in the cheek. Raw mulberry ash comes from live branches, possessing sharp potency to open joints and expel wind-Bi. Height adjusted to sitting ensures even distribution of ash for effective heat penetration. Magao soothes sinews and moistens Bi. "San Fu" means light patting three times. Fine wine promotes the action of herbs for upper-body diseases. Roasted meat aids Stomach Qi to ascend to the collaterals. (From *Gufang Xuanzhu*)
**Xiao Jindan (Little Golden Pill)**
Chensha (Cinnabar) two liang, Xionghuang (Realgar) one liang, Cihuang (Orpiment) one liang, Zijin (Purple Gold) half liang. Mix in a crucible. Seal externally. Bury one chi deep in the earth, compacting the soil. Do not use a furnace or other processing agents. Fire for seven days. Wait for cooling, remove the crucible, bury the medicine in the earth for seven more days. Remove, grind clockwise for three days. Form pills with honey, sized like wu zi (bayberries). Every morning, face east, inhale one breath of solar energy. Swallow one pill with ice water, harmonizing qi as you swallow. Ten pills prevent epidemic interference. (From *Suwen*)
Grinding with gold foil facilitates fine powder. Grinding clockwise refers to rotating left. (Commentary by Xu Lingtai)
