因热毒蕴蓄肠胃，非真土败之比，尽人心力，可冀斡旋。

患蜂起。病家不知，医者亦不经意，究竟是谁之过与？
【案例一】许姓患者，五十八岁。身体发热，排出红色痢疾，整整一个月没有饥饿感且无法进食，脉象带有轻微的弦象。此证属于难治之症，但庆幸神气尚未丧失，暂且投予一方，以观察病情变化。

服用后痊愈。
【案例二】蔡氏，五十九岁。血痢两个月，医治无方。近日连粥饮都无法进服，胸闷干呕，腹痛不止，里急后重，昼夜排便六七十次，形体精神疲惫困倦，脉象细数而沉，噤口痢重症显然。幸好脉象不至于弦劲，虽然形势危险，总因热毒蕴蓄于肠胃，并非真元败绝之比，竭尽人力，可希望扭转局面。
黄连（一钱） 黄芩（一钱五分） 白芍（一钱五分。以上三味，立斋先生认为是治疗热痢的主药） 山楂（三钱） 厚朴（一钱） 陈皮（一钱） 木香（磨汁冲服少许） 扁豆花（二十朵）
服用两剂后，干呕停止，痢疾变为白色，但腹痛仍然，排便次数不能大幅减少。肛门下坠，前方仅能清热调气，对痢疾毫无减少。
因而想起当下天气收敛肃杀，排出秽物时转侧，难道没有感冒吗？肺与大肠相为表里，如今脏腑之气皆郁结而不能舒展，治病必当求本。

服用一剂后，疼痛和痢疾减半，再服一剂，病如消失。饮食逐渐增加，胸膈尚不甚舒畅，是因为肝木动故也。况且年届六十（甲周），大病初愈，中焦脾胃怎能骤然强健？缓缓调理以求平复。

服用两剂后，痢疾减少十分之七，身体发热和腹痛全部消除。只有小便微涩，黄昏时有微热，阴分受伤。

服用三剂后，痢疾减少大半，谷物食物顿增，如此重症，得如此速效，相信用药有缘。现在只需扶持脾胃，不治而可愈矣。异功散加熟地、当归、白芍、山药、山楂肉、砂仁壳，不到十剂而痊愈。
【案例八】周老妇人，六十六岁。红痢三个月不愈，宜逐邪兼顾本原。

[Case 1] Patient Xu, aged 58. The body is feverish, passing red dysentery (dysentery with blood), for a full month without hunger and unable to take food. The pulse carries a slight wiry quality. This syndrome belongs to difficult-to-treat conditions, yet fortunately the Shen Qi (Spiritual Energy) has not been lost. Tentatively administer a formula to observe changes.
Coptidis Rhizoma (Chuanlian, 8 fen, clears summer-heat toxicity); Nelumbinis Semen (Lianrou, 3 qian, removes heart, tonifies spleen with astringency); Paeoniae Radix Alba (Baishao, 1.5 qian, clears blood); Citri Reticulatae Pericarpium (Jupi, 8 fen, promotes qi movement); Magnoliae Officinalis Cortex (Noupu, 6 fen, stir-fried, disperses stagnation); Crataegi Fructus (Zharou, 2 qian, eliminates accumulation); Lonicerae Japonicae Flos (Yinhua, 1.5 qian, resolves toxicity and stops dysentery); Glycyrrhizae Radix et Rhizoma (Gancao, 3 fen, harmonizes middle jiao); Brassicae Pekinensis Caulis (Caihuatou, 5 pieces, this ingredient obtains the harmonious qi of spring, is warm and ascending. It generates all things, used to raise spleen and stomach qi).
After two doses, fever subsides and appetite returns; dysentery reduces by half and turns white. The brilliance of this case lies in the patient not taking medicine lightly, having never taken any prior prescriptions, thus the efficacy of this formula is particularly rapid. The path to recovery has been cleared, and cure can be expected.

[Case 2] Ms. Cai, aged 59. Bloody dysentery for two months, medical treatment ineffective. Recently, even porridge and drinks cannot be taken. Chest tightness, dry retching, unceasing abdominal pain, tenesmus (liji houzhong), defecating sixty to seventy times day and night. Physical form and spirit are exhausted; pulse is fine, rapid, and deep. The severe condition of Jin Kou Li (Dysentery with inability to eat) is evident. Fortunately, the pulse is not wiry and forceful. Although dangerous, it is due to heat toxin accumulating in the intestines and stomach, not comparable to true earth failure. Exerting human effort, reversal can be hoped for.

These three ingredients, Mr. Lizai considered as main drugs for treating heat dysentery); Crataegi Fructus (Shanzha, 3 qian); Magnoliae Officinalis Cortex (Noupu, 1 qian); Citri Reticulatae Pericarpium (Jupi, 1 qian); Aucklandiae Radix (Muxiang, ground and mixed in small amount); Lablab Flower (Biandouhua, 20 flowers).
After two doses, dry retching stops, dysentery turns white, but abdominal pain remains, and frequency does not significantly decrease. Anal prolapse. The previous formula only cleared heat and regulated qi, reducing dysentery by not a bit.
Recalling that the current weather is contracting and肃 (solemn/clear), when expelling filth and turning over, could there not be an external contraction? Lung and Large Intestine are exterior-interior partners. Now, the qi of zang-fu organs is depressed and cannot extend. Treating disease must seek the root.

One dose reduces pain and dysentery by half; two doses make the illness disappear. Diet gradually increases; chest and diaphragm are not very comfortable, due to Liver Wood moving. Moreover, at age sixty (Jia Zhou), newly recovered from major illness, how can the Middle Jiao suddenly become strong? Slowly regulate to restore balance.

[Case 3] Patient Xu, aged 47. Bloody dysentery for half a year, recently added aversion to wind, night sweats with fever. This indicates Yin division injury, combined with external contraction.

[Case 4] Patient Wu, aged 50. Dysentery with red and white discharge, anus sinking, pulse is soggy and soft. This belongs to damp-heat. Use Dong Yuan's lifting method.

[Case 5] Patient Bai, aged 37. Initial onset of red dysentery, thick and greasy, difficult to pass, abdominal pain with diarrhea, body feverish and aversed to cold, pulse fine. This is heat郁 (depressed) in zang-fu, cold binding the muscle surface. Treatment should attend to both.

[Case 7] Patient Li, aged 44. Dysentery with five colors, abdominal pain with heavy bearing-down sensation, unlimited frequency, minimal food intake, lasting fifty days. The aforementioned symptoms are also serious, and the pulse is wiry and large, which is not suitable for dysentery. Fortunately, Shen Qi is not exhausted, belonging to Yang syndrome. Use methods to purge heat, guide out pathogens, and lift. If effective, maintenance can be discussed.

Three doses, dysentery still not reduced, but abdominal pain and tenesmus are gone. Pulse wiry slightly relaxed, tongue coating dry yellow, slight thirst. Thinking that previously using Huang Qin and Chuan Lian to purge heat resulted in yellow coating. Fifty days of chronic dysentery, Yin blood greatly deficient. Suddenly use Jiawei Siwu Tang (Modified Four Substances Decoction).

Three doses, dysentery reduces by more than half, grain food suddenly increases. Such severe illness achieving such rapid effect, believing in the affinity of medication. Now only support Spleen and Stomach, can heal without specific treatment. Yigong San (Five-Ingredient Powder with Astragalus) plus Shu Di Huang, Dang Gui, Bai Shao, Shanyao, Zharou, Sharen Qiao. Cured in less than ten doses.
[Case 8] Elderly Woman Zhou, aged 66. Red dysentery for three months not cured. Should purge pathogens while attending to the root.
