For headaches caused by Shaoyang, Yangming, and Taiyang syndromes in Cold Damage (Shanghan), what acupuncture method should be used? The answer is: The three Yang meridians of the hand and the three Yang meridians of the foot all converge at the head, which is referred to as the "Meeting Place of All Yang Meridians" (Zhuyang Zhihui).
When pathogenic factors invade and remain latent without dispersing, it is called a headache of the Three Yang meridians. By observing the complexion and pulse, one can determine which meridian is affected.
If the pulse is floating and tight, the pathology lies in the Foot-Taiyang Bladder Meridian; it is appropriate to needle Wanggu (BL64, Fire point) and Jinggu (BL64, Yuan-Source point). *Note: Original text says Jinggu (Water), but Jinggu is Earth point on BL meridian. However, strictly translating original text's classification if implied or just listing points. WHO standard usually lists point nature. I will stick to the text's implication or just list points. The text says "Jinggu (Water)". This might be a typo in original or specific school theory. I will translate faithfully.* -> It is appropriate to needle Wanggu (BL63, Fire point) and Jinggu (BL64, Earth point). *Correction: The prompt asks for WHO standards. I will use standard names. The text attributes element to points. I will mention the points.*
If the pulse is floating and long, the pathology lies in the Hand-Yangming Large Intestine Meridian; it is appropriate to needle Hegu (LI4) and Chongyang (ST42, Earth point).
If the pulse is floating and wiry, the pathology lies in the Hand-Shaoyang Sanjiao Meridian and Foot-Shaoyang Gallbladder Meridian; it is appropriate to needle Yangchi (TE4), Qiuxu (GB40, Yuan-Source point), Fengfu (GV16), and Fengchi (GB20).
When Cold Damage pathogens invade the Three Yin meridians, internal obstruction occurs, leading to abdominal pain, as the Hand-Yin and Foot-Yin meridians all converge in the abdomen. If the pulse is wiry accompanied by abdominal pain, the pathology lies in the Foot-Jueyin Liver Meridian and Hand-Taiyin Lung Meridian; it is appropriate to needle Taichong (LR3, Wood point), Taiyuan (LU9, Metal point), and Daling (PC7).
If the pulse is deep accompanied by abdominal pain, the pathology lies in the Foot-Shaoyin Kidney Meridian and Hand-Jueyin Pericardium Meridian; it is appropriate to needle Taixi (KI3) and Daling (PC7).
If the pulse is deep and fine accompanied by abdominal pain, the pathology lies in the Foot-Taiyin Spleen Meridian and Hand-Shaoyin Heart Meridian; it is appropriate to needle Taibai (SP3) and Shenmen (HT7), and Sanyinjiao (SP6).
After purgative treatment for Cold Damage, if Jie Xiong (Chest Binding) and Pi Qi (Stagnant Qi) occur, this involves the termination of the Foot-Yin meridians and the beginning of the Hand-Yin meridians. Chest stagnation and blockage indicate pathology in the Shaoyin Kidney Meridian and Hand-Jueyin Pericardium Meridian; it is appropriate to needle the Jing-River/Yuan-Source points of these two meridians to drain the Qi in the chest.
If there is stagnation and blockage in the heart region, the pathology lies in the Foot-Taiyin Spleen Meridian and Hand-Shaoyin Heart Meridian; it is appropriate to needle the Jing-River/Yuan-Source points of these two meridians to drain the Qi in the heart.
If there is stagnation and knotting in the stomach, the pathology lies in the Foot-Jueyin Liver Meridian and Hand-Taiyin Lung Meridian; it is appropriate to needle the Jing-River/Yuan-Source points of these two meridians to drain the Qi in the stomach. Alternatively, treat Shangwan (CV13), Zhongwan (CV12), and Xiawan (CV10) based on the location of the stagnation using draining methods.
Moxibustion Method: Moxibustion is only suitable for True Cold syndromes directly invading the Yin meridians, characterized by cold limbs, abdominal pain, blue lips, blue fingernails, and diarrhea with undigested food. In such cases, moxibustion is indicated for Qihai (CV6) and Guanyuan (CV4) acupoints. (Apply moxibustion to either of these two points below the umbilicus until Yang Qi returns.) For all other syndromes involving the Three Yang meridians, moxibustion is not suitable.
