Wang Shi stated: When heat resides in the chest, causing the infant to extend and contract while gasping for breath, it may also cause umbilical swelling. The Qianjin Longdan Decoction (Thousand-Gold Gentian Decoction) is indicated for treatment.
Controlling Phlegm-Fluids: To treat Umbilical Wind (Neonatal Tetanus), one should first use this formula to induce vomiting of wind-phlegm, then use Yipi San (Splee-Strengthening Powder) to harmonize the spleen and stomach, and finally use Chensha Gao (Cinnabar Paste) to calm fright; the patient will recover. If there are spasms and curling of the hands and feet, or lockjaw with inability to open the mouth, it is incurable.
Scorpion Tail (5 fen each) Malachite Green (5 fen each) Cinnabar (1 qian) Nifen (1 zi) Musk (a small amount)
Grind the above into powder. Take 1 zi per dose, mixed with clear winter tea decoction to first vomit out wind-phlegm, then harmonize the stomach. Formulas containing Licorice Root may also be used to induce vomiting of phlegm, selected according to the severity of the condition.
Xuanfeng San (Wind-Dispersing Powder): Treats newborn infants who, after umbilical cord separation, suffer from external wind-damp invasion, presenting with cyanotic lips, refusal to suckle, severe panting, and puckering of the mouth.
Full Scorpion (21 pieces, head and feet intact, detoxified, roasted with non-ash wine, and ground into powder) Musk (1 zi, separately ground)
Grind into an extremely fine powder. Take half a zi, dissolved in a decoction of gold and silver, or in Ophiopogonis Decoction (Mai Dong Tang). Administer long after meals.
Fangfeng San (Saposhnikovia Powder): Treats pediatric umbilical wind.
Saposhnikovia Root (Fangfeng) Notopterygium Rhizome (Qianghuo) Angelica Dahurica Root (Baizhi) Chinese Angelica Root (Danggui) Astragalus Root (Huangqi) Licorice Root (Gancao) (5 fen each)
Grind into a fine powder. Take a small amount, mixed with Juncus Decoction (Deng Xin Tang) for oral administration.
The Shenghui Fang discusses that pediatric puckering of the mouth (Cuokou) is caused by exposure to wind-heat in the womb. After birth, when the qi and blood are not yet regulated, bathing while exposed to wind can trigger it. If this disease presents with foaming at the mouth and cold limbs, medication will likely be ineffective. When pediatric wind diseases progress to malignant signs, leading to puckering of the mouth, the condition becomes critical. All forms—umbilical wind puckering, fetal wind puckering, anal-lock puckering, fishing-intestine puckering, and hernia puckering—involve wind pathogen infiltration. When wind enters the Heart and Spleen, it causes puckering of the mouth. The pathology of puckering of the mouth arises from stagnation and constraint in the middle jiao, affecting the intestines and stomach, causing blockage and failure of qi transformation. This leads to abdominal distension, visible blue veins under the abdomen, puckering of the mouth, and refusal to suckle. The syndrome is extremely urgent. Without rapid purgative treatment to unblock and drain, there is no hope of rescue. If puckering of the mouth is treated as a trivial matter and delayed, death will ensue.
