A patient over thirty years of age suffered from an abdominal carbuncle (flesh tumor), with a rapid pulse and a preference for cold drinks. Qi stated: When sores and ulcers swell and rise with hardness, it is an Excess condition. He Jian (Liu Wansu) stated: Swelling and hardness accompanied by mental confusion, oppression, irritability, and a desire to drink cold indicates that the pathogenic factor is inside. Qingliang Yin (Cooling Drink) was used with doubled Dahuang (Rhubarb Rhizome), and after three doses, symptoms slightly alleviated; subsequently, Siwu Tang (Four Substances Decoction) added Huangqin (Scutellaria Root), Huanglian (Coptis Rhizome), Zhizi (Gardenia Fruit), and Mutong (Akebia Stem) was administered for four doses until suppuration occurred; further, Shixuan San (Ten Acupoints Powder) was used, removing Renshen (Ginseng Root), Baizhu (Atractylodes Rhizome), and Guizhi (Cinnamon Twig), and adding Jinyinhua (Honeysuckle Flower) and Tianhuafen (Trichosanthes Root). The patient, desiring rapid efficacy, self-administered warming and tonifying medicines, causing the abdomen to swell further and urination to become difficult. Qingliang Yin was used again, resulting in the discharge of several bowls of pus, and the patient recovered after treatment with Tuoli Yao (Supporting Interior Medicines).
Another patient suffered from abdominal carbuncle pain, irritability, vomiting, and a full pulse. He Jian stated: Sores and ulcers belong to Fire, and one must distinguish between interior and exterior to treat the root cause. He also said: Vomiting, hiccups, heart vexation, swelling and hardness with mental confusion, or unchanged skin and flesh with a deep and full pulse indicate that toxicity is inside. One should unblock the interior to cut off the source. Neishu Huanglian Tang (Interior-Draining Coptis Decoction) was used; after two or three bowel movements, all symptoms disappeared, and the patient recovered after treatment with Lianqiao Xiaodu San (Forsythia Detoxification Powder).
Yet another patient had an abdominal carbuncle where the pus was ripe but rupture was delayed, with a faint and fine pulse. After the pus drained, the exudate from the wound resembled crab bubbles, indicating internal ulceration penetrating the peritoneum. Cases of sores and ulcers penetrating the peritoneum rarely survive; even with strong tonification, rescue is impossible. This serves as a warning against waiting for pus to emerge naturally.
Another patient had a lump inside the abdomen that caused intermittent pain; during severe pain episodes, the patient would lose consciousness and only regain awareness after a long time. Various medicines were ineffective, and the pulse was deep and fine, indicating this was not a sore/ulcer. He Jian stated: Shixiao San (Forget Sorrow Powder) is effective for treating hernia qi and women's blood-q stagnation pain that feels like death. One dose was given, reducing pain by sixty to seventy percent, and subsequent doses brought relief. This medicine is particularly excellent for treating postpartum colic and Eryuzhen Tong (Child Pillow Pain / Postpartum Lochi Stasis Pain).
Another patient, who habitually indulged in alcohol and sexual activity, developed a swelling toxin in the lower abdomen. The pulse was weak, with slight pain, and the patient desired internal resolution (dissolution without suppuration). I advised: One should strengthen the stomach and boost Qi, assisted by herbs that promote circulation and invigorate blood, which could lead to resolution. The patient, seeking rapid results, self-administered detoxifying medicines such as Baidu drugs, causing the condition to worsen significantly. The sore did not suppurate, and appetite decreased. More than two months later, upon re-examination, the pulse became weaker, night sweats persisted, the swelling remained aggregated without rupturing, the muscles felt cold, spontaneous sweating occurred, and the complexion was pale and depleted. This indicated deficiency of both Qi and Blood, preventing the formation of pus. Tenquan Dabu Tang (Complete Great Tonifying Decoction) was administered for over thirty doses; once pus formed, it was lanced. However, irritability and a large pulse appeared, indicating Yang collapse. Shengyu Tang (Sage Preservation Decoction) was given for three doses, and the patient recovered after taking Congtang (Green Onion Decoction) more than a hundred times.
