Pulse Diagnosis
The chi pulse (pulse position corresponding to the kidney and reproductive system) in women is usually vigorous, and a larger pulse on the right hand are all normal phenomena. If the kidney pulse is slightly choppy, or floating or slippery, with an irregular and interrupted rhythm; or if the liver pulse is deep and rapid, these are signs of amenorrhea and menstrual irregularity.
Symptoms
Menarche occurs around age fourteen in girls, and seminal emission around age sixteen in boys; this conforms to the numbers of Yin and Yang and occurs at the appropriate time. Therefore, men's essence should be abundant, and women's menstruation should be regulated. The key to regulating menstruation lies in suppressing excessive Qi to promote blood circulation, where abundant blood and deficient Qi constitute compliance (shun); compliance prevents myriad diseases and enables conception. Moreover, women's diseases caused by seasonal external pathogens or the Six Evils (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and Seven Emotions (Joy, Anger, Worry, Pensiveness, Sadness, Fear, Fright) are treated similarly to those of men. Only conditions during pregnancy and postpartum, seven types of masses and eight types of accumulations (general terms for abdominal masses), metrorrhagia and metrostaxis (profuse or prolonged non-menstrual bleeding), and leukorrhea differ, so separate formulas are provided. Investigating the causes, most stem from menstrual irregularities generating various syndromes; generally, having menstruation arrive on schedule is paramount. If delayed, one must examine whether it is due to Heat or Cold and regulate accordingly. Menstruation arriving early indicates Blood Heat, treated by clearing Heat; menstruation arriving late indicates Blood Cold, treated by warming the channels. However, one must not fail to check for external pathogenic invasion and distinguish Cold or Heat before administering medication. During menstruation, as in the postpartum period, improper care can lead to significant pathology. If startled, Qi and Blood become chaotic, meridians gradually cease to flow; reversing upward causes bleeding from the mouth or nose, while reversing within the body leads to blood-stasis consumptive disease. Excessive labor during menstruation generates deficiency Heat, becoming the root of pain. Emotional extremes cause Qi reversal; Qi reversal leads to Blood reversal, stagnating in the waist, legs, heart, abdomen, back, and flanks. During menstruation, this results in heavy pain. If symptoms subside after delay, but extreme anger injures the Liver, dizziness and vomiting may occur. Combined with meridian leakage, this leads to continuous dripping of blood from orifices. At such times, contracting wind causes wind diseases, exposure to cold causes cold diseases; if untreated for long, metrorrhagia, leukorrhea, and abdominal masses will inevitably follow.
Diagnosis 1: A floating and long cun pulse (pulse position corresponding to the Heart and Lungs) on the right hand of a woman, extending beyond the thenar eminence (the fleshy part at the base of the thumb), indicates excess Qi. This is often because women tend to overthink and harbor jealousy; when things do not go as wished, they become depressed. Accumulated resentment prevents Qi from dissipating, causing Blood to deplete daily while Qi becomes excessive. Yin and Yang contend, causing alternating chills and fever, reduced appetite, emaciation, and a swarm of diseases. Yueju Wan is indicated. This pulse pattern is commonly seen in nuns, widows, long-term unmarried women, wives of scholars and merchants, and frustrated women.
Diagnosis 2: A wiry Jueyin Liver pulse emerging from the cun position and reaching the thenar eminence indicates excess Yin. This arises from longing for a man who cannot be attained. Generally, men rely on Essence; abundant Essence drives sexual desire. Women rely on Blood; abundant Blood facilitates pregnancy. Thus, a wiry Liver pulse emerging from the cun indicates Yin excess.
