变生诸症。有变为牙疳口臭,宣露出血者;有变为头面肿大,口鼻溃烂者;有变为一切疮毒,流脓出血者;

Pulse
A deep and fine pulse indicates Pi Ji (accumulation of masses in the abdomen that are fixed and immovable).
Etiology and Pathogenesis
The Qi and fortune of Heaven and Earth naturally differ between the North and South. Diseases in children also vary by region. In the North, eight or nine out of ten children suffer from Pi Ji; in the South, fewer than two or three out of a hundred suffer from Pi disease. What causes this? It is likely because the water and soil of the South are thin and weak, the diet is soft, and digestion is easy, so such diseases are rare. In the North, the water and soil are dense and solid, the food (wheat noodles) is hard, and digestion is difficult, so such diseases arise. This is also due to the disharmony of the Spleen and Stomach. Therefore, Li Dongyuan considered the Spleen and Stomach as the foundation of the human body. When the Spleen and Stomach are harmonious, no disease arises; when they are deficient, hundreds of diseases emerge. Children's Spleen and Stomach are inherently fragile, and their Zang-Fu organs are tender. Mothers often do not know the methods of care and nursing, indulging in excessive affection without regard for salty, sour, sweet, or rich flavors, nor for raw fruits, cold foods, zongzi, damp wheat noodles, greasy fried foods, or other sticky, dry, and hard-to-digest items. They satisfy the child's desires, leading to overeating, which damages the Spleen and Stomach. Once damaged, the Spleen and Stomach cannot digest food and fluids. Undigested food and fluids stagnate and generate Phlegm. Prolonged fever consumes Yuan Qi (Original Qi). When Yuan Qi is deficient, it cannot move Blood, causing Blood to stagnate and not disperse. It remains between the hypochondrium and ribs, eventually forming blood clots located between the skin and fascia, immovable. Initially, they are the size of a coin. With fever, they grow daily, taking shapes like turtles, snakes, pig liver, or lungs, varying in length and size. Inside, there are blood holes connecting them; outside, blood vessels coil and fix them. These vessels extend straight down to below the spine, opposite the umbilicus, where arterial pulsation occurs—this is the root of Pi disease. The blood vessels of the human body circulate day and night without end. When Qi and Blood flow to this area, Blood enters the vessels, then into the holes, and finally into the Pi mass. The Pi mass grows gradually by nourishment from Blood, and the pathogen strengthens with the assistance of Blood. Thus, Righteous Qi becomes more exhausted, and Blood becomes more depleted, manifesting as tidal fever and leading to various diseases. Symptoms may include spontaneous sweating on the head, throbbing in the chest or neck, abdominal distension with visible blue veins, dry and brittle hair, yellowish complexion with emaciation, and dry limbs. If prolonged, toxic factors emerge, causing diverse symptoms: some develop into dental gangrene with bad breath and bleeding gums; some into swelling of the head and face with ulceration of the mouth and nose; some into various sores and toxins with pus and bleeding; some into limb edema, abdominal distension, and asthma; some into alternating chills and fever resembling malaria but not quite; some into phlegm cough, wheezing, heat, epistaxis, hematemesis, and hematochezia; some into vomiting, diarrhea, dysentery, rectal prolapse, and bearing-down sensation; some into heart and abdominal pain, and hernia with unilateral descent. All these are caused by the attack and eruption of Pi toxin. There are numerous variations, difficult to list completely. This is a critical illness with nine deaths for every one survival, not curable by a single formula or method.
Treatment Principles
Treatment should first employ acupuncture and moxibustion to cut off the root, preventing Blood from entering the vessels, so that the Pi mass, deprived of Blood nourishment, will self-destruct. Externally apply plasters; internally administer decoctions and pills to attack the pathogen. Generally, tonify the Spleen and nourish Qi to treat the root, clear Heat and dissipate masses to treat the branch, addressing both simultaneously. One must weigh the situation according to the child's strength and the severity of the disease. If strong and mild, use more branch-treating herbs than root-tonifying ones; if weak and severe, use more root-tonifying herbs than branch-treating ones. Formulas such as Fei Er Wan (Child-Nourishing Pill), Wu Jin Wan (Black Gold Pill), A Wei Wan (Asafoetida Pill), and Qian Jin Bao Tong Wan (Thousand-Gold Child-Protecting Pill) are half-attacking, half-tonifying, and mild formulas. They should be selected according to the syndrome. After acupuncture, they can achieve complete success. I believe this disease warrants detailed study.
