Zhao Yangkui stated: The Classics state that all panting (asthma) belongs to the Upper Jiao; it is also said that all rebellious Qi rushing upward belongs to Fire. Therefore, Liu Hejian discussed panting under the category of heat diseases, Hua Tuo said that excess Lung Qi causes panting, and the *Ren Shu* (Book of Life) stated that excess Qi leads to panting. Later generations compiled formulas based on symptoms, merely following these views. Only Wang Haizang differentiated this, stating that "excess Qi" should be understood as Qi deficiency, and "excess" should be recognized as insufficiency. If Lung Qi were truly in excess, clear Qi would descend naturally, how could it result in panting? It is because Fire pathogen enters the Lung, scorching True Yin, leading to weakness and insufficiency, thereby causing panting. Wang Haizang's differentiation surpassed predecessors, but unfortunately, he did not fully explore the origin of the Fire. I believe that excess Fire stems from insufficient Water; excess Yang stems from insufficient Yin. All rebellious Fire rushing upward originates from the Ministerial Fire of the Chong and Ren vessels in the Lower Jiao, arising from the Liver and Kidney. When Kidney Water is deficient and weak, Ministerial Fire becomes hyperactive, consuming Qi through vigorous Fire, and melting Lung Metal, how can one not pant?
Danxi stated: There is a type of panting due to Yin deficiency, where one feels Fire rising from the lower abdomen upward. One should use Si Wu Tang (Four Substances Decoction) with Qing Dai (Indigo Naturalis), Zhu Li (Bamboo Sap), and Chen Pi (Tangerine Peel), decocted with Tong Bian (Child's Urine). If accompanied by Phlegm, use Si Wu Tang with Zhi Ke (Aurantium Fructus) and Ban Xia (Pinellia Rhizome), tonifying Yin to resolve Phlegm. Danxi was the first to propose panting due to Yin deficiency, which was an insight into issues previously unaddressed by predecessors. However, his treatment methods have led to detrimental practices among later practitioners. For what is termed Yin deficiency refers to the True Yin within the Kidney, not merely the Yin Blood supplemented by Si Wu Tang. The Fire rising is the Dragon-Lei Fire of the Lower Jiao, which cannot be subdued by cold and bitter herbs. Among cases, some have Phlegm, some do not. In those with Phlegm, it is Water carrying Wood-Fire ascending; can Zhu Li, Zhi Ke, and Ban Xia resolve this? One must use Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) plus Mai Dong (Ophiopogon Radix) and Wu Wei Zi (Schisandra Fruit), decocted in large doses to strengthen the Master of Water (Kidney Water). Then Water ascends and Fire descends, and panting will subside.
There is another category that resembles Fire but is not Fire, and resembles panting but is not panting. The Classics state: When Shaoyin is affected, vomiting, coughing, and rebellious Qi occur. Panting occurs when Yin Qi is below and Yang Qi is above; various Yang Qi floats without root or return, causing upper body panting. The Yellow Emperor's *Zhen Jing* (Needle Classic) states: Disharmony in the stomach collaterals causes panting due to rebellion of Yangming Qi. Yangming Qi should descend, but now rebels and ascends, causing panting. Consumption of True Yuan causes panting due to rebellion of Kidney Qi; this is not Lung panting, but Qi failing to return to its source. External symptoms may include cold limbs, red face, irritability, and aversion to heat. This is not Fire, but the True Yuan Fire of the Mingmen leaving its palace and failing to return. Pulse examination reveals floating, large, and rapid pulses at both Cun positions, but faint and powerless pulses at both Chi positions, or pulses that seem present yet are elusive. Those unaware, believing it to be Fire, use cooling herbs to clear heat. Due to severe panting difficult to bear, they assist with Si Mo Tang (Four-Mortar Decoction) to widen the chest. Little do they know that widening by one part consumes another part of Qi. If cold and Qi-moving agents are repeatedly administered, death is near. Only skilled physicians can trace the origin and use herbs that assist Yuan, connect True Qi, and anchor/subdue, allowing the body to return to its origin. Recovery may be possible, but strong tonification must be avoided. Initially, use Ba Wei Wan (Eight-Ingredient Pill), An Shen Wan (Peace the Kidney Pill), or Yang Zheng Dan (Nourish Righteousness Pellet), taken with decoctions of Ren Shen Sheng Mai San (Ginseng Generating the Pulse Powder). Once Qi stabilizes, then use tonics like Ren Shen (Ginseng), Po Gu Zhi (Cibotii Rhizoma), E Jiao (Donkey-hide Gelatin), Niu Xi (Achyranthes Root) to anchor downward. Further, make pills from Ba Wei Wan plus He Che Da Zao (Placenta Hominis), taken when hungry. However, one must abstain from sexual activity and desires to preserve life; otherwise, death is inevitable.
Another type is Fire Stagnation syndrome. The six pulses are all choppy, even deep and hidden. Limbs are completely cold, even cold to the extremities. Breath is short and panting, seemingly excessive. Attempting to treat as Yin deficiency, pressing the Chi position reveals a drum-like pulse. This is due to long-standing stagnation, where Yang Qi is suppressed and cannot circulate to the exterior, resulting in cold body, faint pulse, chest oppression, and urgent panting. Cold purgatives should not be used, nor hot tonics. Instead, use Xiao Yao San (Free and Easy Wanderer Powder) plus Zhu Yu (Evodia Fruit) and Lian Zi (Lotus Plumule) to ventilate and disperse accumulated heat. Recovery comes with sweating. This is called "Ventilate Fire Stagnation, Reach Wood Stagnation," consistent with the *Jin Gui* saying that for deep-hidden pulses, dispersion resolves heat and stops panting. Afterwards, using Liu Wei Di Huang Wan to nourish Yin and harmonize Yang is appropriate. The above details examples of panting due to Yin deficiency. Regarding panting due to Yang deficiency, Dongyuan has elaborated extensively; regarding external contraction-induced panting, Zhongjing has elaborated extensively.
Wang Jiezhai stated: Panting and distension are causally related, always accompanied by difficult urination. Panting leads to distension, and distension leads to panting. One must identify the root and branch, and the sequence: Panting preceding distension indicates the Lung as the primary organ; distension preceding panting indicates the Spleen as the primary organ. Why? Lung Metal governs descending, and externally connects to skin and hair. The Classics state: The Lung directs all vessels, regulates water passages, and transports fluids downward to the Bladder. It is also said: The Bladder is the official of the capital, storing fluids; only through Qi transformation can they be excreted. Thus, urination depends on the descending and transforming function of Lung Qi. If the Lung is invaded by pathogens and pants, losing its descending command, urine becomes scant, leading to water overflowing into the skin and causing edema and fullness. Here, panting is the root, and distension is the branch. Treatment should focus on clearing Metal and descending Fire, with promoting urination as secondary.
Spleen Earth dislikes dampness and externally governs muscles; Earth controls Water. If Spleen Earth is injured and cannot control Water, dampness runs wild, soaking the muscles. As water ascends, pathogens invade the Lung, preventing Qi descent and causing panting. Here, distension is the root, and panting is the branch. Treatment should focus on strengthening the Spleen and promoting urination, with clearing Metal as secondary. If Lung disease is treated with drying Spleen herbs, Lung Metal becomes dry and panting worsens; if Spleen disease is treated with clearing Metal herbs, Spleen Earth becomes cold and distension worsens. Modern practitioners treating panting and distension only promote urination without distinguishing between Spleen and Lung, so I clarify this point.
Li Shicai stated: The *Neijing* discusses many causes of panting, ultimately revolving around Fire rebelling upward and Qi failing to descend. Danxi said: Excess Fire can be drained, Deficient Fire can be tonified. Yet laypeople encountering panting patients purely use Qi-breaking methods, unaware that nine out of ten deaths from panting are due to deficiency, while only one out of ten is due to excess. Excess cases respond quickly to attack and are easy to treat; deficiency cases require slow tonification and take time to succeed. The progression and regression of symptoms are difficult to manage. Therefore, differentiation is urgent, especially for panting. Gu Chaoshi and Yan Shi only mentioned excess heat. Only Wang Haizang proposed Lung Qi deficiency and Lung Fire excess, offering new insights. Unfortunately, he did not provide detailed prescriptions, creating rigid patterns for later generations. Allow me to elaborate.
For Qi deficiency with Fire entering the Lung, tonify Qi first: Liu Jun Zi Tang (Six-Gentleman Decoction) or Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). For Yin deficiency with Fire attacking Metal, strengthen Water urgently: Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia). For Wind-Cold, release the pathogen: San Ao Tang (Three Unbinding Decoction) or Hua Gai San (Flower Canopy Powder). For Dampness, promote urination: Shen Shi Tang (Dampness Percolating Decoction). For Summerheat, clear vexation: Bai Hu Tang (White Tiger Decoction) or Xiang Ru Yin (Elsholtzia Drink). For Lung Heat, clear the upper burner: Er Dong (Winter and Summer Anemones), Er Mu (Fritillaria and Adenophora), Gan Jiang (Licorice and Platycodon), Zhi Zi (Gardenia), Huang Qin (Scutellaria). For Phlegm obstruction, resolve it: Er Chen Tang (Two-Aged Decoction). For Qi stagnation, soothe it: Si Qi Tang (Seven-Word Decoction). For Fluid retention, induce vomiting; if ineffective, use Bai Zhu Fang Ji Tang (Atractylodes and Stephania Decoction). For Excess Fire, clear it: Bai Hu Tang plus Gua Lou Ren (Trichosanthes Seed), Zhi Ke (Aurantium Fructus), Huang Qin (Scutellaria). For Lung Abscess with panting, protect Metal and transform toxins: Xing Ren (Apricot Seed), Gan Cao Jie (Licorice Joint), Jie Geng (Platycodon Root), Bei Mu (Fritillaria Bulb), Fang Feng (Saposhnikovia Root), Yin Hua (Honeysuckle Flower), Ju Hong (Tangerine Pith), Mai Dong (Ophiopogon Radix). For Lung Distension with panting, promote urination and scatter pathogens. Symptoms of Lung Distension include panting, irritability, eyes protruding, and floating-large pulse: Yue Bi Jia Ban Xia Tang (Yuebi Decoction plus Pinellia); if pulse is floating and there is fluid under the heart, use Xiao Qing Long Tang (Minor Green Dragon Decoction) plus Shi Gao (Gypsum). For Kidney deficiency with Fire not returning to its channel, guide the Dragon to the sea: Ba Wei Wan (Eight-Ingredient Pill). For Kidney deficiency with flooding Water evil, drain water downward: Jin Gui Shen Qi Wan (Kidney Qi Pill from the Golden Cabinet).
Xue Lizhai stated: For panting due to latent Heat in the Lung failing to generate Water, causing panting and thirst, use Huang Qin Qing Fei Yin (Scutellaria Clearing Lung Decoction) to treat the Lung, and Wu Lin San (Five-Difficulty Powder) to clear the bladder. For Lung-Spleen deficiency failing to regulate water passages, use Bu Zhong Yi Qi Tang to cultivate Original Qi, and Liu Wei Di Huang Wan to tonify Kidney Water. For panting due to accumulation of Heat in Lung-Spleen from rich foods, use Qing Wei San (Clear Stomach Powder) to treat the Stomach, and Zi Shui Wan (Nourish Water Pill) to promote urination. for Heart Fire overcoming Lung Metal failing to generate Kidney Water, use Ren Shen Ping Fei San (Ginseng Calming Lung Powder) to treat the Lung, and Zi Shui Wan to nourish the bladder. For Kidney Yin deficiency with Fire scorching Lung Metal and no urine production, use Liu Wei Di Huang Wan to tonify Kidney Water, and Bu Zhong Yi Qi Tang to cultivate Spleen Earth. For Spleen Qi deficiency failing to control and causing panting, use Bu Zhong Yi Qi Tang to cultivate Original Qi, and Liu Wei Di Huang Wan to generate Kidney Water. for Liver Wood overcoming Spleen Earth failing to control and causing panting, use Liu Jun Zi Tang, Chai Hu (Bupleurum Root), and Sheng Ma (Cimicifuga Rhizome) to cultivate Original Qi, and Liu Wei Wan to tonify the Kidney. For Spleen-Kidney Yang deficiency failing to control and causing panting and distension, use Ba Wei Wan to tonify Lung-Spleen and generate Kidney Water. For Lung-Kidney Yang deficiency failing to regulate water passages and causing panting and distension, use Jin Gui Jia Wei Shen Qi Wan (Modified Kidney Qi Pill from the Golden Cabinet) to tonify Lung-Spleen and generate Kidney Water. For excessive alcohol and sex damaging the three Yin meridians of the foot, leading to panting, distension, surging Phlegm, and irregular urination/defecation, or pain radiating between them, use the aforementioned pills; many survive.
