Generally, overwork leads to accumulated fatigue. The fire of the five emotions (Wuzhi) is prone to ignite. Upper: nasal orifices are blocked. Lower: hemorrhoids and fistulas with bleeding. In winter and spring, warm pathogens arise from the exuberance of Yang Qi. Pathogenic factors take advantage of deficiency to lie dormant inside. The latent pathogen is not like an acute external contraction that can be resolved by dispersing and releasing. Moreover, the body suffers from internal injury due to fatigue and overwork. The illness has persisted for ninety days. The dorsum of the feet swells daily. Bowel movements occur five to six times a day. The stool is sticky in consistency. Its color is yellow-red, purple, and greasy. Urination is difficult and only slightly improves with bowel movements. This indicates that the mechanism of the Kidney gate has failed, and the Yang Fu organs of the two intestines have lost their control. The food and water ingested are not properly transported and transformed by the Spleen and Stomach. Dampness brews and sinks downward, transforming into a form of stasis and putrefaction. Just as Earth Qi is prosperous, entering summer, the Spleen and Stomach govern the Qi. This internal invasion of dampness results from the daily damage to the Spleen and Kidney. If treated by physicians who do not understand the principles, one mistake after another will inevitably lead to abdominal fullness. The slow and choppy pulse on the left reflects the damage to Yin and Yang in chronic illness, which is reasonable. However, the wiry and large pulse on the right—could it be due to stagnation of excess physical substances? As Zhang Zhongjing stated, "Wiry indicates a decrease in Stomach Qi; Large indicates the progression of disease." It also implies the gradual relaxation of the Yangming meridians, consistent with the principle that swelling rises from below upward. Focusing solely on regulating the middle Jiao hinders digestion and causes fullness. The general principle should be to promote circulation in the middle and separate clear from turbid in the lower. Only when urination becomes free and the opening and closing of the Fu organs function normally can there be a turning point. If it continues for another month, when Yin generates at the Summer Solstice, it will be difficult to reverse.

Three types of malaria were attempted to be intercepted but were ineffective. It must be that the Yin Zang organs are affected. Epistaxis, heat, thirst, undigested food intake, painful diarrhea. These two conditions seem contradictory. Considering the illness has prolonged, and dietary restrictions were ignored, forcing food intake during illness cannot nourish the spirit but instead causes stagnation and turbidity blocking the collaterals. First, regulate with formulas for distension and diarrhea, do not confuse it with malaria.

Mental labor and worry, irregular eating times, injury from hunger and fullness, Spleen and Stomach are diseased. Spleen loses its transportive transformation. Night belongs to Yin darkness. Until dawn, profuse sticky diarrhea occurs. Food is unappetizing. Weak Spleen dislikes soft and turbid flavors. Wuling San opens the Bladder, separating and draining dampness. It has already exited through the anterior yin orifice. Using it yielded slight effect. Dongyuan said, "When Middle Qi is insufficient, urination and defecation change. When Yang does not circulate, dampness often forms the Five Diarrheas."

Pulse is deep and slow. Muscles are abundant. This is the constitution of Water and Earth. Yang Qi is insufficient for circulation. Water and grain accumulate dampness, spreading to the collaterals. The Lower Jiao often has heavy and painful sensations. If food is slightly not transported, diarrhea easily occurs. Menstrual blood is pale. Water and dampness mix. Generally, it concerns the Taiyin Spleen Zang. If regulation does not hit the mark, fear of abdominal distension disease arises.

Morning diarrhea is unbearable. Evening is slightly peaceful. Prone to hunger and good appetite, yet still difficult to digest. The reason lies in the disharmony of Yin and Yang between Spleen and Stomach. Reading Dongyuan's Treatise on the Spleen and Stomach, it states, "Spleen should ascend to be healthy; Stomach should descend to be harmonious." Citing ascending and descending as the method.

Able to eat but unable to digest. Abdominal pain and diarrhea. If wind-cold attacks externally, and muscles touch cold, the disease arrives instantly. Last year, Gao Shi Anshen Wan was used. Initially suitable, but ineffective after two weeks. Knowing the disease is in the Middle Jiao, no need to secure the lower. Self-reported walking dozens of li without feeling fatigued. Pain surrounds the umbilicus. Proposing to use Zhizhong method. Foot-Taiyin and Yangming govern this.
