White Swollen Tongue
This condition is caused by the accumulation and stagnation of wind-cold pathogens within the interior. The pulse at all six positions (cun, guan, chi on both hands) is wiry and tight (Xian Jin), and the tongue is swollen, stiff, and painful. Initially, treat with Liuwei Tang (Six-Ingredient Decoction) added with: Xixin (Asari Radix, 3 fen), Suye (Perilla Leaf, 1.5 qian), Baizhi (Angelicae Dahuricae Radix, 1 qian), Danggui (Angelicae Sinensis Radix, 1.5 qian), Chuanxiong (Ligustici Chuanxiong Rhizoma, 1 qian), and Gegen (Puerariae Lobatae Radix, 1 qian). If there are black spots on the white coating that appear slippery and moist, decoct and administer Dan Fuzi (Prepared Aconiti Lateralis Radix) and Ganjiang (Zingiberis Rhizoma, each 5 fen). Subsequently, grind Ganjiang (Zingiberis Rhizoma), Bingpian (Borneolum Syntheticum), Shexiang (Moschus), and Qingpi (Citri Reticulatae Pericarpium Viride) in equal parts into a fine powder, and apply it to the tongue frequently until recovery.
Rotting Edge Tongue
This condition results from damp-heat turbidity in the Spleen system. The tongue is enlarged, with its margins developing canker sores (Gan), characterized by white spots and ulceration. Treat with Liuwei Tang (Six-Ingredient Decoction) added with: Xiaoshengdi (Rehmanniae Radix Recens, 2 qian), Huashi (Talcum, 3 qian), Dan Zhuyez (Lophatheri Herba, 1 qian), Yirenmi (Coicis Semen, 1.5 qian), Zhuling (Polyporus, 1.5 qian), Zexie (Alismatis Rhizoma, 1 qian), Cheqian (Plantaginis Semen, 1 qian), and Gancao Shao (Glycyrrhizae Radix et Rhizoma, tip portion, 1 qian). Recovery is expected after two doses. For swelling and ulceration of the lips and gingiva, the same treatment method applies.
Red-Dotted Purple Tongue
This condition is caused by extreme heat in the Heart and Spleen meridians. The entire mouth presents with red dots turning purple, accompanied by ulceration and pain, or red macules appearing on the body. Treat with Liuwei Tang (Six-Ingredient Decoction) added with: Shu Shigao (Gypsum Fibrosum, 1 liang), Gegen (Puerariae Lobatae Radix, 1.5 qian), Chuanlian (Coptidis Rhizoma, 1 qian), Qingdai (Indigo Naturalis, 1 qian), Huangqin (Scutellariae Radix, wine-fried 2 qian), Huangbai (Phellodendri Cortex, 1 qian), Mutong (Akebiae Caulis, 1 qian), and Shanzhi (Gardeniae Fructus, 1 qian). In severe cases, add Dahuang (Rhei Radix et Rhizoma, raw, 3 qian). If the pulse at all six positions is not rapid (Shu), do not use this prescription.
Pure Purple Tongue
This condition arises because patients with Shanghan (Cold Damage Disease) used scallion and wine to induce sweating, allowing wine toxicity to enter the Heart, resulting in a tongue that is entirely purple. It is appropriate to use Shengma ( Cimicifugae Rhizoma, 1 qian), Gegen (Puerariae Lobatae Radix, 1 qian), Zhizi (Callicarbis Fructus, 2 qian), Shigao (Gypsum Fibrosum, 2 qian), Chuanlian (Coptidis Rhizoma, 1.5 qian), Huashi (Talcum, 3 qian), Mutong (Akebiae Caulis, 1 qian), and Renzhonghuang (Nitrum, 3 qian). If there is mental restlessness and irritability, add Shanzhi (Gardeniae Fructus, 1 qian) and Danshichi (Sojae Semen Praeparatum, 1 qian). If there is nausea and a desire to vomit, fearing the emergence of macules, add Yuansui (Coriandri Fructus, 1 liang). Externally, it is excellent to rinse Coriandri Fructus (Yuansui) with liquor and wipe the back.
