The "Four Characters" elucidated in the Upper Volume are the general principles of throat disease treatment; their flexible application depends on the physician. Herein, the names, syndromes, and treatments of twenty-two important types of Hou Feng (Throat Wind) are listed as follows.
When the pathogenic wind is inside the throat, a mass forms on one side. After two or three days, chills and fever occur, and swallowing becomes impossible. First, administer Fangfeng Xiaodu San (Saposhnikovia Decoction for Dispelling Toxins) for one or two doses. If symptoms do not subside, use acupuncture until bleeding stops, then remove the needle. Haozhu Jian Xiaodu San (Haozhu Arrow Decoction for Dispelling Toxins) can be used; even if residual symptoms remain, it may be taken. Alternatively, Yancao Gen (Saltwort Root, also known as Yanzao) or Aihé Gen (Dwarf Lotus Root, also known as Liangshan Tree Root) can be held in the mouth for treatment. (Bingzhang notes: This syndrome must involve accumulated phlegm due to constrained fire. Herbs such as Qianghuo, Fangfeng, Shengma, Jiegeng, Chuanxiong, and Banxia should all be contraindicated and avoided. Instead, add Yuanshen (Scrophulariae Radix), Chuanbeimu (Fritillariae Cirrhosae Bulbus), Kunbu (Eckloniae Thallus), and Haizao (Sargassum Thallus) to soften hardness and resolve phlegm.)
When the pathogenic wind is inside the throat, masses form on both sides, making swallowing difficult. Accompanied by wind-heat, restlessness, and dry mouth, holding Yancang Gen, Aihé Gen, and raw Danfan (Alumen) in the mouth yields immediate effect. (Bingzhang notes: Internal administration of Yangyin Qingfei Tang (Nourish Yin and Clear Lung Decoction) and similar formulas is also required.)
When the pathogenic wind is inside the throat, there is swelling and fullness, though not severe. Three or four blood vessels appear, resembling cotton threads, causing dry mouth and restlessness. This syndrome should present with saliva. First, apply Danfan (Alumen) locally. Internally, administer Shigao Tang (Gypsum Decoction) to clear stomach heat.
In this syndrome, four or five fleshy excrescences grow inside the throat, either on both sides of the throat or on the tongue, resembling pine nuts, making swallowing impossible. First, insufflate Shenyaomo (Divine Medicinal Powder). After several applications, puncture the area to release blood. If six or seven excrescences grow, the condition is incurable.
When the pathogenic wind is on the right side, the face swells, the jaw is stiff, eating and drinking are impossible, and headache with chills and fever occurs. Acupuncture can be used, and Jinyin Er Xiao Dan (Gold and Silver Two Elimination Powder, i.e., Jin Suoshi and Yin Suoshi) can be insufflated. This syndrome is difficult to cure. (Bingzhang notes: It is appropriate to internally administer formulas that dispel wind, reduce swelling, resolve phlegm, and clear fire.)
