[Appendix: Differentiation and Prescription for Deficient Heat Throat Disorder]
In the initial stage of deficient heat throat disorder, the progression is not rapid but gradual. The patient feels dryness when swallowing saliva and slight obstruction when swallowing air. There are no white plaques resembling goose-mouth disease (鹅口疮) nor obvious swelling in the throat; however, the entire throat may appear red or purplish. This is caused by the upward rush of Mingmen Xianghuo (Ministerial Fire residing in the Gate of Life, which becomes deficient fire pathologically). The syndrome belongs to the depletion of Kidney Water, where Ministerial Fire scorches Kidney Yin, preventing it from descending to control the fire. Consequently, deficient fire rushes to the throat, causing slight pain and obstruction, without aversion to cold but with a preference for heat. At this time, diaoyao (a treatment drug inducing vomiting to expel phlegm by stimulating the throat) should not be used, as it may damage body fluids, doing more harm than good. It is advisable only to hold Shengjin Runfei Wan (Saliva-Generating and Lung-Moistening Pill) in the mouth, swallow it slowly, and take Ziyin Jianghuo Tang (Nourishing Yin and Descending Fire Decoction) for several doses. Acupuncture, diaoyao, and chuiyao (blowing medicinal powder into the throat) are strictly prohibited.
Ziyin Jianghuo Tang (Nourishing Yin and Descending Fire Decoction)
Shengdi Huang (Rehmanniae Radix Preparata, 2 qian) Xuanshen (Scrophulariae Radix, 2 qian) Tiandong (Asparagi Radix, 2 qian) Baishao (Paeoniae Radix Alba, 1 qian) Maidong (Ophiopogonis Radix, 2 qian) Yan Baihuang (Phellodendri Cortex Preparata cum Sale, 1 qian) Jiegeng (Platycodonis Radix, 1 qian) Ku Huangqin (Scutellariae Radix Ustas, 1 qian) Zhizi (Gardeniae Fructus, 7 fen) Gancao (Glycyrrhizae Radix et Rhizoma, 3 fen) Zhimu (Anemarrhenae Rhizoma, 1 qian) Shandougen (Sophorae Tonkinensis Radix et Caulis, 5 fen) Danpi (Moutan Cortex, 1 qian) Zexie (Alismatis Rhizoma, 1 qian) Bohe (Menthae Haplocalycis Herba, 5 fen; omit if spontaneous sweating occurs)
Add two bowls of water, decoct until reduced to eight-tenths, and take on an empty stomach. (Bingzhang's Note: For syndromes of Kidney deficiency with Yin fire flaming upward, it is appropriate to guide heat downward, such as using Jingyue's Yunnv Jian (Jade Woman Decoction) with added Xuanshen, which yields the best results. The Jiegeng in this formula has a lifting action that carries medicine upward, which is most contraindicated for this syndrome.)
Shengjin Runfei Wan (Saliva-Generating and Lung-Moistening Pill)
Pengsha (Borax, 3 qian, half raw and half calcined) Hanshuishi (Calcitum, 2 qian) Shandougen (Sophorae Tonkinensis Radix et Caulis, 2 qian) Wuweizi (Schisandrae Chinensis Fructus, 1 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian) Ku Huangqin (Scutellariae Radix Ustas, 2 qian) Wumei (Mume Fructus, 1 qian) Bohe (Menthae Haplocalycis Herba, 3 qian)
Add Bingpian (Borneolum Syntheticum, 2 fen) to the above herbs, grind all together into a fine powder, form pills with honey, size similar to a longan seed, hold in mouth to dissolve and swallow, to generate saliva and descend fire.
[Appendix: Notes on Acupuncture and Moxibustion]
St acupuncture at Baihui (GV20): one needle. Qian Ding (GV21): one needle, or three needles. Hou Ding (GV19): one needle, or three needles. Jiache (ST6): one needle, or three needles. Needling can be performed on both left and right sides. Fengchi (GB20): one needle, left side for males, right side for females. Shaoshang (LU11): one needle. Hegu (LI4): one needle. Lieque (LU7): one needle. Quchi (LI11): one needle. All follow the principle of left for males, right for females.
[Image: Diagram of proportional measurement for upper limbs]
Guang's Note: Baihui (GV20) is located at the center of the head. Qian Ding (GV21) is 1.5 cun anterior to Baihui. Hou Ding (GV19) is 1.5 cun posterior to Baihui. Jiache (ST6) is below the earlobe. Fengchi (GB20) is in the depression at the hairline, specifically at the lateral end of the lower part of the two large tendons at the back of the neck. Shaoshang (LU11) is at the radial corner of the nail base of the thumb. Hegu (LI4) is in the depression between the bases of the first and second metacarpal bones on the dorsum of the hand; contraindicated in pregnancy. Quchi (LI11) is in the depression lateral to the olecranon process of the humerus; when flexing the elbow towards the chest, it corresponds to the end of the transverse crease. Lieque (LU7) is on the medial aspect of the forearm, 1.5 cun proximal to the wrist crease.
When encountering such disorders, start with Shaoshang (LU11), Hegu (LI4), Lieque (LU7), and Quchi (LI11), needling according to the male-left/female-right principle and standard techniques. If the condition is severe, begin needling Qian Ding (GV21), Baihui (GV20), Hou Ding (GV19), Fengchi (GB20), and Jiache (ST6) to unblock the meridians throughout the body, allowing wind-heat pathogenic factors to dissipate and blood and qi to flow smoothly. Combined with internal administration of special medicines, recovery will naturally be achieved. If no blood appears after needling, it indicates severe congestion of wind-heat and deep pathogen invasion, often leading to untreatable conditions.
Before needling, use the thumbnail of the left hand to press firmly on the acupoint to open the flow of qi and blood. Instruct the patient to focus their mind inward and avoid distraction before inserting the needle, so as not to injure Ying (Nutritive) and Wei (Defensive) qi.
When needling, upon reaching the acupoint and hitting the pathological site, withdraw the needle quickly. Immediately press the needle hole with the thumb of the left hand to prevent bleeding; this is called the tonification method. When withdrawing the needle, pull it out slowly without pressing the needle hole, allowing bleeding; this is called the sedation method. Generally, excess syndromes are treated with sedation, deficiency syndromes with tonification, or a combination of sedation followed by tonification, depending on the symptoms.
Although acupuncture and moxibustion for Houfeng (Throat Wind) can cure the root cause and prevent recurrence, patients who do not abstain from fried, spicy, and heat-toxic foods may experience recurrence after one or two years. This must be known. Therefore, dietary restrictions are necessary after needling to avoid future complications. Before needling, blow Sanfeng medicinal powder into the throat first, then proceed with needling. After needling, seal the needle sites with medicinal powder. If needling does not resolve the condition after blowing medicine, use diaoyao (vomiting-inducing medicine) blown into the throat.
Sanfeng Powder Formula (For blowing into throat and sealing needle sites)
Quanxie (Scorpio, 6 fen, washed clean, remove head and feet, processed with child's urine, weighed accurately) Caowu (Aconiti Kusnezoffii Radix Preparata, 1 qian, remove rhizome and process) Bohe (Menthae Haplocalycis Herba, 1.5 qian)
Grind these three ingredients into powder. Separately, grind extremely finely in a mortar, mix with Qianjin Pi Xiao San (Qianjin Pi Xiao San, 1 qian), add Bingpian (Borneolum Syntheticum, 1 fen) and Shexiang (Moschus, 5 li).
Diaoyao Formula
Eluecao (Radix Iridis Tectoriae, also known as Jiandaohao Gen) Shandahuang (Radix Rumis, also known as Shuituisha Gen) Yenanxing (Rhizoma Lycoris Radiatae, also known as Shisuan Tou)
Grind these three ingredients in water, swallow, and induce immediate vomiting. The expulsion of phlegm from the diaphragm contains the meaning of dispersing. Dispersal leads to sweating, thus wind exits with sweat.
Guang's Note: Eluecao is not recorded in Materia Medica. It is suspected to be Ebao (Iris tectorii), pending verification. Recently, "Health Gazette" introduced Tianmingjing (Flos Vernoniae Cinereae) for treating Houbi (Throat Bi) with swelling and pain, showing excellent efficacy in inducing vomiting of phlegm. Li Shizhen, a former expert, also praised its effects. Shandahuang is named Suanmo (Rumex acetosella) in Materia Medica, sour and cold in nature, kills parasites and treats scabies. Yenanxing is Shisuan (Lycoris radiata), pungent and warm; Materia Medica states it effectively induces vomiting and sweating.
